Microfilaria Infection in Metastatic Node in a Case of Breast Carcinoma.

Filariasis is a major public health problem in tropical and subtropical countries such as India. It is caused by nematodes, principally Wuchereria bancrofti and Brugia malayi and is transmitted by the Culex mosquito. W.bancrofti accounts for 90% of cases of lymphatic filariasis. Filaria of the breast with axillary lymphadenopathy and lymphedema, mimicking breast carcinoma has been previously reported. Microfilariae have also been previously observed as coincidental findings with various inflammatory and neoplastic lesions. Few examples are hemangiomas of the liver, meningiomas, intracranial he…

Strategies to Control Human Lymphatic Filarial Infection: Tweaking Host's Immune System.

Human lymphatic filariasis (LF), a parasitic infection caused by the nematodes Wuchereria bancrofti, Brugia malayi and B. timori, and transmitted by mosquito, results in a debilitating disease commonly identified as 'elephantiasis'. LF affects millions of people in India and several other tropical and sub-tropical countries imposing a huge economic burden on governments due to disability associated loss of man-hours and for disease management. Efforts to control the infection by WHO's mass drug administration (MDA) strategy using three antifilarials diethylcarbamazine, albendazole and ivermect…

Sensitivity and specificity of ultrasound detection and risk factors for filarial-associated hydroceles.

To better understand risk factors for hydrocele as a consequence of Wuchereria bancrofti infection, 342 men more than 15 years of age in an endemic area in Papua New Guinea were evaluated. Thirty-four subjects (9.9%) had hydrocele by physical examination. Ultrasound examination detected hydroceles in 57 men (16.7%). Compared with ultrasonography, the sensitivity of physical examination was 44.3%, the specificity was 98.2%, and the positive predictive value was 73.5%. Hydrocele was independently associated with age (odds ratio [OR] = 3.3, P < 0.01) and intensity of infection as determined by fi…

No evidence of Wolbachia endosymbiosis with Loa loa and Mansonella perstans.

Endosymbiotic Wolbachia bacteria from different filarial species, including major pathogens of humans such as Wuchereria bancrofti, Brugia malayi and Onchocerca volvulus, seem to play an important role in the development, viability and fertility of these worms. Wolbachia trigger inflammatory host responses as well as adverse reactions against standard treatment regimens and are therefore under investigation as novel treatment targets. We investigated whether Wolbachia are also endosymbiotic in Loa loa and Mansonella perstans. In both male and female adult L. loa, we found no evidence of bacter…

Impact of insecticide-treated materials on filaria transmission by the various species of vector mosquito in Africa.

Nocturnally periodic bancroftian filariasis is maintained by three mainly endophilic vectors in East Africa: Culex quinquefasciatus, Anopheles funestus and the An. gambiae complex. Permethrin-impregnated bednets provide considerable protection against these mosquitoes, but the species respond differently. The degree of protection conferred by treated bednets was determined in Kenyan communities where all three vectors actively transmit Wuchereria bancrofti. The annual transmission potential in the communities (i.e. an estimate of the number of human infective, third-stage larvae of W. bancroft…

New tools for diagnosis and monitoring of bancroftian filariasis parasitism: the Polynesian experience.

Bancroftian filariasis is endemic in French Polynesia and control programs with diethylcarbamazine, started in the 1950s, led to a sharp red … e include: (1) the detection of adult worm circulating antigens in humans, and (2) the detection of Wuchereria bancrofti larvae in mosquitoes, using DNA probes. In this paper, Luc Nicolas reviews the available diagnostic tools to detect W. bancrofti and their implementation in epidemiological areas, based on the Polynesian experience.

Histological evidence for adulticidal effect of low doses of diethylcarbamazine in bancroftian filariasis.

The ability of diethylcarbamazine (DEC) to kill adult Wuchereria bancrofti worms was evaluated by examining lymphatic nodules formed after treatment with 4 different treatment schedules of 193 males living in the endemic area of Greater Recife, Brazil. Lymphatic nodules appeared in the spermatic cord or upper extremities in 43 of 138 microfilaraemic individuals, in 3 of 30 amicrofilaraemic patients with filarial disease manifestations, and in 1 of 25 asymptomatic amicrofilaraemic residents of the endemic area treated with DEC. Fourteen of these nodules were surgically removed 10-150 d after th…

Filarial vector studies in a diethylcarbamazine-treated and in untreated villages in Papua New Guinea.

