Provocative testing for cubital tunnel syndrome.

This prospective study evaluated the clinical usefulness of provocative testing in 32 subjects with electrodiagnostically proven cubital tunnel syndrome and 33 control subjects. Four provocative tests were included: Tinel's sign, elbow flexion, pressure provocation, and combined elbow flexion and pressure provocation. The mean age of the control group was 41 years and 46 years for the group with cubital tunnel syndrome. In the control group, provocative tests were rarely positive. In 44 extremities with cubital tunnel syndrome, 31 had a Tinel's sign, 33 had a positive elbow flexion test, 39 ha

Nerve entrapment and reflex sympathetic dystrophy after fractures of the distal radius.

In a series of 542 conservatively-treated fractures of the distal radius 30 patients (5,5%) developed compression neuropathy: of the median nerve (n = 23), of the ulnar nerve (n = 5), and of the radial nerve (n = 2). Three patients required decompression of the carpal tunnel, and one patient's ulnar symptoms were relieved after shortening osteotomy of the ulna. The remaining neuropathies resolved spontaneously over a period of three months. Five patients developed typical reflex sympathetic dystrophy with diffuse pain, loss of hand function, and significant autonomic dysfunction. None of them

Cubital Tunnel Syndrome in Patients with and without Prior Elbow Trauma: A Retrospective, Observational Case Series.

Cubital tunnel syndrome (CuTS) is the second most common peripheral nerve disorder of the upper limb and results from compression at anatomically vulnerable points along the course of the ulnar nerve. A total of 26 consecutive patients with CuTS who underwent surgical treatment by a single surgeon between January 2017 and March 2022 were retrospectively reviewed. Demographic characteristics, history of prior elbow trauma, clinical presentation, electrophysiological findings, surgical findings, and postoperative outcomes were analyzed. Three patients had documented major prior elbow trauma and

Ultrasound-guided minimally invasive cubital tunnel thread release: An experimental study.

INTRODUCTION: Cubital Tunnel Syndrome is characterized by the compression of the ulnar nerve in the cubital tunnel and can cause restrictions to the activities of daily living. Traditional treatment includes conservative methods and, in more severe cases, different types of surgery. In these cases, common side effects of surgery may limit the clinical success. A new alternative could be a minimally invasive Ultrasound-guided (US-guided) thread release of cubital tunnel retinaculum also known as the Osborne's ligament. The aim of this study was to assess the efficacy and safety of ultrasound-gu

Statin Treatment Is Associated With a Higher Risk of Carpal Tunnel Syndrome, But not of Ulnar Nerve Entrapment, in Type 2 Diabetes-A Nationwide Register Study.

BACKGROUND: Individuals with diabetes (DM) are often treated with statins, which have a rare side effect: peripheral neuropathy. We aimed to study whether statin treatment affects the risk of carpal tunnel syndrome (CTS) and ulnar nerve entrapment (UNE) in type 2 DM (T2DM). METHODS: We combined multiple national registers in Sweden to identify individuals diagnosed with CTS or UNE in specialized care 2011-2014. Individuals diagnosed with T2DM within 5 years before CTS or UNE diagnosis were included in the diabetes group. Statin treatment was defined as a prescription within 5 years before bas

Outcome More Than 20 Years After Surgery for Ulnar Nerve Compression at the Elbow.

BACKGROUND: Ulnar nerve entrapment at the elbow is a common neurological entrapment neuropathy. Previous research has shown that surgical treatment can be highly successful. Only a few studies have reported on long-term outcome after surgery. In this study, we report on the long-term follow-up after surgery for ulnar nerve compression at the elbow. METHODS: We performed a long-term follow-up study, more than 20 years after surgery, for ulnar neuropathy at the elbow. We also compared 2 methods: simple decompression and anterior subcutaneous transposition. A short outcome questionnaire, adapted

Perioperative ulnar neuropathy in orthopaedics: association with tilting the patient.

