The activity of adenosine deaminase (ADA) was investigated in pleural effusions from 10 patients with tuberculous pleurisy and 76 patients with pleural effusions of other aetiology. The ADA activity in the tuberculous patients was significantly higher than in the other groups, with the exception of those with empyema. Specificity (87%) and sensitivity (100%) of this test for tuberculosis is high when a reference limit of more than 53 U/l is taken.

Binding sites for 2-chloro-[3H]-adenosine were found to be present in rat kidney membranes. Binding was inhibited by adenosine receptor agonists (5'-N-ethylcarboxamide adenosine, NECA; L-phenylisopropyl adenosine, L-PIA) and antagonists (theophylline; 3-isobutyl-1-methylxanthine, IBMX) but unaffected by an adenosine uptake inhibitor (papaverine). The binding sites exhibited the properties of an adenosine A2 receptor: micromolar affinity for 2-chloroadenosine; nearly identical potencies of theophylline and IBMX for inhibition of binding; NECA more potent than L-PIA for inhibition of binding.

Intravenous adenosine proved effective in eight out of ten episodes of acute spontaneous supraventricular tachycardia. AV node blockade induced by adenosine occurred 20 s after injection, and disappeared very quickly. Side-effects were transient and never serious. Therefore, it is thought that adenosine can be proposed safely for the short-term treatment of supraventricular tachycardia. Because of the different mechanisms of action, the specific side-effects observed after ATP administration are unlikely to be encountered.

Plasma cAMP, serum glucose, and cardiovascular responses to isoproterenol infusion were examined in seven normal subjects and seven patients with pseudohypoparathyroidism (PHP), type I (three with deficiency of the hormone receptor-cyclase coupling protein (N-protein) and four who were N-protein replete). The discovery of the causal association of adenosine deaminase (ADA) and purine nucleoside phosphorylase (PNP) deficiency with some forms of primary immunodeficiency disease had led to new approaches to therapy, such as enzyme replacement. The effects of an injection of 40 mg of ATP were studied in 48 subjects with an overt or latent preexcitation syndrome. The results showed that the slowing of anterograde or retrograde conduction by ATP was not specific for nodal conduction. This phenomenon was observed in conduction through Kent bundles with long refractory periods and the possibility of preexcitation due to the association of James and Mahaim fibres should also be considered.
