Timeliness of yellow fever surveillance, Central African Republic.

During January 2007-July 2012, a total of 3,220 suspected yellow fever cases were reported in the Central African Republic; 55 were confirmed and 11 case-patients died. Mean delay between onset of jaundice and case confirmation was 16.6 days. Delay between disease onset and blood collection could be reduced by increasing awareness of the population.

Yellow fever vaccination: is one dose always enough?

In March 2013, the World Health Organization (WHO) Strategic Advisory Group of Experts on Immunisation (SAGE) considered a number of issues in order to update the WHO Position Paper on Yellow Fever (2003). A key conclusion of this review was that a single dose of yellow fever (YF) vaccine appears to confer life-long protection against YF disease, and that a booster dose of YF vaccine is not needed to maintain immunity. While the efficacy of YF vaccine in the majority of vaccine recipients is not in doubt, the WHO announcement is somewhat surprising as there are some limitations in the evidence…

The phylogeny of yellow fever virus 17D vaccines.

In recent years the safety of the yellow fever live vaccine 17D came under scrutiny. The focus was on serious adverse events after vaccinations that resemble a wild type infection with yellow fever and whose reasons are still not known. Also the exact mechanism of attenuation of the vaccine remains unknown to this day. In this context, the standards of safety and surveillance in vaccine production and administration have been discussed. Therein embodied was the demand for improved documentation of the derivation of the seed virus used for yellow fever vaccine production. So far, there was just…

[Resurgence of yellow fever epidemics in Côte-d'Ivoire].

Resurgence of yellow fever epidemics in Côted'Ivoire remains a major problem of public health. To describe this disease through the early alarm system of National Institute of Public Hygiene (NIPH) is convenient to us. This is a cross-sectional study with descriptive aiming, which proceeded with epidemiological surveillance service of NIPH. The data were collected over 7 years (from 2001 to 2007), starting from the cards of notification, reports, and registers of the service. The main findings of our study are that: 1) we listed 1468 suspect cases including 41 confirmed and the lethality rate …

Critical aspects of yellow fever control in Brazil.

This paper presents epidemiological and control characteristics of yellow fever in Brazil, taking its wild and urban transmission cycles into consideration. No urban cases have been reported in Brazil since 1942, but urban yellow fever cases were reported in Paraguay in 2008, after more than 50 years without registered cases in the Americas. The two main objectives of yellow fever control programs in Brazil are to reduce the number of wild cases and to maintain zero incidence of urban cases. Although there is a consensus regarding control measures that should be applied in areas endemic for th…

Assessing yellow Fever risk in the ecuadorian Amazon.

This study reports results of a cross-sectional study based on interviews and seroepidemiological methods to identify risk factors for yellow fever infection among personnel of a military garrison in the Amazonian rainforest. Clinical symptoms and signs observed among yellow fever cases are also described. Humoral immune response to yellow fever, Mayaro, Venezuelan equine encephalitis, Oropouche, and dengue 2 infection was assessed by evaluating IgM and IgG specific antibodies. A yellow fever attack rate of 13% (44/341, with 3 fatal cases) was observed among military personnel. Signs of digest…

Yellow fever-associated viscerotropic disease in Barcelona, Spain.

Yellow fever vaccine is a live, attenuated viral preparation from the 17D virus strain. Since 1996, 34 cases of yellow fever vaccine-associated viscerotropic disease (YEL-AVD) have been described. We report a new case of YEL-AVD. Given the potential risks associated with the vaccine, physicians should consider vaccination only for patients truly at risk for exposure to yellow fever, especially for primovaccination.

Analysis of yellow fever vaccination practice in England.

BACKGROUND: Prior to initiation of a program of registration, training, standards, and audit for yellow fever vaccination centers (YFVCs) in England, the National Travel Health Network and Centre (NaTHNaC) assessed the current practice and resource needs of these centers. METHODS: A questionnaire was sent to all YFVCs on the English Department of Health (DH) database in November 2004. It surveyed type of practice, administration of travel vaccines, training and duties of staff, vaccine storage and record keeping, access to travel health information, and resource and training needs. RESULTS: …

Does yellow fever 17D vaccine protect against melanoma?

