Impact of accountable care organizations on acute cholecystitis outcomes in the Rio Grande Valley.

BACKGROUND: Accountable care organizations through the Affordable Care Act are to improve Medicare beneficiaries' health while reducing costs. We hypothesize that this model may shift care, disease burden, and costs to nonaffiliated hospital facilities in patients with acute cholecystitis. METHODS: A retrospective difference-in-differences analysis was performed to compare severity, postoperative complications, diagnostic modality, length of stay, and costs in patients with acute cholecystitis from a post-accountable care organization implementation period (January 2014 through December 2015)

Strength is negatively associated with depression and accounts for some of the sex difference: A replication and extension.

BACKGROUND: Depression occurs about twice as often in women as in men, a disparity that remains poorly understood. In a previous publication, Hagen and Rosenström predicted and found that grip strength, a highly sexually dimorphic index of physical formidability, mediated much of the effect of sex on depression. Striking results like this are more likely to be published than null results, potentially biasing the scientific record. It is therefore critical to replicate and extend them. METHODOLOGY: Using new data from the 2013-14 cycle of the National Health and Nutrition Examination Survey, a

A neurocognitive computational account of word production, comprehension, and repetition in primary progressive aphasia.

Computational models have elucidated word production, comprehension, and repetition in poststroke aphasia syndromes, but simulations are lacking for primary progressive aphasia (PPA) resulting from neurodegenerative disease. Here, the WEAVER++/ARC model, which has previously been applied to poststroke aphasia, is extended to the three major PPA variants: nonfluent/agrammatic, semantic, and logopenic. Following a seminal suggestion by Pick (1892/1977) and modern empirical insights, the model assumes that PPA arises from a progressive loss of activation capacity in portions of the language netwo

Straatsma Syndrome: Should Visual Prognostic Factors Be Taken into Account? A Case Report.

Straatsma syndrome is the triad of myelinated retinal nerve fibers, myopia, and amblyopia and may be associated with strabismus, nystagmus, hypoplastic optic nerve, and heterochromia iridum. The degree of anisometropia, presence of strabismus, extent of myelination, and macular involvement have been reported to be associated with poor visual acuity after occlusion therapy for amblyopia in this syndrome. Here we present two cases of Straatsma syndrome with different responses to occlusion therapy and discuss their treatment responses according to prognostic factors for post-occlusion visual acu

Rhinos and risk assessments: Adjusting risk assessment methodologies to account for 'unforeseeable' events.

It is wrong to describe the COVID-19 pandemic as a 'black swan' (ie a catastrophic event that no one saw coming). Far more apt would be to call it a 'grey rhino' - something that has long been predicted but overlooked by leadership the world over. This paper argues that it is time to stop relying on outdated risk formulas and adjust risk assessment methodologies to account for these grey rhinos. Simply put, it is time to accept that the potential impact of an event is more important than its likelihood.

Attitude of Medical Students About Their Role and Social Accountability in the COVID-19 Pandemic.

Background: In this study, we aimed to explore the attitude of medical students toward their role and social accountability in this pandemic era. An online survey was developed to elicit information on (1) the role of medical students in the pandemic era; (2) Medical education in the "new normal," and (3) the impact of COVID-19 on medical students. Methods: The online survey, developed by a team consisting of three medical students, three psychiatry residents, and three professors of psychiatry, was conducted on 574 participants (213 medical students, 180 graduates, and 181 professors) in the

Commentary: Moral and Ethical Leadership in the Age of Diversity, Equity, Inclusiveness and Social Accountability.

This article weighs three important moral and ethical considerations for leaders in healthcare: their obligations to society, their privilege as leaders and how value goes beyond a simple cost analysis. Leaders highly motivated by the bottom line have avoided the long overdue action on moral and ethical considerations critical to a more just and fairer society. Leaders are now being tasked to develop strategies for health equity, anti-oppression, anti-racism, social justice, diversity, equity and inclusiveness, community engagement, the social determinants of health and environmental accountab

My pigs are ok, why change? - animal welfare accounts of pig farmers.

Intensive pig production systems are a source of stress, which is linked to reduced animal welfare and increased antimicrobial use. As the gatekeepers of the welfare of the animals under their care, farmers are seen as the stakeholder responsible for improving animal welfare. The aim of this study was to explore the knowledge and attitudes of pig farmers towards pig welfare and the impact of such attitudes on farmers' selection of management strategies on the farm. We conducted in-depth semi-structured interviews with 44 pig farmers in one of the main pig producing regions of Brazil. Interview

An Ethnographic Account of the British Equestrian Virtue of Bravery, and Its Implications for Equine Welfare.

