Decreased expression of transporters reduces folate uptake across renal absorptive surfaces in experimental alcoholism.

In this study, we examined the mechanistic insights of folate reabsorption during alcoholism, considering enhanced renal excretion as one of the major contributing factors to alcohol-induced folate deficiency. Male Wistar rats were fed 1g/kg body weight/day ethanol (20% solution) orally for 3 months. The results on characterization of the folate transport system in renal basolateral membrane (BLM) suggested it to be a carrier-mediated, acidic pH-dependent and saturable one. Chronic ethanol feeding decreased the uptake mainly by increasing the K (m) and decreasing the V (max) of the transport p…

Follow-up study of anxiety disorder and alcohol dependence in comorbid alcoholism treatment patients.

BACKGROUND: Anxiety disorders are present in a high percentage of alcoholism treatment patients. We tested the prediction that having a comorbid anxiety disorder increases the prospective risk for relapse to drinking after alcoholism treatment. We also explored the prospective associations of specific anxiety syndromes (and depression) with drinking and anxiety outcomes. METHODS: We assessed the diagnostic status and daily drinking patterns of 82 individuals approximately one week after they entered alcoholism treatment (baseline) and again approximately 120 days later (follow-up) (n=53). RE…

Gender differences in prevalence, risk, and clinical correlates of alcoholism comorbidity in bipolar disorder.

OBJECTIVE: The prevalence of lifetime alcohol abuse and/or dependence (alcoholism) in patients with bipolar disorder has been reported to be higher than in all other axis I psychiatric diagnoses. This study examined gender-specific relationships between alcoholism and bipolar illness, which have previously received little systematic study. METHOD: The prevalence of lifetime alcoholism in 267 outpatients enrolled in the Stanley Foundation Bipolar Network was evaluated by using the Structured Clinical Interview for DSM-IV. Alcoholism and its relationship to retrospectively assessed measures of …

Impact of family history of alcoholism on cocaine-induced subjective effects and pharmacokinetic profile.

RATIONALE: The importance of genetic factors in the development of alcoholism has been demonstrated repeatedly. However, the impact of a family history of alcoholism on the development of other drug use has been less thoroughly studied. OBJECTIVE: The present study was conducted to investigate whether individuals with a positive family history of alcoholism (FHP) differ from individuals without a history (FHN) in their pharmacokinetic profile, subjective and physiological response to an acute intranasal dose of cocaine (0.9 mg/kg). METHODS: Nine FHP and nine FHN male occasional cocaine users…

The effect of comorbid alcoholism on recurrence in affective disorder: a case register study.

BACKGROUND: Studies of the effect of comorbid alcoholism on the risk of recurrence in affective disorder have given contradictory results. METHOD: Using survival analysis, the rate of recurrence was calculated in a case register study including all hospital admissions with primary affective disorder in Denmark during 1971-1993. The rate of recurrence was estimated following each new affective episode. RESULTS: In all, 20 350 patients were discharged after first admission with a main diagnosis of affective disorder of depressive or manic/circular type. Among these, 518 patients (2.6%) had an …

[Social psychiatry aspects of alcoholism: epidemiology, treatment structures and recent results of therapy research].

In Germany the per capita consumption of alcohol has tripled since World War II with a slight decrease in recent years. The number of alcoholics has been estimated between 2.5 mill. and 10 mill. Epidemiological studies suggest a 6-month prevalence rate of 3.1% (USA 2.9%): 10.4% in males, 0.8% in females; Life-time prevalence estimates range up to 13%. More recent findings also suggest that alcoholism in adolescents is an increasing problem. The resources for treatment of alcoholism are limited: Only 1% of all alcoholics are treated in special addiction rehabilitation programmes, whereas the ma…

Effects of disulfiram on positron emission tomography and neuropsychological studies in severe chronic alcoholism.

Disulfiram is an aldehyde dehydrogenase inhibitor that is widely used as an adjunctive agent in the treatment of patients with severe chronic alcoholism. Recent positron emission tomography (PET) studies of local cerebral metabolic rates for glucose (ICMRglc) and benzodiazepine receptor binding in alcoholic patients have shown regional cerebral abnormalities; however, some of the patients were studied while receiving disulfiram, which could influence the biochemical processes under investigation. In a retrospective investigation, we examined the influence of disulfiram administration on the re…

[Search for biochemical factors implicated in the pathogenesis of alcoholism in different ethnic populations].

