Of almost 8,000 patients referred for neurological consultation, 6.1 per cent had "dizziness" as a presenting complaint. Dizziness is a nonspecific complaint, used loosely to describe funny feelings in the head or lightheadedness by anxious or depressed patients; or it may mean vertigo-a hallucination of movement of self or surroundings in horizontal, rotatory or vertical direction. An analysis of 400 cases showed the complaint "dizziness" to be functional in about 25 per cent of patients. The cause in the remaining cases varied from epilepsy from cortical lesions, to lesion of the brain stem,…

Benign paroxysmal positional vertigo (BPPV) is a very frequent cause of vertigo. BPPV occurs when there is a pathological presence of otolith debris in the semicircular canals. The history of BPPV is often typical, with recurrent episodes of vertigo lasting less than one minute, triggered by head movements, without other neurological signs. The characteristics of the nystagmus observed during the diagnostic maneuvers of Hallpike and Supine Head Roll test allow the identification of the affected semicircular canal and the choice of the appropriate therapeutic maneuver, which in most cases will …

OBJECTIVES: To develop a simple questionnaire for self-diagnosis of benign paroxysmal positional vertigo (BPPV).
METHODS: We developed a questionnaire that consisted of 6 questions, the first 3 to diagnose BPPV and the next 3 to determine the involved canal and type of BPPV. From 2016 to 2017, 578 patients with dizziness completed the questionnaire before the positional tests, a gold standard for diagnosis of BPPV, at the Dizziness Clinic of Seoul National University Bundang Hospital.
RESULTS: Of the 578 patients, 200 were screened to have BPPV and 378 were screened to have dizziness/vertigo…

OBJECTIVE: We studied the clinical features of benign paroxysmal positional vertigo (BPPV) associated with Meniere's disease.
METHODS: The medical records of 120 patients with BPPV was retrospectively analyzed. Complete otolaryngological, audiological, and neurotological evaluation results were available for all patients, including nystagmography. All patients were diagnosed using the Dix-Hallpike test or roll test and treated with the canalith repositioning procedure. The outcomes were compared among the three groups.
RESULTS: A series of 120 BPPV cases. Results showed that Group A and Grou…

We hypothesize that peripheral vertigo is associated with gastroesophageal reflux disease (GERD). Two mechanisms could be considered – gastric acids may directly irritate the respiratory mucosa and cause inflammation, or Helicobacter pylori (H. pylori) could be present and cause local infection. Reflux material (Hydrochloric acid (HCl) and pepsin) could get into the middle ear via Eustachian tube and affect osseous structures directly. Disturbance of ossicles could cause tinnitus, which is more common for peripheral vertigo. H. pylori could also get in the esophagus and in the upper respirator…

Migrainous headache is determined by pathogenetic mechanisms that are also able to affect the peripheral and/or central vestibular system, so that vestibular symptoms may substitute and/or present with headache. We are convinced that there can be many different manifestations of vestibular disorders in migrainous patients, representing true different clinical entities due to their different characteristics and temporal relashionship with headache. Based on such considerations, we proposed a classification of vertigo and other vestibular disorders related to migraine, and believe that a particu…

The aim of this study was to present the demographic, pathogenetic and clinical features of benign paroxysmal positional vertigo (BPPV) secondary to vestibular neuritis (VN). The medical records of 22 patients, who presented with BPPV within 12 weeks after the onset of VN, were reviewed. Data of a complete otolaryngological, audiological, neurotologic and imaging evaluation were available for all patients. Two hundred and eighty-four patients with idiopathic BPPV were used as a control group. The patients with BPPV secondary to VN presented the following features, in which they differed from t…

Benign paroxysmal positional vertigo is a common cause of disabling vertigo with a high rate of recurrence. Although connections between vitamin D deficiency and osteoporosis, as well as between osteoporosis and benign paroxysmal positional vertigo have been suggested respectively in the literature, we are not aware of any publication linking vitamin D and benign paroxysmal positional vertigo. As a hypothesis, we suggest that there is a relation between insufficient vitamin D level and benign paroxysmal positional vertigo. In order to test this hypothesis, in a small retrospective pilot study,…

Unlike brainstem strokes, small cerebellar strokes can cause isolated dizziness/vertigo with horizontal nystagmus mimicking vestibular neuritis or direction-changing apogeotropic positional nystagmus mimicking benign paroxysmal positional vertigo. Horizontal nystagmus caused by cerebellar strokes were directed toward the side of the lesion, and enhanced when the patient lie on the non-affected side; the enhanced nystagmus were apogeotropic at this position. Direction-changing apogeotropic positional nystagmus, caused by similar cerebellar lesions, were also enhanced when the patient lie on the…

STUDY DESIGN: Case report.
OBJECTIVE: To report an unusual case of refractory positional vertigo without headache that developed after the placement of an epidural catheter for postoperative pain and that was treated with an epidural blood patch.
SUMMARY OF THE BACKGROUND DATA: Although it is common practice to use epidural blood patch (EBP) to treat positional headache from postdural puncture (PDP), the literature is limited to one letter to the editor describing PDP positional vertigo treated with EBP.
METHODS: Description of a clinical case.
RESULTS: Here we present a case where the pat…

OBJECTIVE: To determine if variations in common treatments for benign paroxysmal positional vertigo (BPPV) affected efficacy.
STUDY DESIGN: Prospective, pseudo-randomized study.
SETTING: Outpatient practice in a tertiary care facility.
SUBJECTS AND METHODS: Patients (n = 118) with unilateral BPPV of the posterior canal, including 13 patients with BPPV of the lateral canal, were tested at a tertiary care center on one of five interventions: canalith repositioning maneuver (CRP), CRP plus home exercise, modified CRP, CRP for patients with involvement of two semicircular canals, and self-CRP h…
