Phobic disorders and anxiety states: how do they differ?

A review of the clinical literature to date has shown that the nature of the relationship between phobic disorders and anxiety states is still unclear. As a wide range of symptoms are shared by patients with all DSM-III anxiety disorder diagnoses, at this stage there is still a need to investigate the latent dimensions which distinguish the anxiety disorder subtypes. In the present study 176 patients with the DSM-III diagnoses of agoraphobia with panic attacks, social phobia, panic disorder and generalized anxiety disorder completed the Fear Survey Schedule, Fear Questionnaire, Hostility and D…

Panic disorder and subthreshold panic in the light of comorbidity: a follow-up study.

Especially in the presence of agoraphobia and comorbid conditions, panic disorder causes significant impairment in life quality. Although there are several studies about epidemiology and clinical features, subthreshold symptoms and courses of comorbidity have not been studied sufficiently in panic disorder. The current study assessed the courses of panic disorder and subthreshold panic symptoms in consideration of the major and subthreshold comorbid conditions. Patients with Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition-diagnosed panic disorder were assessed using the p…

Is the Beck Anxiety Inventory a good tool to assess the severity of anxiety? A primary care study in the Netherlands Study of Depression and Anxiety (NESDA).

BACKGROUND: Appropriate management of anxiety disorders in primary care requires clinical assessment and monitoring of the severity of the anxiety. This study focuses on the Beck Anxiety Inventory (BAI) as a severity indicator for anxiety in primary care patients with different anxiety disorders (social phobia, panic disorder with or without agoraphobia, agoraphobia or generalized anxiety disorder), depressive disorders or no disorder (controls). METHODS: Participants were 1601 primary care patients participating in the Netherlands Study of Depression and Anxiety (NESDA). Regression analyses …

Panic attacks and panic disorder: the great neurologic imposters.

Patients who experience panic attacks, panic disorder, or agoraphobia have significant impairment associated with their disorders. The cost to society in health care costs as well as the human suffering and mortality is high and may be even higher than necessary because of misdiagnosis and inadequate treatment of these patients. Although we do not have many answers in the areas of pathophysiology or neurochemistry of panic disorder, we do have effective treatments for panic disorder and agoraphobia. If these are conceptualized as distinct disorders with specific symptoms, making a diagnosis of…

A cluster analysis of early onset in common anxiety disorders.

Early onset is regarded as an important characteristic of anxiety disorders, associated with higher severity. However, previous findings diverge, as definitions of early onset vary and are often unsubstantiated. We objectively defined early onset in social phobia, panic disorder, agoraphobia, and generalised anxiety disorder, using cluster analysis with data gathered in the general population. Resulting cut-off ages for early onset were ≤22 (social phobia), ≤31 (panic disorder), ≤21 (agoraphobia), and ≤27 (generalised anxiety disorder). Comparison of psychiatric comorbidity and general wellbei…

Hyperventilation and anxiety in panic disorder, social phobia, GAD and normal controls.

Patients with DSM-III Agoraphobia, Panic Disorder, GAD, Social Phobia and normal controls underwent a series of experimental procedures and measures to determine whether panic attack patients show a greater tendency towards hyperventilation that is independent from their anxiety levels. Contrary to expectations, the Agoraphobia and Panic Disorder patients did not show significantly lower levels of expired pCO2 at rest than the other anxious or non-anxious groups. However, the panic attack patients did show significantly higher levels of anxiety and hyperventilatory symptoms during a hyperventi…

The Oxford Agoraphobic Avoidance Scale.

BACKGROUND: Agoraphobic avoidance of everyday situations is a common feature in many mental health disorders. Avoidance can be due to a variety of fears, including concerns about negative social evaluation, panicking, and harm from others. The result is inactivity and isolation. Behavioural avoidance tasks (BATs) provide an objective assessment of avoidance and in situ anxiety but are challenging to administer and lack standardisation. Our aim was to draw on the principles of BATs to develop a self-report measure of agoraphobia symptoms. METHOD: The scale was developed with 194 patients with …

Specificity of lactate infusions in social phobia versus panic disorders.

Lactate infusions produced panic attacks in one of 15 patients with social phobia, four of nine with agoraphobia, and 10 of 20 with panic disorder in a blind study. The proportion of patients with social phobia who panicked in response to lactate was significantly lower than that of patients with agoraphobia or panic disorder. These findings lend validity to the DSM-III classification of anxiety states. They also suggest that the pathophysiology of social phobia differs from that of disorders characterized by spontaneous panic attacks.

Treatment of spontaneous panic attacks with chlomipramine.

Recurrent spontaneous panic attacks are psychophysiological symptoms present in panic disorder and agoraphobia with panic attacks. Effective pharmacological treatment for panic attacks is essential to any treatment plan for these conditions. The authors administered chlomipramine to 20 patients who had panic disorder or agoraphobia with panic attacks, and after 8 weeks of treatment 75% of the patients with each diagnosis were asymptomatic. These results support the claim that chlomipramine is highly effective in preventing recurrence of panic attacks.

A psychometric reanalysis of the Albany Panic and Phobia Questionnaire.