A Chukotka indigenous population (the Chukchis, Eskimos) craving alcohol was examined. Changes in the levels of endogenous ethanol (EE) and its relation to calcium homeostasis, hypophyseal-adrenocortical hormones (stress) in the body are regarded as a possible role in alcohol motivation and ethanol dependence. The frequency and pattern of alcohol consumption, high alcoholism incidence rates in the family history may consider the Arctic Mongoloids as a group at high risk for alcoholism. Manifestations, such as lost control over the dose used, alcoholic amnesia, occasional use of alcohol poisoni…

The diagnosis of dementia associated with alcoholism: a preliminary report of a new approach.

The historical, clinical, neurological and neuropsychological features of 13 subjects with independently diagnosed dementia associated with alcoholism (AlcD) were compared to 13 subjects with probable Alzheimer's disease (AD), matched for age and severity of dementia. Neurological abnormalities were present in all the subjects with AlcD even though the diagnosis of Wernicke-Korsakoff was recorded in only one of these subjects. Only one subject with probable AD demonstrated any neurological abnormality. There was no difference between the AD and AlcD subjects in either the total scores of the M…

Self-regulation and self-medication factors in alcoholism and the addictions. Similarities and differences.

Addicts and alcoholics suffer vulnerabilities and deficits in self-regulation. A principal manifestation of their self-regulation disturbances is evident in the way they attempt to self-medicate painful affect states and related psychiatric problems. Individuals select a particular drug based on its ability to relieve or augment emotions unique to an individual which they cannot achieve or maintain on their own. Addicts and alcoholics usually experiment with all classes of drugs, but discover that a particular drug suits them best. Usually, painful affect states interact with other problems in

Characteristics of the memory loss of a patient with Wernicke-Korsakoff's syndrome without alcoholism.

The memory loss in alcoholic Wernicke-Korsakoff syndrome has been well characterized. However, it has been suggested that some of the neuropsychological defects seen in these patients are due to frontal lobe dysfunction resulting from chronic alcohol abuse. The present report details the nature and extent of the amnesia in a Wernicke-Korsakoff patient who did not have a history of alcoholism. In spite of her normal performance on measures of frontal lobe function, this patient showed many characteristics similar to those seen in alcoholic Wernicke-Korsakoff syndrome including a graded loss of …

[Effect of chronic alcoholism on the state of the microcirculatory bed of the myocardium].

Ultrastructure of myocardial capillaries of rats was studied in cases of chronic alcohol intoxication, experimental alcoholic cardiomyopathy (ACM) and its correction with antioxidants (vitamin E, dibunol). Alterations in the microcirculatory bed were similar in all groups of animals irrespective of disturbances in cardiomyocytes. Cardiomyocyte ultrastructure was improved after treatment with antioxidants, but capillary bed was the same as in untreated animals. High dibunol doses caused the onset of perivascular sclerosis. Disturbances in the microcirculatory bed are, probably, the first step i

[Clinical features of early alcoholism in adolescents with a slight handicap of exogenous etiology].

On the basis of clinico-psychopathological and experimental-psychological examination of male children and adolescents with general mental retardation of exogenous etiology and the signs of alcoholism of different severity, the authors describe the diagnostic importance of additional symptoms of alcoholism. They also consider correlations between the age at the beginning of systematic alcohol abuse and the clinical picture of the first states of intoxication with the clinico-physiological form of oligophrenia. The data on the psychopathological manifestations of the major syndromes of early al…

[Increased efficacy of teturam combined with L-DOPA in experimental alcoholism in the rat].

In an experiment on Wistar male rats the authors studied the effects of teturam, diethyldithiocarbamate and Esperale in combination with L-DOPA on the consumption of alcohol and metabolism of biogenic amines in the brain. The use of L-DOPA prolonged the inhibitary effect of teturam on alcohol consumption. The administration of L-DOPA following Esperale implantation prevented the development of the abstinence syndrome. An increase in the efficacy of teturam and Esperale under the effect of L-DOPA was attended by elevated synthesis of cerebral noradrenaline.
