Prediction of successful outcome in patients with primary aldosteronism.

Primary aldosteronism is one of the most common causes of secondary hypertension. In recent years the prevalence has risen dramatically, from 1% to 14% of all hypertensive patients. This has been largely attributed to an increase in diagnosis. Primary aldosteronism is characterized by hypertension with or without hypokalemia and a high plasma aldosterone concentration (PAC) with a concurrent low plasma renin activity (PRA). The most common subtypes of primary aldosteronism are aldosterone-producing adenoma (42%) and bilateral idiopathic hyperaldosteronism (58%). Other less common subtypes (<1%

Primary aldosteronism.

The mineralocorticoid-excess state caused by primary aldosteronism usually causes hypokalemia and moderate to sever hypotension. A directed approach to the patient with suspected primary aldosteronism is essential. Appropriate use of biochemical and diagnostic imaging studies can identify the etiology of the primary aldosteronism in an efficient and noninvasive way in most cases. Precision in defining the etiology of the mineralocorticoid-excess state logically leads to therapeutic strategies that usually cure or improve the hypertensive state.

[Functioning tumors of the adrenal cortex in children. Clinical and therapeutic considerations on 11 cases].

In the last 20 years, eleven children with adrenocortical functional tumors were treated in the National Children Hospital of Costa Rica. There were nine females and two males and their ages ranged from nine months to 14 years. Eleven patients had features of virilism, five had stigmas of Cushing's syndrome and three hyperaldosteronism. The clinical diagnosis was established given the symptoms, hormonal tests and radiological and imagenological studies. The histologic diagnosis was carcinoma in six by clinical picture in one, and were adenoma in four. Three patients had regional and distant me

[A rare case of primary aldosteronism due to bilateral functioning adrenocortical adenomas].

A 43-year-old man was admitted to our hospital in January, 1991 for further examination of polydipsia, polyuria and hypertension. He had had a personal history of hypertension since 1976 and of diabetes mellitus since 1982. Physical examination and routine laboratory studies showed that the patient was characterized by asymptomatic hypertension in the presence of hypokalemia and increased urinary potassium excretion. Plasma aldosterone concentrations (PAC) were elevated and plasma renin activity (PRA) was suppressed, resulting in a considerable increase in the ratio of PAC to PRA. PAC was not

Hemoperitoneum secondary to traumatic rupture of an adrenal tumor in a dog.

Hemoperitoneum secondary to traumatic rupture of an adrenocortical adenocarcinoma was diagnosed in a 10-year-old male dog. Immediate surgical attention was required to remove the tumor and to control hemorrhage. The dog appeared to develop transient hypoadrenocorticism after surgery, but recovered with short-term exogenous corticosteroid administration. At 6 months after surgery, the dog was clinically normal.

[Conn's adenoma as a cause of hypertension].

The authors describe two observations of primary hyperaldosteronism, caused by a cortical adrenal adenoma (Conn's adenoma). Both patients were treated by unilateral adrenalectomy from lumbotomy. (In the second patient subtotal resection). The problem is discussed from the aspect of morphological diagnosis and location with emphasis on the primary importance of CT. The surgical operation is aimed and simple without the necessity of peroperative confirmation of the morphological background of the syndrome.

Massive retroperitoneal hemorrhage from an asymptomatic adrenal cortical adenoma. Report of a case.

A 52-year-old man was admitted to the General Surgical Service, with acute onset of left back and flank pain. Two weeks prior to admission he had been subjected to a 3G to 4G acceleration in an ejection seat training simulator. On the day of admission, he had performed a 100 yard swim in flight gear following a seven foot jump into water. He denied any injury during the above exercises or any other trauma. A falling hematocrit was demonstrated by serial determinations and a computerized tomography scan revealed a left retroperitoneal hematoma with normal bilateral renal function and no obvious

Adrenal cortical carcinoma in a 4-year-old child: temporal changes in steroid secretion.

A case is reported of a 4-year-old female with an adrenal cortical carcinoma that developed at the site of a previously removed adrenal gland. Initially, the tumor was only virilizing, but with time, it also became the source of a large secretion of glucocorticoids and, to a lesser degree, mineralocorticoids. It is one of the largest and most functional tumors described in a child.

Pemphigus erythematosus and thymoma.

A sixty year old woman was found to have a benign thymoma and pemphigus erythematosus. Her skin disease was controlled with oral and topical corticosteroids. The association between thymoma and pemphigus erythematosus is discussed, and the literature reviewed. Most previous cases have also had myasthenia gravis and skeletal muscle antibodies. This is the first case in which neither of these was present.

Rare adrenal tumors in children.

Apart from neuroblastomas, adrenal tumors are exceedingly rare in children and young adults. In this age group, the vast majority of patients present with clinical signs associated with excess hormone production. The most common tumor to arise from the adrenal cortex is an adrenocortical carcinoma (ACC). Similar to the situation in adults, this tumor is frequently diagnosed at a late stage and carries a very poor prognosis. ACCs require extensive/aggressive local resection followed by mitotane chemotherapy. A multidisciplinary approach is essential, and these children should be referred to uni

Corticomedullary mixed tumor of the adrenal gland.

Corticomedullary mixed tumors of the adrenal gland are quite rare, with only five well-documented cases reported in the literature.(1-4) Herein, we report the light microscopic and immunohistochemical features of two cases of this rare tumor. Patient 1 is a 34-year-old woman who presented with hypertension, hair loss, and amenorrhea of 1-year duration. Patient 2 is a 52-year-old woman who presented with flank pain and what appeared to be a renal mass on arteriogram with no history of hypertension, Cushing's syndrome, or other endocrine abnormalities. At surgery, the tumor was noted to arise fr