Entomological studies were undertaken in three villages in the East Sepik Province of Papua New Guinea. The inhabitants of one village, Nanaha, had been treated with diethylcarbamazine (DEC) to reduce the prevalence and density of microfilaraemia of Wuchereria bancrofti. No intervention was undertaken in the other two villages, Yauatong and Musenau, in which bancroftian filariasis was present but with markedly different human prevalence rates and mean parasite densities. In Yauatong, infection rates in anopheline vectors (Anopheles punctulatus and An. koliensis) varied from 20.5 to 46.6% with …

Bancroftian filariasis-differential reactivity of anti-sheath antibodies in microfilariae carriers.

Anti-sheath antibodies have been detected using an immunofluorescent assay (IFAT) in the sera of microfilariae carriers (AS cases) residing in areas endemic for Bancroftian filariasis. Microfilariae (mf) of Wuchereria bancrofti purified from five different mf carriers were used separately as antigen to identify anti-sheath antibodies. The reactivity of sera from AS cases to mf sheath was found to be variable to the five different mf preparations. While as high as 25% of the sera reacted with mf purified from one individual, none of them reacted with mf purified from two other individuals. Such…

Identification and characterization of a parasite antigen in the circulating immune complexes of Onchocerca volvulus infected patients.

A PEG-ELISA was used to demonstrate parasite specific immune complexes in a significant proportion (25/26) of Onchocerca volvulus infection … This antigen was not present in the circulating immune complexes (CIC) prepared from patients with Wuchereria bancrofti infection, but a cross-reactive molecule of the same size was weakly recognized in the CIC of Loa loa and Mansonella perstans infected patients. No association between the level of parasite specific CIC and clinical disease was observed in …

Natural infections of Dirofilaria immitis in Aedes (Stegomyia) polynesiensis and Aedes (Finlaya) samoanus and their implication in human health in Samoa.

Dirofilaria immitis infections were observed in Aedes polynesiensis and Ae. samoanus in Samoa, together with Wuchereria bancrofti infections, in a study on sub-periodic bancroftian filariasis during 1978-1980. In the 4 indicator villages, the infection rate in Ae. polynesiensis was 0.46% and the infective rate 0.09% (15,223 mosquitoes were dissected). The infection rate in Ae. samoanus was 0.20% and the infective rate 0.08% (10,089 dissected). In 45 selected villages throughout the country, Ae. polynesiensis infection and infective rates were 0.92% and 0.29% (7575 dissected) and the rates for …

Clinical epidemiology of bancroftian filariasis: effect of age and gender.

A cross-sectional survey was used to determine the prevalence of disease (n = 6493) and microfilaraemia (n = 24,946) due to Wuchereria bancrofti in Pondicherry, south India. The total disease attributable to filariasis was significantly higher in males (13.67%) than females (2.26%), due to the occurrence of hydrocele in males. While the prevalence of chronic signs was clearly age-dependent in both sexes, that of acute signs was independent of age. Thus the age and gender structure of the survey sample will crucially influence apparent prevalence. Examination of the gender differences in the po…

A technique for microfilarial detection in preserved blood using nuclepore filters.

Nuclepore filtration is now the most widely used method of detecting microfilaremia, particularly if microfilariae are few in number. However, this system requires the blood sample to be processed promptly after collection. Using blood from Wuchereria bancrofti-infected patients, 3 solutions were tested to determine whether blood could be held for delayed processing. Of these, we identified 1, 2% formalin/10% Teepol, in which microfilaremic blood can be held for at least 9 mo without deterioration of microfilarial structure or decrease in microfilarial numbers. In addition, this mixture passed…

Onchocerca volvulus heat shock protein 70 is a major immunogen in amicrofilaremic individuals from a filariasis-endemic area.

