Prospective diagnosis of 1,006 consecutive cases of congenital heart disease in the fetus.

OBJECTIVE: This report describes our experience with fetal congenital heart disease since 1980. BACKGROUND: Knowledge and expertise in the diagnosis, management and natural history of fetal congenital heart disease is increasingly demanded by both obstetricians and parents. The analysis of a large series should help the pediatric cardiologist to provide this service. METHODS: The notes of 1,006 patients, where a prospective diagnosis of fetal congenital heart disease was made, were reviewed. The reason for referral, the diagnosis made, the accuracy of diagnosis, the fetal karyotype and the o

Microangiopathy in Ehlers-Danlos syndrome type IV.

In two patients with Ehlers-Danlos syndrome type IV, an autosomal dominant disorder characterized by fragility of large vessels, excessive bruising, and deficiency in type III collagen, capillary microscopy was performed at the nailfold. Indocyanine green and Na-fluorescein were used as fluorescent tracers. Both patients exhibited microangiopathy of the skin capillaries with microbleedings, presence of microaneurysms and increased transcapillary diffusion. Microvascular involvement appears to be an additional manifestation of the syndrome.

An examination of three methods of psychodynamic formulation based on the same videotaped interview.

While psychodynamic theory and therapy are approaching their centennial, the science of psychodynamics is still in an earlier developmental stage. Any scientific field generates the most controversy and excitement when it is still developing. For psychodynamic psychology this means that its basic units of observation as well as its rules for justifying clinical inference in formulating and testing dynamic hypotheses require more development. In short, we are still evaluating different methods for both discovering and validating psychodynamic propositions. This is especially true for central fe

Diastolic filling in acute left ventricular dysfunction: role of the pericardium.

Patients with congestive heart failure and elevated left ventricular filling pressures demonstrate an abnormal pattern of diastolic filling that is characterized by a redistribution of diastolic filling to early diastole with reduced reliance on late diastolic filling. The diastolic filling pattern superficially resembles that which is seen with constrictive pericarditis. To examine potential mechanisms for these clinical findings, a model of ischemic left ventricular dysfunction was produced in seven dogs by repeated coronary microsphere embolization, producing a dilated left ventricle with r

Normal erect swallowing. Normal function and incidence of variations.

Of 871 candidates presenting for upper gastrointestinal examinations, 16 met the rigorous criteria established for selecting asymptomatic normal volunteers. Frame-by-frame evaluation of their videorecorded pharyngeal swallow confirmed many observations made previously utilizing cine recording at much higher radiation dosages. In addition, new observations were made: the nasopharynx may not occlude until the bolus is entirely within the pharynx; air mixes with the bolus if the swallow is an "open" type; the epiglottis always inverts in normal individuals regardless of the type of swallow ("open

Verbal and nonverbal marital interaction patterns of alcoholics and nonalcoholics.

In a study of the interaction of hospitalized alcoholic and nonalcoholic husbands and their wives, husbands tended to speak more during alcohol-related conversations, while wives spoke more when discussing other topics. Wives in both groups looked at their spouses more than their husbands did during alcohol-focused periods.

Situational cue and strategy influence on speechreading.

The influence of situational cues and reasoning strategies on speechreading was studied in normal-hearing and minimally hearing-impaired adults. Everyday Speech Sentences (Davis & Silverman, 1968) produced by a female talker were videotaped without sound in the context of a visual background scene. A non-verbal acoustic signal was matched to each sentence and dubbed onto the recording. Thus, each test item consisted of an inaudible sentence presented with an optical and acoustic situational cue. The experimental task was structured so that subjects speechread sentences using deductive or induc

Early deficits in cortical control of swallowing in Alzheimer's disease.

The goal of this study was to determine whether functional changes in cortical control of swallowing are evident in early Alzheimer's disease (AD), before dysphagia (swallowing impairment) is evident. Cortical function was compared between an early AD group and a group of age-matched controls during swallowing. Swallowing oropharyngeal biomechanics examined from videofluoroscopic recordings were also obtained to more comprehensively characterize changes in swallowing associated with early AD. Our neuroimaging results show that the AD group had significantly lower Blood-Oxygen-Level-Dependent (

Changes of cerebral current source by audiovisual erotic stimuli in premature ejaculation patients.

INTRODUCTION: Premature ejaculation (PE) is one of the most common forms of male sexual dysfunction. The mechanisms of PE remain poorly understood, despite its high prevalence. AIM: To investigate the pathophysiology and causes of PE in the central nervous system, we tried to observe the changes in brain current source distribution by audiovisual induction of sexual arousal. METHODS: Electroencephalograpies were recorded in patients with PE (45.0 +/- 10.3 years old, N = 18) and in controls (45.6 +/- 9.8 years old, N = 18) during four 10-minute segments of resting, watching a music video exce

Factors associated with aggressive behavior among nursing home residents with dementia.

