This report describes our experience with a 60 year old male who suffered from a recrudescence of groove pancreatitis. He had been treated by conservative medication therapy by proton pump inhibitor used for therapy of duodenal ulcer, and was in remission. During a follow-up one year later, endoscopy revealed gastric cancer, for which a proximal gastrectomy and vagotomy were performed. The patient continues to remain in remission for the groove pancreatitis. Our experience with the clinical course of this disease, in which treatment for duodenal ulcer was used effectively, offers new insights …

The role of gamma-hydroxybutyric acid (GHBA) and gamma-aminobutyric acid (GABA) in the mechanisms of respiratory arrest after systemic serot … ion, but excluded or significantly reduced the stage of respiratory arrest. The effect of bilateral vagotomy on the respiratory pattern upon serotonin administration was very similar to the action of GHBA. It is suggested that GHBA as a neuromodulator probably participates in the mechanisms of respiratory control after serotonin administration.

BACKGROUND: Highly selective vagotomy and complete circular or partial duodenectomy have been applied to complicated duodenal ulcer for many years. These procedures seem to provide better clinical results than truncal vagotomy and antrectomy.
METHODS: A retrospective analysis was conducted of 120 patients with complicated duodenal ulcer who underwent surgical treatment between 1986 and 1999. Patients with obstruction were treated with either circular complete (17) or partial duodenectomy (3) combined with highly selective vagotomy or truncal vagotomy and antrectomy (37). Those with perforatio…

The acid-producing function of the stomach and the state of its hormonal regulation were assessed on the basis of the immediate and long-term follow-up results obtained after truncal (334 patients) and combined (96 patients) vagotomy for duodenal ulcer. According to the data of aimed endoscopic pH-metry and gastroimpedancemetry the acid level changes in different parts of the stomach and duodenum were the same at the nearest and remote periods as well as the dynamics of the main hormones responsible for the acid production.

The influence of truncal vagotomy (TV), selective vagotomy (SV) and selective proximal vagotomy (SPV) with concomitant pylorus-preserving duodenoplasty (DP) on gastric secretion, stimulated by pentagastrin, was studied in chronic experiment on dogs with gastric fundus fistulas. SPV with pylorus-preserving DP causes the most prominent influence on neurohormonal phase of gastric secretion. After SPV and DP conduction in 345 patients the result was significantly better than after SPV with gastric drainage operation.

Prostaglandins (PG) PGE2, PGF2, cAMP, cGMP were measured in blood plasma and gastric mucosa biopsy specimens from 42 patients with continuously relapsing peptic ulcer of duodenal localization, using radioimmunoassay. In duodenal ulcer, there occurs a significant increase in blood plasma levels of PGE2 and PGF2, while those of PGE2 in gastric mucosa biopsies are lowered. Selective proximal vagotomy is associated with a significant decrease in PGE2 and PGF2 in blood plasma and significant increase in PGE2 in a gastric mucosa biopsy. Cyclic nucleotides in blood plasma showed some tendency to rise…

The biochemical background of ethanol-(ETOH) induced gastric mucosal damage was studied in rats with intact vagus and after acute surgical vagotomy. Observations were carried out on Sprague-Dawley (CFY) strain rats of both sexes. Gastric mucosal lesions were produced by intragastric administration of 1 ml 96% ethanol. Bilateral truncal surgical vagotomy was carried out 30 min before ETOH administration. The number and severity of gastric mucosal lesions was noted 1 h after ETOH administration. Biochemical measurements (gastric mucosal level of ATP, ADP, AMP, cAMP and lactate) were carried out …

The kinetics of somatostatin-producing cells (D-cells) in the rat antral mucosa after truncal vagotomy were studied using double immunostaining for bromodeoxyuridine (BrdU) and somatostatin. Both the concentration of somatostatin and D-cell density in the antral mucosa demonstrated a significant increase on the 3rd day after truncal vagotomy. With the single labeling of BrdU, a few D-cells showed positive immunostaining for BrdU throughout the experimental period in both vagotomized and sham operated rats. With cumulative labeling, the BrdU labeled cells demonstrated a linear increase in an id…

To avoid proximal gastrectomy which destroys the gastroesophageal closing mechanism, modified segmental gastrectomy with vagotomy was performed on 3 patients with gastric ulcers located in the stomach near the gastro-esophageal junction. These were all patients in whom a proximal gastrectomy would usually have been performed. The proximal line of resection did not encroach upon the mucosal rosette being within 1 cm of it following the margin of the ulcer. In each patient, the modified segmental resection of the upper stomach consisted of the surgical removal of a continuous strip of tissue inc…

Lesser curve necrosis is a rare but known complication of proximal gastric vagotomy (PGV). Incidence seems to increase when PGV is combined with fundoplication. In reports of this complication, the necrosis was of various sizes, but generally limited to the lesser curve. We report a 54-year-old patient with hiatus hernia, severe peptic esophagitis, and increased acid output who developed necrosis of the whole intraabdominal esophagus, cardia, and proximal third of the stomach after PGV and fundoplication with division of several short gastric vessels. A proximal gastric resection with intratho…

Analysis of the results of organ-preserving operations in 246 patients with ulcerous gastroduodenal hemorrhages revealed a predisposition to motor-evacuation disorders of gastric performance, which plays an essential role in the individual choice of the operation in patients with ulcerous gastroduodenal hemorrhages. This makes it possible to reduce the number of postvagotomy complications linked with disturbed passage of food in the stomach and duodenum.

During Shay-ulcer formation damages to the barrier of the gastric mucosa develop even before the appearance of macroscopic ulceration. Proximal selective vagotomy prevents these damages. Following pyloric ligation the prostaglandin content of the mucosa changes in parallel with the injuries of the mucosal barrier: TXB2 content of the forestomach increases, while PGF2 alpha content of both the forestomach and the antrum decreases. Following PSV operation the 6-keto-PGF1 alpha content of the mucosa decreases, whereas PGF2 alpha and TXB2 contents exhibit no alteration. As a combined effect of pro…

Choice of draining operations for treatment of duodenal ulcer by vagotomy is greatly responsible for the evacuatory function of the stomach in the early and remote periods after operation. In patients with stenosis of the duodenum, low localization of ulcer and considerable scarring process in the ulcer area pyloroplasty after Finney and gastroduodenoanastomosis after Jaboulay are considered to be most advantageous draining operations of the stomach. If the operations are not possible technically, shortloop gastrojejunostomy is expedient. Pyloroplasty after Heinicke-Mikulicz is admissible in p…

In order to prevent postoperative gastric stasis, we devised and tested the following improvements in the selective gastric vagotomy with antrectomy. (1) The gastroduodenostomy was made so as to have an acute angle to the longitudinal axis of the remnant stomach, and (2) the downward traction being exerted on the remnant stomach by the transverse colon was eliminated by dissecting the greater omentum, and then (3) the posterior wall of the corpus on the lesser curvature side was fixed to the stump of the hepatogastric ligament, and the posterior wall of the corpus on the greater curvature side…
