Ethical and Clinical Considerations in the Workup of HIV-Associated Neurocognitive Decline.

Human immunodeficiency virus (HIV) can invade the central nervous system, leading to a spectrum of neurocognitive disorders known as HIV-ass … ver, even when clinical pathways are evident, stigma, fractured family dynamics, and the absence of advance directives can obstruct timely and appropriate care. Optimal management of HIV-associated neurocognitive decline extends beyond biomedical treatment. A stigma-informed, ethically sensitive approach, including early counseling, advance care planning, surrogate …

A plea for end-of-life discussions with patients suffering from Huntington's disease: the role of the physician.

Euthanasia and physician-assisted suicide (PAS) by request and/or based on an advance directive are legal in The Netherlands under strict conditions, thus providing options for patients with Huntington's disease (HD) and other neurodegenerative diseases to stay in control and choose their end of life. HD is an inherited progressive disease characterised by chorea and hypokinesia, psychiatric symptoms and dementia. From a qualitative study based on interviews with 15 physicians experienced in treating HD, several ethical issues emerged. Consideration of these aspects leads to a discussion about

"Choice" in end-of-life decision making: researching fact or fiction?

PURPOSE: The contribution of bioethics to clinical care at the end of life (EOL) deserves critical scrutiny. We argue that researchers have rarely questioned the normative power of autonomy-based bioethics practices. Research on the ethical dimensions of EOL decision making has focused on an idealized discourse of patient "choice" that requires patients to embrace their dying to receive excellent palliative care. DESIGN AND METHODS: Our critique is based on a comprehensive review of empirical research exploring bioethics practices at the EOL. In addition we will provide a brief review of our

[Consideration of patient autonomy at the end of life : End of life care between the protection of life and the right to self-determination].

A person's rights to freedom and self-determination are a valuable asset. At the same time, the state has an obligation to protect life. Tension can arise between these equally important, immutable guarantees in Articles 1 and 2 of the German Basic Law if a person is suffering from severe pain due to an illness and there is no prospect of a cure. If a patient in this situation expresses the wish to die, this represents a major challenge for those providing treatment. Personal conflicts of conscience and uncertainty about the legal situation mean that the patient does not receive the treatment

Pulmonary management of the ALS patient.

Amyotrophic lateral sclerosis (ALS) is a chronic progressive motor neuron disease with a poor prognosis which eventually weakens and paralyzes the respiratory muscles. ALS is characterized by progressive degeneration of both cortical and alpha motor neurons of the final common pathway. Early symptoms usually begin with alpha motor neuron involvement and then progress to include cortical motor neuron involvement. Degeneration of respiratory nerve centers in the anterior horn at the C3-C5 levels results in respiratory muscle fatigue, respiratory failure and eventually death. Treatment consists o

Advance Medical Decision-Making Differs Across First- and Third-Person Perspectives.

BACKGROUND: Advance healthcare decision-making presumes that a prior treatment preference expressed with sufficient mental capacity ("T1 preference") should trump a contrary preference expressed after significant cognitive decline ("T2 preference"). This assumption is much debated in normative bioethics, but little is known about lay judgments in this domain. This study investigated participants' judgments about which preference should be followed, and whether these judgments differed depending on a first-person (deciding for one's future self) versus third-person (deciding for a friend or str

Palliative Care and Quality Outcomes in Patients With Brain Metastases and Poor Prognosis: A Multi-Institutional Analysis.

PURPOSE: To analyze the patterns of palliative care (PC) consultation for patients with brain metastases (BMETs) and its association with treatment, overall survival (OS), and quality metrics (eg, advance directives [ADs], hospice enrollment). METHODS: We performed a multi-institutional retrospective quality review of consecutive patients diagnosed with BMETs from 2017 to 2019. We collected patient demographics, cancer diagnosis, performance status, treatment, PC consultation, emergency department utilization, AD completion, hospice enrollment, and OS data. We included patients with a predict…

Informed choice, resident-centered care decisions, and the review of living wills and/or advance directives are essential in the decision-making process. After enteral nutrition is started, healthcare practitioners must carefully review the physician's orders and administer and monitor the resident's tolerance, checking for potential complications.

Implementing recovery-oriented interventions with staff in a psychiatric hospital: A mixed-methods study.

WHAT IS KNOWN ON THE SUBJECT?: Recovery-oriented care is a means of providing mental health treatment, focused on the patient's individual needs and active involvement in one's own care. However, this approach presents with challenges, particularly in psychiatric hospitals, which tend to be focused on symptom reduction. WHAT DOES THE PAPER ADDS TO EXISTING KNOWLEDGE?: This study examines the influence of three different recovery-oriented training programmes/interventions (namely, illness management and recovery, peer support, and psychiatric advance directives) on the attitudes and practice of…

Opportunities and challenges of joint crisis plans in mental healthcare: a qualitative interview study with mental health professionals.

BACKGROUND: Psychiatric advance directives (PADs) and joint crisis plans (JCPs) are documents that allow users of mental health services to state their preferences for treatment for a future situation in which they are unable to give consent. In Germany, both PADs and JCPs are legally binding in the context of mental healthcare. OBJECTIVES: The objective of this study was to examine mental health professionals' views on PADs and JCPs, identify challenges in their application in clinical practice, and derive recommendations for their implementation. METHODS: We conducted semi-structured inter…

Clinicians must navigate these dilemmas within diverse legal and cultural frameworks, where restrictive policies and societal attitudes toward suffering and death strongly influence practice.