PURPOSE: In an attempt to more thoroughly describe aggressive behavior in nursing home residents with dementia, we examined background and proximal factors as guided by the Need-Driven Dementia-Compromised Behavior model. DESIGN AND METHODS: We used a multivariate cross-sectional survey with repeated measures; participants resided in nine randomly selected nursing homes within four midwestern counties. The Minimum Data Set (with verification by caregivers) identified participants. We used a disproportionate probability sample of 107 participants (51% with a history of aggressive behavior) to

Assessment of head tilt in young children with unilateral posterior crossbite by video recording.

14 children with unilateral posterior crossbites (PCB) participated in this study and 14 children with Angle's class I occlusion. Body posture analysis was made by a video recording technique. The results showed greater tilt in the angles of head tilt in PCB children when compared to neutral occlusion children. We also observed that head tilt followed the side of crossbite. Such results suggest that unilateral PCB could be related to the development of head tilt on the same side of the crossbite.

Intended and unintended effects of an eating disorder educational program: impact of presenter identity.

OBJECTIVE: This study examines the impact of presenter identity on the intended and unintended effects of an eating disorder educational program. METHOD: High school students viewed one of two identical videotaped discussions about eating disorders. In one condition, the presenter was identified as an "eating disorder specialist"; in the other condition, she was identified as a "recovered patient." Before and after watching the video, participants reported on their awareness of various eating disorder symptoms, their beliefs about individuals with eating disorders, and their opinion of the pr

Clinical and anatomical correlates of apraxia of speech.

In a previous study (Dronkers, 1996), stroke patients identified as having apraxia of speech (AOS), an articulatory disorder, were found to have damage to the left superior precentral gyrus of the insula (SPGI). The present study sought (1) to characterize the performance of patients with AOS on a classic motor speech evaluation, and (2) to examine whether severity of AOS was influenced by the extent of the lesion. Videotaped speech evaluations of stroke patients with and without AOS were reviewed by two speech-language pathologists and independently scored. Results indicated that patients wit

Application of red laser video-rate scanning confocal microscopy to in vivo assessment of tubular function in the rat: selective action of diuretics on tubular diameter.

This study examined the use of a red laser illuminated, video-rate scanning confocal reflection microscopy (VRSCM) system, with improved structural and functional imaging at high temporal resolution, to visualize physiological changes in the kidney in response to pharmacological stimuli. We applied VRSCM to superficial nephrons in vivo and measured temporal changes in the diameter of proximal and/or distal tubular segments in response to the administration of three major classes of diuretics with known selective actions at specific nephron sites. Mannitol caused measurable increases in both pr

Psychogenic seizures: clinical features and psychological analysis.

Approximately 25-30% of patients referred to epilepsy centers for refractory epilepsy are found to have nonepileptic seizures (NES). In many cases psychological assessments are performed to evaluate for underlying psychiatric disorders. The authors analyzed the clinical features of 23 patients with NES and correlated the features with underlying psychological status. Thirteen of the twenty-three patients (56.5%) had motor manifestations and 10 presented with limpness and unresponsiveness. There was no significant difference in the occurrence of depression, anxiety, posttraumatic stress disorde

Emotional responding to fearful and disgusting stimuli in specific phobics.

Individuals with small animal and blood-injection-injury (BII) phobias respond to phobia-relevant stimuli with both fear and disgust. However, recent studies suggest that fear is the dominant emotional response in animal phobics whereas disgust is the primary emotional response in BII phobics. The present study examined emotional responding toward pictures of spiders, surgical procedures, and two categories of general disgust elicitors (rotting food and body products) among analogue spider phobics, BII phobics, and nonphobics. Dominant emotional responses to phobia-relevant stimuli clearly dif

Sharing humour and laughter in autism and Down's syndrome.

Everyday humour and laughter can tell us about children's ability to engage with and understand others. A group of 19 pre-school children with autism and 16 pre-school children with Down's syndrome, matched on non-verbal mental age, participated in a cross-sectional study. Parental reports revealed no group differences in overall frequencies of laughter or laughter at tickling, peekaboo or slapstick. However, in the autism group, reported laughter was rare in response to events such as funny faces or socially inappropriate acts, but was common in strange or inexplicable situations. Reported re

Storybook-based communication intervention for girls with Rett syndrome and their mothers.

PURPOSE: Storybook reading provides a natural language learning context in which to support early symbolic communication. In this study, we explored the impact of (1) resting hand splints, (2) light tech augmentative communication systems such as voice-output devices and symbols, and (3) very basic parent training on the symbolic communication and labelling behaviours of six girls with Rett syndrome. METHOD: Mothers and daughters were videotaped as they read familiar and unfamiliar storybooks in their homes. RESULTS: Group and individual data collected from the six girls indicated that they