BCG vaccine, vaccinia vaccine and certain pathogens that were shown in previous studies to protect against melanoma have antigenic determinants homologous in their amino acids sequence with the melanoma antigen HERV-K-MEL, encoded by a human endogenous retrovirus K (HERV-K), which is expressed in about 95% of malignant melanocytes. Yellow fever vaccine (YFV) likewise contains an antigenic determinant with a close homology to HERV-K-MEL and might therefore also confer protection against melanoma. To investigate this possibility we carried out a cohort study (28,306 subjects) and a nested case-c…

Early exposure to yellow fever vaccine during pregnancy.

OBJECTIVE: To investigate the association of Yellow Fever Vaccination (YFV) during pregnancy with the presence of structural defect in exposed babies. METHODS: An observed/expected frequencies study, before and after the vaccination campaign against YF was designed. 304 babies exposed to YFV during the prenatal period underwent dysmorphological examinations. The expected frequencies of malformations were obtained from a reference population of 10,691 births occurred in the period immediately prior to the vaccination campaign and born in the same region. These frequencies were evaluated using …

Yellow fever virus NS3 protease: peptide-inhibition studies.

A recombinant form of yellow fever virus (YFV) NS3 protease, linked via a nonapeptide to the minimal NS2B co-factor sequence (CF40-gly-NS3pro190), was expressed in Escherichia coli and shown to be catalytically active. It efficiently cleaved the fluorogenic tetrapeptide substrate Bz-norleucine-lysine-arginine-arginine-AMC, which was previously optimized for dengue virus NS2B/3 protease. A series of small peptidic inhibitors based on this substrate sequence readily inhibited its enzymic activity. To understand the structure-activity relationship of the inhibitors, they were docked into a homolo…

Acute viscerotropic disease following vaccination against yellow fever.

Acute viscerotropic disease following yellow fever vaccination (YEL-AVD) is a rare but serious complication of vaccination with 17D yellow fever vaccine. This paper reviews the existing literature regarding YEL-AVD and discusses possible etiologic mechanisms. A greater understanding of this condition is essential to assuring safe and effective prevention of yellow fever and vaccination against other arboviral diseases for which 17D-based vaccines are being developed.

Evaluation of yellow fever vaccination centers in Canada.

BACKGROUND: Health Canada regulates the designation of yellow fever vaccination centers (YFVCs) in Canada to ensure a national standard in the safe, effective, and appropriate use of yellow fever vaccine for Canadian travelers. The process for the designation of YFVC has evolved over the years, particularly with the development and implementation of specific criteria for designation. Standardized site visits in 1999 to 2001 demonstrated that noncompliance with the criteria jeopardized the objectives of the federal program. METHODS: A pilot evaluation questionnaire was developed and tested in …

Carlos Finlay and yellow fever: triumph over adversity.

The eradication of yellow fever in Havana, Cuba, was achieved by a fruitful collaboration between American and Cuban physicians. Carlos Finlay, a Cuban physician who proposed the mosquito-vector theory in 1881, shared his ideas, his publications, and a sample of mosquito eggs with the U.S. Army Yellow Fever Commission. The commission, headed by MAJ Walter Reed, used human volunteers to confirm Finlay's theory. MAJ William Gorgas adopted mosquito-control measures in his sanitation program and, within 6 months, yellow fever was controlled in Havana for the first time. Finlay held fast to his ide…

High fidelity of yellow fever virus RNA polymerase.

Three consecutive plaque purifications of four chimeric yellow fever virus-dengue virus (ChimeriVax-DEN) vaccine candidates against dengue virus types 1 to 4 were performed. The genome of each candidate was sequenced by the consensus approach after plaque purification and additional passages in cell culture. Our data suggest that the nucleotide sequence error rate for SP6 RNA polymerase used in the in vitro transcription step to initiate virus replication was as high as 1.34 x 10(-4) per copied nucleotide and that the error rate of the yellow fever virus RNA polymerase employed by the chimeras…