The psychometric properties of the 27-item Albany Panic and Phobia Questionnaire (APPQ) were evaluated in 1930 outpatients with DSM-IV anxiety and mood disorders. Although prior findings of a 3-factor latent structure were upheld in several replications (Social Phobia, Agoraphobia, Interoceptive), three items failed to load on their predicted factor (Interoceptive). Multiple-groups CFAs indicated that the measurement properties of the APPQ were invariant in male and female patients, with the exception of an intercept of one item from the Agoraphobia scale which evidenced bias against females. …

Coping with anxiety and panic: a factor analytic study.

OBJECTIVES: This study aimed to explore the coping styles that agoraphobia sufferers adopt when attempting to cope with symptoms of anxiety and panic. It aimed to extend Watts's (1989) Coping with Anxiety Questionnaire (CAQ) by including items to assess self-vigilance. It was hypothesized that agoraphobia sufferers would adopt consistent coping styles that would be related to symptom severity. DESIGN: A factor analysis was performed on questionnaire data. METHOD: A postal questionnaire was completed by members (N = 112) of a self-help group for agoraphobia and panic sufferers. All participan…

Panic attacks. A review of treatments and pathogenesis.

In the psychiatric literature, panic attacks have been considered as part of the clinical manifestations of anxiety neurosis, agoraphobia, functional cardiovascular disturbances, and the phobic depersonalization syndrome. Even though recurrent spontaneous panic attacks are described in these entities, the importance ascribed to them has been minor one. Therefore, panic attacks are poorly understood from a psycho-physio pathological point of view. In the past years, new trends in the nosology of psychiatric disorders have grouped the recurrent spontaneous panic attacks under the heading "panic …

Agoraphobics 5 years after imipramine and exposure. Outcome and predictors.

Five years after treatment in a controlled trial, in which all had received self-exposure homework, a group of 40 agoraphobic outpatients retained marked improvement in agoraphobia, mood, and free-floating anxiety. Frequency of spontaneous panics decreased as much in those who had placebo and self-exposure as in those who received imipramine and self-exposure. Few patients, however, were completely well at 5 years and over half had received further treatment for agoraphobia during the follow-up. Patients who were still highly phobic at the end of the clinical trial were more often prescribed p…

Individualization: the categorical imperative of behavior therapy practice.

The fact that unadaptive neurotic anxiety response habits are learned makes it necessary for the stimulus antecedents to be accurately defined in every case in order to plan effective treatment. This requirement has been frequently neglected in recent years that have witnessed a growing tendency to apply common treatments to cases with common labels, e.g. agoraphobia. This paper argues for the desirability of accurate individual diagnosis, and illustrates its benefits in unique cases and also in respect of three common syndromes--agoraphobia, panic attacks and depression.

Space phobia: syndrome or agoraphobic variant?

Four elderly women had intense fears of falling when there was no visible support at hand or on seeing space cues while driving. Two patients had cervical spondylosis. The mean age at onset of the fear was 54--thirty years later than that for agoraphobia. Fear of public places and of heights was not prominent, nor was depersonalisation or depression. These "space phobias" might be a hitherto unrecognised syndrome or an unusual variant of agoraphobia. The visuospatial reflexes involved might illuminate the pathogenesis of certain fears.

Effectiveness of cognitive-behavioral group therapy for panic disorder in a specialized unit.

INTRODUCTION: The efficacy of cognitive-behavior therapy for panic disorder (PD) with or without agoraphobia is well established, but few data exist on its effectiveness using a group format. The goal of the present study was to assess the effectiveness of group CBT in a sample of PD patients in a specialized unit. METHODS: Treatment consisted of nine weekly group sessions. Patients with PD (n=56) were assessed at baseline, after the treatment and in one and three-months followups. RESULTS: There were significant reductions in panic/agoraphobia symptoms and related variables between baseline…

Space and motion discomfort in Brazilian versus American patients with anxiety disorders.

Space and motion discomfort (SMD) was studied in 38 Brazilian and 50 U.S. patients belonging to one of three diagnostic groups: (a) panic disorder with agoraphobia, (b) panic disorder without agoraphobia, and (3) other nonpanic anxiety disorders. A group of 30 U.S. normal controls was also included. SMD was assessed by the Situational Characteristics Questionnaire (SitQ), which includes two scales for SMD--the Smd1 and the Smd2, and one scale for non-space-related agoraphobic discomfort, the Ag1. The score in the Smd2 is based on the sum of Likert style items, while the scores of the Smd1 and …

Virtual reality exposure therapy does not provide any additional value in agoraphobic patients: a randomized controlled trial.

BACKGROUND: A number of studies have demonstrated the efficacy of virtual reality exposure therapy (VRET) in specific phobias, but research in seriously impaired patients with agoraphobia is lacking. In this randomized controlled trial with patients with agoraphobia and panic disorder, VRET and exposure in vivo were compared in terms of outcome and processes involved. METHODS: Patients with panic disorder with agoraphobia (n = 55) were randomly assigned to receive 4 sessions of cognitive behavioral therapy (CBT) followed by either 6 sessions of VRET or 6 sessions of exposure in vivo or to a w…