This article describes the virtue of bravery in British equestrian culture and suggests that riders' tactics for bolstering bravery may have negative implications on equine welfare. These observations are based on 14 months of ethnographic research among amateur riders and the professionals who support them (n = 35), utilising participant observation and Dictaphone recordings. Riders suffering from 'confidence issues' could be belittled and excluded. Instructors' approaches towards bolstering bravery involved encouraging riders to 'get tough'-on both themselves and on their horses. Narrative t

In the context of a deficiency of some essential nutrients in the diet of the majority of Russian population, studies aimed at evaluation of the accumulation of nutrients in plant foods that determine the nutritional value and contribute to the improvement of the nutritional structure of the human diet are relevant. The aim of this work was to evaluate content of macro- and micronutrients that determine nutritional value in strawberries, taking into account varietal characteristics, and to assess its opportunity to improve the structure of human nutrition when eaten fresh and in processed prod

Men, chronic illness and healthwork: accounts from male partners of women with endometriosis.

Currently dominant in medical discourse, the concept of self-management sees the responsibility for health and illness shift from the state to the individual. However, while this emphasis on individual responsibility and management has burgeoned, the role and status of partners and other family members in the management of chronic illness remains under-theorised. While self-management privileges individual responsibility for the management of chronic illness, the role of partners remains unclear. This paper utilises data from a study of heterosexual couples' experiences of living with the chro

Perceived burdensomeness uniquely accounts for suicidal ideation severity in social anxiety disorder.

BACKGROUND: High rates of suicidal ideation in those with social anxiety disorder (SAD) have been attributed to feelings of thwarted belongingness and perceived burdensomeness, but most work has been in non-clinical samples. We assessed the contributions of thwarted belongingness and perceived burdensomeness to suicidal ideation severity, over clinical covariates, in individuals diagnosed with SAD. METHODS: Participants were 58 adult outpatients (mean age 25.62 years, 69% female) with SAD. Hierarchical linear regression assessed contributions of thwarted belongingness and perceived burdensome

Xist Repeats A and B Account for Two Distinct Phases of X Inactivation Establishment.

X chromosome inactivation (XCI) is a global silencing mechanism by which XX and XY mammals equalize X-linked gene dosages. XCI begins with an establishment phase during which Xist RNA spreads and induces de novo heterochromatinization across a female X chromosome and is followed by a maintenance phase when multiple epigenetic pathways lock down the inactive X (Xi) state. Involvement of Polycomb repressive complexes 1 and 2 in XCI has been intensively studied but with conflicting conclusions regarding their recruitment and role in Xi silencing. Here, we reveal that establishment of XCI has two

The Self and its World: A Neuro-Ecological and Temporo-Spatial Account of Existential Fear.

The current international crisis situation caused by the COVID-19 pandemic is having a strong psychological impact on our subjectivities. We are constantly threatened by the danger of i) being infected, ii) infecting other people, and (iii) by the loss of social relation. Departing from these premises, we here aim to investigate the psychological and neurodynamics of this complex phenomenon. First, we discuss about recent psychological and neuronal findings on fear and its disorders, related to an unbalanced intero-exteroceptive processing and emotional regulation. Secondly we move to the psyc

Spatial analysis of hypospadias cases in northern France: taking clinical data into account.

BACKGROUND: Strong evidence for a causal role of environmental factors in a congenital anomaly is still difficult to produce. The collection of statistical data is crucial for gaining a better understanding of the epidemiology and pathophysiology of these anomalies. We aimed to evaluate spatial variations in hypospadias within our region and it's association to socioeconomic and ecological factors, taking clinical data into account. METHODS: All boys with hypospadias born in northern France and seen in Lille University Medical Center (Lille, France) between 1999 and 2012 were included in the

Transmitted trauma as badge of honor: Phenomenological accounts of Holocaust descendant resilient vulnerability.

This study explores the phenomenological experience of the transmitted trauma legacies of Jewish-Israeli Holocaust descendants and their self-perceived sense of vulnerability and resilience. 55 in-depth interviews were conducted with second-generation Holocaust survivors in Israel in 2001-2004 and another 20 interviews in 2010-2011. Ethnographic interviews reveal unique local configurations of emotional vulnerability and strength. Respondents normalize and valorize emotional wounds describing them as a "scratch" and as a "badge of honor". This self-depiction challenges the typical profile in t

Auditory gating in adults with dyslexia: An ERP account of diminished rapid neural adaptation.

OBJECTIVE: A recent functional magnetic resonance imaging (fMRI) study of adults with dyslexia showed a general deficit in suppressing responses to various types of repetitive stimuli. This diminished neural adaptation may interfere with implicit learning and forming stable word representations. With fMRI, spatial but not temporal characteristics of the adaptation response could be identified. We address this knowledge gap using event-related potentials. METHODS: Fourteen adults with dyslexia and 14 controls participated in an auditory gating paradigm using tone pairs. Response amplitudes and

Changes in healthcare delivery following spinal fracture in Medicare Accountable Care Organizations.

BACKGROUND CONTEXT: Accountable Care Organizations (ACOs) were designed to reduce healthcare costs while simultaneously improving quality. Given that the success of ACOs is predicated on controlling costs, concerns have been expressed that patients could be adversely affected through restricted access to surgery, including in the context of spine fracture care. PURPOSE: Evaluate the impact of Medicare ACO formation on the utilization of surgery and outcomes following spinal fractures. STUDY DESIGN: Retrospective review of Medicare claims (2009-2014). PATIENT SAMPLE: Patients treated for spi
