Tarlov cyst with self-healing cauda equina syndrome following combined spinal-epidural anesthesia: a case report.

BACKGROUND: Cauda Equina Syndrome (CES) after Combined Spinal-Epidural Anesthesia (CSEA) is a rare disease that most of the time need surgery to relieve spinal cord compression. CASE PRESENTATION: A 34-year-old male patient underwent a procedure for prolapse and hemorrhoids (PPH) under CSEA. Anesthesia and surgery were uneventful. However, the patient gradually experienced urinary retention, lower abdomen and back pain, changes in bowel habits and neurological dysfunction of the lower limbs when the catheter was removed. It was later determined that the patient had Tarlov cyst at the left S1

Lengthy complex lumbar fusion surgery in high-risk elderly patient under spinal anesthesia: A case report.

INTRODUCTION: Spinal Anesthesia (SA) continues to be an emerging technique for lumbar fusion surgery in the elderly population. SA is an appealing option in the high-risk geriatric population for several reasons, including the potential for reduced systematic stress, reduced blood loss, and reduced post-operative delirium. The safe limits of spine surgery under SA remain undetermined. PRESENTATION OF CASE: The following case-study describes an elderly high-risk patient (ASA III) with severe spinal stenosis and degenerative scoliosis who presented with lower back and right leg pain and underwe

Hemodynamic effects of lateral tilt before and after spinal anesthesia during cesarean delivery: an observational study.

BACKGROUND: Post-spinal hypotension is a common maternal complication during cesarean delivery. Aortocaval compression by the gravid uterus has been assumed as a precipitating factor for post-spinal hypotension. The role of left lateral tilting position in improving maternal cardiac output after subarachnoid block (SAB) is unclear. The aim of this work is to investigate the effect of left lateral tilting on maternal hemodynamics after SAB. METHODS: A prospective observational study was conducted including 105 full term pregnant women scheduled for cesarean delivery. Mean arterial pressure, he

Low-dose bupivacaine with sufentanil prevents hypotension after spinal anesthesia for hip repair in elderly patients.

BACKGROUND: Hip fracture is common in the geriatric population. Patients in this group are often at high risk for perioperative complications from concurrent diseases. Conventional spinal anesthesia can be associated with hypotension but has a better postoperative outcome compared to general anesthesia. We judged that a reduced dose of bupivacaine in combination with sufentanil could give reliable blocks with minimal hypotension. METHODS: Fifty elderly patients were randomized into two groups. The study group received spinal anesthesia as a combination of hyperbaric bupivacaine 7.5 mg and suf

Coronary artery plaque disruption as cause of acute myocardial infarction during cesarean section with spinal anesthesia.

A 31-year-old parturient delivered twins at 35 weeks' gestation by cesarean section with spinal anesthesia. Following anesthesia induction, hypotension and bradycardia occurred, and were immediately treated with theodrenaline plus cafedrin (Akrinor) and atropine. Blood pressure and heart rate increased to 180/100 mmHg and 140 beats per minute, respectively. Several minutes later, the patient developed a myocardial infarction (MI) that she survived after intensive care treatment without sequelae. Although the coronary angiography showed normal coronary vessels, an intravascular ultrasound study

Preoperative inferior vena cava-abdominal aorta ultrasound examination to guide the positioning of spinal anesthesia to reduce post-spinal hypotension: a prospective, randomized trial.

BACKGROUND: The effectiveness of a 15° left-lateral tilt for alleviating the inferior vena cava (IVC) compression caused by a gravid uterus has been questioned. This study assessed the benefits of conducting pre-spinal anesthesia IVC-abdominal aorta ultrasound examinations and utilizing the IVC to abdominal aorta (IVC/Ao) ratio as a guide for post-spinal anesthesia positioning. METHODS: 200 parturients undergoing cesarean section were randomized 1:1 into an ultrasound-guided and a control group (groups U and C, respectively). The parturients in group C were positioned with a 15° left-lateral

Transition from failed epidural labor analgesia to combined spinal-epidural anesthesia: What to do when encountering difficulties in spinal cord puncture? A case report.

Although labor epidural analgesia can effectively alleviate pain during childbirth, some women may experience a decrease in the effectiveness of the initially effective epidural analgesia or a complete loss of effectiveness (disappearance of the epidural block level) during labor. For cases where the analgesic effect is insufficient after the administration of additional medication or where the epidural block level is completely lost, it is recommended to replace the epidural catheter. When reinserting the epidural catheter, combined spinal-epidural anesthesia should be considered for labor an

Impact of Spinal Anesthesia Dosage in Elective Cesarean Section on the Duration of Stay in Post-Anesthesia Care Unit at the Women's Health Hospital, National Guard Health Affairs.

