Aspergillus tubingensis pneumonia with pulmonary oxalosis in a virologically suppressed person with HIV: A case report.

Opportunistic aspergillosis has occasionally been reported in individuals with advanced HIV infection but remains rare overall and is particularly uncommon under well-controlled conditions. Here, we present a case of Aspergillus tubingensis pneumonia in a virologically suppressed person with HIV (PWH) in his late sixties. He was receiving bictegravir-based antiretroviral therapy, with a CD4 cell count of 368/μL and undetectable HIV RNA two months before symptom onset. His symptoms began with a cough, followed 20 days later by a fever and dyspnea, which required hospitalization. Computed tomogr

Safety and Virological Outcomes of a Modified Albuvirtide Intensification Regimen with Standard ART in People with HIV.

BACKGROUND: To control HIV-1 viral load and maintain optimal immune function, albuvirtide (ABT) combined with antiretroviral therapy (ART) is used in designated AIDS hospitals in China. However, the standard weekly 320-mg ABT injection regimen poses adherence-related challenges. This study explored the safety and virological outcomes of a modified ABT intensification regimen in people with HIV. METHODS: This single-arm, uncontrolled prospective cohort enrolled 20 people with HIV who received intravenous ABT 320 mg for three consecutive days every four weeks while continuing their baseline ART

Virological characteristics of SARS-CoV-2-related coronaviruses dynamically circulating in Southeast Asia.

By sampling horseshoe bats-the reservoir hosts of SARS-CoV-2-related coronaviruses (SC2r-CoVs)-in Thailand, we present two clades of SC2r-CoVs co-circulating in the same bat population. Through a comprehensive set of experimental approaches, including cryo-electron microscopy (cryo-EM), pseudovirus and live virus assays, and hamster experiments, we characterize the virological properties of these new viruses. We show that one of the two clades discovered in this study is able to bind the human angiotensin converting enzyme 2 (ACE2) receptor; however, it exhibits reduced fusogenicity and replic

A Comprehensive Analysis of Diagnostic and Virological Surveillance During the 2023-2025 Measles Epidemic Scenario.

Background/Objectives: Since 2023, a significant increase in measles cases has been reported worldwide, and Italy has been among the most affected European countries. In this context, the integration of laboratory and epidemiological data enables timely case classification and helps distinguish between imported and indigenous cases, supporting disease control. However, most studies address only selected aspects of surveillance. Therefore, this study aimed to provide an integrated analysis of virological and epidemiological surveillance activities conducted between November 2023 and December 20

Epidemiologic, Entomologic, and Virologic Findings during Reemergence of Western Equine Encephalitis Virus, Argentina.

During November 2023-April 2024, a total of 1,543 horses with neurologic disease from rural areas in 17 provinces of Argentina were reported to national animal health authorities. Nested reverse transcription PCR, quantitative real-time reverse transcription PCR, and sequencing of equine brain necropsies confirmed western equine encephalitis virus (WEEV) infection in 23 horses and 1 sheep. Phylogenetic analyses identified a lineage previously detected in Argentina in 1957, overlapping areas affected during the 1980s epizootics. Overall, we aspirated 1,362 female mosquitoes (12 species) from 1

HBx mutant-regulated RPL13AP25 mediates suboptimal virological response to entecavir and HCC progression.

BACKGROUND: A number of effective antiviral agents for chronic hepatitis B are available worldwide, and these agents have exhibited satisfactory virologic suppression, but suboptimal responses still occur in some patients. We aimed to explore the contribution of long non-coding RNAs (lncRNAs) in mediating suboptimal responses in chronic hepatitis B patients, which remains to be fully elucidated. METHODS: Cox regression models were used to analyze associations between suboptimal response and clinical factors. Hepatitis B virus X (HBx) gene was sequenced from entecavir-treated patients who deve

Determinants of virological outcomes in kidney transplant recipients treated with maribavir for refractory cytomegalovirus infection.

Maribavir has demonstrated promising efficacy in treating refractory cytomegalovirus (CMV) infections among solid organ transplant recipients; however, factors influencing favorable outcomes remain uncertain. We conducted a retrospective cohort study that involved 88 kidney transplant recipients (KTRs) from 22 transplantation centers, all of whom had refractory CMV infections treated with maribavir (63 with and 25 without ganciclovir resistance mutations). CMV clearance at week 8 was achieved in 45 KTRs (51.1%). Among the 49 KTRs (55.7%) who achieved CMV clearance within 16 weeks, 37 (42%) did

Virological and Immunological Outcomes of Combined Therapeutic Interventions and Dendritic Cell Therapy in People With HIV.