The incidence and causes of perioperative ulnar neuropathy in a prospective series of 203 consecutive patients were examined. Three percent of patients had ulnar neuropathy develop in the entire prospective series. The incidence was 6% in patients having total hip arthroplasty. There was a highly significant association between a tilted body position on the operating table and development of ulnar neuropathy on the contralateral side. This position rotates the arm internally and places the ulnar nerve at risk for direct compression.

Medial epicondylectomy for cubital tunnel syndrome: Outcomes from an 8-year retrospective cohort study.

BACKGROUND: Cubital tunnel syndrome (CuTS) reflects ulnar nerve dysfunction due to compression at the elbow. There is no consensus regarding optimal management, with conflicting evidence in the literature. This study aimed to report the treatment of CuTS with medial epicondylectomy (ME), including clinical outcomes and complication rates. METHODS: A retrospective cohort study of cubital tunnel decompression with medial epicondylectomy was conducted over an 8-year timeframe at a single centre for peripheral nerve surgery. A minimum of 24 weeks of follow-up was required for outcome analysis. Di

[Combined idiopathic forearm entrapment syndromes].

OBJECTIVE: The aim of this study was to investigate the frequency of combined entrapment syndromes of the forearm in a clinical collective with regard to the question of whether this is an especially configured subgroup. METHODS: We included 1 051 patients (747 females aged 52.93 +/- 14.08 years, 304 males aged 52.63 +/- 12.49 years) with clinically and electrophysiologically verified entrapment syndromes (carpal tunnel syndrome n = 957, sulcus ulnaris syndrome n = 143, supinator syndrome n = 12). We excluded patients with symptomatic entrapment syndromes, e. g., post-traumatic or patients wi

Ultrasound-guided Percutaneous Ulnar Nerve Release at the Elbow: A Stepwise Approach.

Cubital tunnel syndrome is the second most common compressive neuropathy in the upper limb. A wide range of surgical techniques have been described, from simple in situ decompression to anterior transposition, both through open and endoscopic approaches. Recent advances in ultrasonography and the development of dedicated instrumentation have enabled the emergence of ultrasound-guided cubital tunnel release as a feasible, effective, minimally invasive alternative. However, concerns regarding potential complications and inconsistent outcomes have limited widespread adoption. The aim of this stud

Evaluation of patients with thoracic outlet syndrome.

This prospective study evaluated 50 patients with thoracic outlet syndrome. Detailed history and pain scale evaluation preceded physical examination, which included provocative tests (positional and compressive) and sensory evaluation (baseline and postprovocative vibration thresholds and two-point discrimination). Only one patient had a positive nerve conduction study/electromyograph at the brachial plexus level. Thirty-two percent of the patients had a compressive anatomic abnormality as seen on a computed tomography scan. Ninety-four percent had positive provocative position and compression

Free Functional Gracilis Muscle Transfer for Severe Pediatric Volkmann Ischemic Contracture After Traditional Splinting of a Supracondylar Humerus Fracture: A Case Report.

Volkmann ischemic contracture is a severe and largely preventable complication of untreated or delayed compartment syndrome. In children, it is most commonly associated with supracondylar humerus fractures, especially when external compression from casts or splints is not promptly recognized. We report the case of a six-and-a-half-year-old right-handed girl who developed severe left upper-limb dysfunction after a closed supracondylar humerus fracture treated in a rural setting with tight traditional splint immobilization, locally known as "Jbira." No fasciotomy was performed during the acute p

Surgical treatment of a tomaculous neuropathy.

Compressive neuropathy of the ulnar nerve at the elbow is the second most common nerve entrapment in the upper limb. Eight possible anatomical points of constriction have been identified. The most common constriction being the intermuscular septum proximally or between the two heads of the flexor carpi ulnaris in the cubital canal distally. Surgical release is successful in 80-90% of cases. Certain rare genetic conditions can predispose susceptible peripheral nerves to similar compressive neuropathies but there is no literature on surgical treatment of such patients. We present a case of hered
