Real-time ultrasound evaluation of tuberculous pleural effusions.

An evaluation of 21 patients with tuberculous pleurisy was carried out to assess the role of ultrasonography in the diagnosis of tuberculous pleural effusion. Ultrasonography revealed winding structures of different lengths and thicknesses (winding bands) in 8 patients, and filiform structures of a higher echogenicity and a shorter length (linear echoes) in 13. These findings were associated with exudates having a high content of fibrin and protein, respectively, suggesting that winding bands might be formed by fibrin and linear echoes by protein macroaggregates. Based upon these observations,

Pulmonary rehabilitation of a 72-year-old male with tracheostomy combined with unilateral tuberculous pleural effusion after cerebral infarction: A case report and literature review.

INTRODUCTION: While poststroke rehabilitation primarily addresses motor, linguistic, cognitive, and swallowing impairments, pulmonary dysfunction (PD) is frequently neglected. PD following stroke, attributed to cortical-diaphragm pathway damage, can lead to increased mortality and prolonged hospitalization. Tracheostomy in such patients can exacerbate PD by increasing airway resistance and the risk of respiratory infections. This case study aims to report the successful integration of early pulmonary rehabilitation (PR) in a high-risk patient with poststroke tracheostomy complicated by unilate

The Value of Adenosine Deaminase 2 in the Detection of Tuberculous Pleural Effusion: A Meta-Analysis and Systematic Review.

Adenosine deaminase 2 (ADA2) is reported as a novel diagnostic biomarker for tuberculous pleural effusion (TPE) in many studies. This meta-analysis was conducted to systematically evaluate the general diagnostic performance of pleural ADA2 in TPE. After searching for relevant studies that investigated the diagnostic performance of pleural ADA2 in TPE in several databases, we assessed and selected eligible studies to calculate pooled parameters by STATA 16.0 software. A final set of thirteen studies entirely met the inclusion standards and were used to calculate pooled parameters in our meta-an

Peripheral Blood Monocyte to Lymphocyte Ratio for Prediction of Tuberculous Pleuritis.

OBJECTIVE: To examine the peripheral monocyte to lymphocyte ratio (ML ratio) of patients with tuberculous (TB) pleuritis and the ML ratio changes after treatment. METHODS: Clinical and laboratory information were collected from patients with lymphocytic exudative pleural effusion admitted to Chiang Mai University Hospital from 2013 to 2019. This study compared the ML ratios between tuberculous pleuritis and other diagnoses in patients who were followed after treatment. RESULTS: A total of 152 patients were included: 57 with tuberculous pleuritis and 95 with other lymphocytic exudates. The ma

Macrophage M1/M2 ratio as a predictor of pleural thickening in patients with tuberculous pleurisy.

We evaluated the association between macrophage polarization and the development of pleural thickening in patients with tuberculous pleurisy. Patients with tuberculous pleurisy admitted to our hospital between October 2018 and March 2019 were prospectively recruited. Pleural fluid samples were obtained before treatment for detection of adenosine deaminase (ADA) and macrophage phenotype (M1: CD14+ CD86+; M2: CD14+ CD163+). Peripheral blood samples were subjected to interferon gamma release assay (IGRA). All subjects were administered standard anti-tuberculosis regimen (2HREZ/4HR); high-resoluti

Accuracy of interleukin-27 assay for the diagnosis of tuberculous pleurisy: A PRISMA-compliant meta-analysis.

BACKGROUND: The concentration of interleukin-27 (IL-27) in pleural effusions was found to be increased in tuberculous pleurisy and several studies have investigated the diagnostic value of IL-27 for tuberculous pleural effusions (TPEs), but the results varied a lot. We conducted the present study to comprehensively evaluate the diagnostic value of IL-27 for TPE. METHODS: Primary diagnostic test studies of IL-27 for TPE was searched and identified from databases. The pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ration, diagnostic odds ratio, and receiver oper

Immunobiology of pleural inflammation: potential implications for pathogenesis, diagnosis and therapy.

Although infectious, inflammatory and neoplastic diseases frequently involve the pleural space and walls, little is known about the immunological and molecular mechanisms underlying pleural disorders. This article provides an overview of recent insights into immunobiological processes likely to play a role in the pathogenesis of pleural disorders. Pleural involvement in certain diseases is associated with the infiltration of a number of different types of immune cells, such as neutrophils, eosinophils or lymphocytes, in various proportions depending on both the course and the aetiology of the

Unusual cause of chest pain: empyema necessitans and tubercular osteomyelitis of the rib in an immunocompetent man.

A 33-year-old man, born in India but resident in the UK for 5 years, presented to the emergency department with a 4-week history of a dry cough and right-sided pleuritic chest pain. He reported systemic features, including fever and unintentional weight loss. His medical history included vitamin D deficiency. He had travelled to India 10 months previously and denied any exposure to tuberculosis (TB). He was an ex-smoker with a 20 pack history. Respiratory examination confirmed decreased air entry of the right lower lobe and stony dullness on percussion. His C reactive protein was 178 mg/L. A c

Left-sided hydrothorax in connection with severe pre-eclampsia. Case reports.

Four cases of left-sided pleural hydrothorax in connection with severe pre-eclampsia are reported. In addition an increased formation of ascitic fluid was observed in the patients. A long-lasting bed rest in the left lateral recumbent position preceded the discovery of the roentgenologically verified pleural changes. Hypoproteinemia caused by the excretion of large amounts of protein in the urine seems to be a partial etiological factor aggravated by the renal retention of sodium and water. Pleural transudate disappeared spontaneously within two weeks from the delivery. In such cases, immediat

Development of Pseudochylothorax in a Patient with Lung Squamous Cell Carcinoma and Rheumatoid Arthritis.

Pseudochylothorax is a rare disease entity. It is characterized by a milky white pleural fluid with a cholesterol/triglyceride ratio >1. According to published reports, most patients had associated diseases, including rheumatoid arthritis and tuberculosis, with only one patient having oropharynx carcinoma associated with lung metastases. Herein, we describe the development of pseudochylothorax in a patient with rheumatoid arthritis and lung squamous cell carcinoma. Cancer cells were confirmed in the pleural fluid, but there was no thoracic duct damage caused by cancer invasion or tuberculosis-