BACKGROUND: Except for hematopoietic cell transplantation, therapeutic-driven HIV-1 curative approaches have been unsuccessful. Here, we describe a 2-step randomized clinical trial designed to evaluate the safety and impact of individual and combinatorial therapeutic strategies on changes in peripheral and gut mucosal HIV reservoirs, immune activation and function in people with HIV with chronic disease and high CD4 T-cell nadirs. METHODS: Thirty participants were enrolled and randomized equally into 6 study arms based on treatments with either standard antiretroviral therapy (ART) alone or a

Follow-up post-HCV virological response to DAA in advanced chronic liver disease.

Direct-acting antivirals (DAA) achieve high virological response rates with minimal side effects for many patients. Despite their significant impact on the progression and epidemiology of hepatitis C virus (HCV) associated liver disease, the global annual incidence of chronic infections is expected to remain relatively constant, averaging 1.42 million new cases each year until 2030. Furthermore, by 2030, there will be a 14-17% increase in end-stage liver disease outcomes such as liver-related deaths, hepatocellular carcinoma (HCC), and decompensated cirrhosis in adults aged 18 years and over.

Prevalence of and Risk Factors for Liver Enzyme Elevation After Hepatitis C Virologic Cure.

A subset of patients with chronic hepatitis C virus (HCV) infection demonstrate liver enzyme elevation (LEE) after achieving sustained virologic response (SVR). Risk factors for LEE are not well characterised. We conducted a single-centre retrospective cohort study of adults with HCV infection in the Duke University Health System who received direct-acting antiviral therapy and achieved SVR. We performed multivariable logistic regression to assess the relationship between potential risk factors and LEE. We used generalised linear mixed-effects models to explore longitudinal relationships betwe

Virologic and Immunologic Characterization of Coronavirus Disease 2019 Recrudescence After Nirmatrelvir/Ritonavir Treatment.

We isolated a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) BA.2 variant from a person with coronavirus disease 2019 recrudescence after nirmatrelvir/ritonavir treatment. Antiviral sensitivity and neutralizing antibody testing were performed with both parental SARS-CoV-2 and multiple variants of concern. We found that neither nirmatrelvir resistance nor absence of neutralizing immunity was a likely cause of the recrudescence.

Sustained virological response to antiviral drugs in treatment of different genotypes of HCV cirrhotic patients.

Cirrhosis and liver cancer are both caused by hepatitis C virus (HCV) infection of the liver. Patients with HCV cirrhosis may be treated with one of many antiviral medications, depending on their specific genotype. Samples of cirrhotic HCV were obtained from 190 people at the Khyber Teaching Hospital and the Hayatabad Medical Complex in Peshawar, Pakistan. Multiplex and real-time PCR were used to assess the genotypes and viral loads of the samples, respectively. Sixty patients were given sofosbuvir plus daclatasvir with ribavirin, while the remaining 56 patients were given sofosbuvir with riba

Mpox as AIDS-defining event with a severe and protracted course: clinical, immunological, and virological implications.

A 59-year-old treatment-naive patient with advanced HIV infection presented with a severe and protracted course of mpox (formerly known as monkeypox) that did not respond to the current mpox treatment options. The patient worsened clinically, and developed new mucocutaneous lesions and necrotic evolution of pre-existing ones, along with multiple bilateral lung nodules and the appearance of a tracheal necrotic lesion. Although severe forms of mpox have been observed in people with severe immune system deficiency, including those with advanced HIV presentation, the immunological mechanisms under

Clinical, virological, imaging and pathological findings in a SARS CoV-2 antibody positive cat.

This paper reports a presumptive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in a cat. A cat with respiratory disease living with three individuals with coronavirus disease 2019 showed bilateral ground-glass opacities in the lung on X-ray and computed tomography. The clinical swabs were negative for SARS-CoV-2 RNA, but the serum was positive for SARS-CoV-2 antibodies. Interstitial pneumonia and prominent type 2 pneumocyte hyperplasia were noted on histopathology. Respiratory tissues were negative for SARS-CoV-2 RNA or antigen, but the cat was positive for feline parv

Immunological and virological outcomes in children on lamivudine monotherapy: A South African public sector experience.

In resource-limited settings, holding regimens such as lamivudine monotherapy (LAM) have been used to manage HIV-positive children failing combination antiretroviral therapy to mitigate the risk of drug resistance developing, while adherence barriers are addressed or when access to second- or third-line regimens is restricted. South African HIV treatment guidelines previously advocated the use of LAM to manage HIV-infected children with virological failure. However, the outcomes of patients on LAM compared with those who continued on a failing regimen have not been well described. Objectives.