In the field of obstetrics, cesarean sections have now become the most prominent procedure for the delivery of newborns. Cesarean sections may be handled with a variety of different anesthetic approaches, of which most focus seems to be on that of spinal forms, due to their rapid and effective action. Dosages of spinal anesthesia formulations differ depending on multiple variables, such as depth of anesthesia, level of analgesia, and desired duration of effects. Furthermore, length of stay in the hospital is also a crucial variable to take into account when using spinal anesthesia due to its i

Selective spinal anesthesia with hyperbaric prilocaine provides better perioperative pain control than local anesthesia for ambulatory inguinal hernia repair without affecting discharging time: a randomized controlled trial.

PURPOSE: Local anesthesia is the most used anesthetic technique for inguinal hernia repair, despite its unpredictability. Selective spinal anesthesia with a short-term local anesthetic guarantees rapid recovery, predictable duration and low incidence of side effects. We tried to assess the efficacy of this neuraxial technique in ambulatory setting. METHODS: One hundred thirty-two ASA I-III, aged > 18 patients scheduled for inguinal hernia repair have been randomized into two groups receiving unilateral spinal anesthesia with 40 mg of hyperbaric prilocaine (group A) or local anesthesia with me

Spinal Anesthesia with Targeted Sedation based on Bispectral Index Values Compared with General Anesthesia with Masked Bispectral Index Values to Reduce Delirium: The SHARP Randomized Controlled Trial.

BACKGROUND: Reducing depth of anesthesia and anesthetic exposure may help prevent delirium, but trials have been conflicting. Most studies were conducted under general anesthesia or in cognitively impaired patients. It is unclear whether reducing depth of anesthesia beyond levels consistent with general anesthesia reduces delirium in cognitively intact patients. The authors' objective was to determine whether a bundled approach to reduce anesthetic agent exposure as determined by Bispectral Index (BIS) values (spinal anesthesia with targeted sedation based on BIS values) compared with general

Dose-Response Study of Phenylephrine for Preventing Spinal-Induced Hypotension During Cesarean Delivery with Combined Spinal-Epidural Anesthesia Under the Effect of Prophylactic Intravenous Ondansetron.

BACKGROUND: Ondansetron reduces the median effective dose (ED50) of prophylactic phenylephrine to prevent spinal-induced hypotension (SIH) during cesarean delivery. However, the exact dose response of phenylephrine in combination with prophylactic ondansetron for preventing SIH is unknown. Therefore, this study aimed to determine the dose-response of phenylephrine to prevent SIH in cesarean delivery when 4 mg of ondansetron was used as a preventive method. METHODS: A total of 80 parturients were enrolled and divided randomly into four groups (n = 20 in each group) who received either 0.2, 0.3

The ED50 and ED95 of Prophylactic Norepinephrine for Preventing Post-Spinal Hypotension During Cesarean Delivery Under Combined Spinal-Epidural Anesthesia: A Prospective Dose-Finding Study.

Background: Hypotension commonly occurs with spinal anesthesia during cesarean delivery. Norepinephrine is an alternative to phenylephrine which can be used to prevent or treat hypotension, with better maintained cardiac output and less bradycardia. However, an appropriate initial prophylactic infusion dose of norepinephrine remains unclear. The aim of this study was to describe the dose-response relationship of prophylactic norepinephrine infusion during cesarean delivery under combined spinal-epidural anesthesia. Methods: We performed a prospective, randomized, double-blinded dose-finding st

Clinical Application and Safety of Intrathecal Adjuvants in Single-Shot Thoracic Segmental Spinal Anesthesia: A Narrative Review.

Thoracic segmental spinal anesthesia (TSSA) is an emerging regional anesthesia (RA) technique. TSSA with local anesthetic (LA) alone is associated with a relatively short duration of surgical anesthesia and postoperative analgesia, which may limit its clinical utility. Intrathecal adjuvants have been widely used in lumbar spinal anesthesia (LSA) to prolong block duration and improve block quality. However, their role in TSSA remains poorly defined. This review summarizes commonly used intrathecal adjuvants during TSSA, their doses, adverse effects, and safety considerations. A comprehensive li

Haptic Virtual Reality Training for Combined Spinal-Epidural Anesthesia in Anesthesiology Trainees: Prospective Randomized Controlled Pilot Trial.

BACKGROUND: Combined spinal-epidural (CSE) anesthesia is a complex psychomotor skill requiring precise tactile discrimination. Traditional manikin-based training may be limited by degraded haptic fidelity and inconsistent tissue feedback. Whether skills acquired through haptic virtual reality (VR) training transfer to supervised clinical procedures remains uncertain. OBJECTIVE: This study assessed the feasibility and early clinical application of CSE skills after high-fidelity haptic VR training in junior anesthesiology trainees vs traditional manikin-based training. METHODS: We conducted a