Infestation with organisms causing lymphatic filariasis (i.e. Wuchereria bancrofti and Brugia malayi) results in a variety of clinical presentations. It is possible that some of the variation is due to differences in host response to parasite. To determine whether individuals who live in an endemic area but differ in their clinical manifestations respond to different filarial antigens, we screened Onchocerca volvulus expression libraries with sera from a number of individuals belonging to different clinical groups. The results of the study demonstrate that there are indeed differences in the r…

Filariasis in West Kalimantan (Borneo), Indonesia.

A survey was carried out among persons residing in 8 villages in the Province of West Kalimantan, Indonesia to determine the prevalence of filariasis. Finger tip blood smears were obtained at night from over 3,000 people and microfilariae of Brugia malayi were found in 108 (3.5%) and Wuchereria bancrofti in 10 (0.3%). Most B. malayi (96 carriers) was found in Kakap, a village near the coast, 20 km from the provincial capital of Pontianak. Nine of 10 cases of W. bancrofti were located in Pahauman, a village 130 km northeast of the provincial capital. Periodicity studies indicate the strain of B…

Subcutaneous nodules as a manifestation of onchocerciasis. A rare condition in the United States.

Although infection with tissue nematodes is a significant health problem in many parts of the world, such cases are unusual in the continental United States. This report describes a case of Onchocerca volvulus infection, acquired by a 38-year-old female missionary in northern Brazil, that manifested as subcutaneous nodules in the groin and flank two years after the patient's return to the United States. A year previously, a wrong diagnosis of Wuchereria bancrofti was made at biopsy. At that time, treatment with diethylcarbamazine citrate had to be discontinued because of intense pruritus. Exam…

[Lymphatic filariasis transmission assessment survey in schools three years after stopping mass drug treatment with albendazole and ivermectin in the 7 endemic districts in Togo].

The aim of this study is to verify the level of transmission of lymphatic filariasis three years after stopping mass drug treatment in the 7 endemic districts in Togo. The survey was conducted in 2012 in Togo's 7 endemic districts grouped into four evaluation units (EU) using the WHO-recommended transmission assessment survey (TAS) protocol. Children aged 6-7 years were screened for Wuchereria bancofti antigen using the immunochromatographic card (ICT) method. A cluster sampling method was used to select eligible children in schools as the net primary-school enrolment ratio is greater than or …

Filariasis-Associated Secondary Membranous Nephropathy: A Rare Presentation.

Parasitic infections like filariasis are uncommon causes of secondary membranous nephropathy (MN) which requires serological tests to detect circulating Wuchereria bancrofti antigens for its diagnosis or the identification of microfilariae in the capillary lumen on kidney biopsy. The immunochromatographic card tests is simple, non invasive and has high sensitivity and specificity. We report a case of 30 year old lady who presented to us with nephrotic syndrome which on kidney biopsy showed features of membranous nephropathy with negative staining for Anti PLA2R, THSD7A and exostosin. A case report on rare finding of microfilaria in pus sample of an ulcer over elephantiasis leg.

Skin ulcerations are a significant cause of morbidity and can be challenging to manage. Among the various causes of chronic non-healing ulcers, lymphedema is also considered a possible diagnosis in countries such as Nepal. Lymphatic filariasis has been a significant public health issue in endemic areas. Wuchereria bancrofti is a common nematode parasite that causes lymphatic filariasis. Excessive retention of lymphatic fluid in the interstitial compartment can cause localized tissue swelling, known as lymphedema, which is caused by impaired lymphatic drainage. Microfilariae can be detected in …

Acute scrotum: Hansen's disease versus filariasis.

Hansen's disease is caused by Mycobacterium leprae. The disease is known to involve the visceral organs including the testis apart from the skin and nerves in the lepromatous pole of leprosy due to widespread hematogenous dissemination of lepra bacilli. Furthermore, there can be testicular pain during the type 2 reaction in Hansen's disease. Filariasis is a disease caused by the parasitic nematode, Wuchereria bancrofti. This infection most commonly results in lymphedema and secondary vaginal hydrocele with an associated epididymo-orchitis. Acute epididymo-orchitis is either seen in the acute p…
