Perioperative complications of intraventricular neuroendoscopy: a 7-year experience.

AIM: Despite being a minimally invasive procedure, serious perioperative complications are reported during neuroendoscopy, largely generated by its unique surgical maneuvers. We report here the complications of elective neuroendoscopic surgery for the treatment of hydrocephalus and other intraventricular pathology in 298 patients over a 7-year period at our institute. MATERIAL AND METHODS: The complication rate was determined by recording intraoperative hemodynamic variables, core temperature, bleeding episodes, time to arouse from anesthesia, serum electrolytes and neurological deterioration

A physiologically based, recirculatory model of the kinetics and dynamics of propofol in man.

BACKGROUND: The disposition of propofol in man is commonly described using a three-compartment mamillary model. However, these models do not … may ultimately find a role in improving the fidelity of patient simulators currently used to train anesthetists and for clinical practice simulation to optimize dosing and management strategies.

[Problems in anesthesia in pulmonary cystic adenomatosis].

Congenital cystic adenomatoid malformation (CCAM) of the lung is a rare disease. It is about an abnormal proliferation of mesenchymal elements and failure of maturation of bronchiolar structures, characterized by the replacement of normal pulmonary tissue with "cysts" in variable size and number. These lesions communicate with the tracheobronchial tree. During fetal period hydrops and polyhydramnios can be associated with CCAM. A cystic adenomatoid malformation can be detected by antenatal ultrasound, and, at the birth, it is confirmed by chest radiography. From January 1990 to December 1998,

Exploratory Analysis of Impact of Gabapentin on Incidence of Postoperative Nausea and Vomiting in Patients Undergoing Knee and Hip Arthroplasty With Neuraxial Anesthesia.

The incidence of postoperative nausea and vomiting (PONV) is unknown in neuraxial anesthesia for orthopedic surgery. The effect on PONV of adding gabapentin to an evidence-based antiemetic regimen as part of an opioid-sparing analgesic protocol is also unknown in this population. A retrospective analysis of all adults undergoing hip and knee arthroplasty and receiving neuraxial anesthesia in 2017 was conducted. The overall incidence of PONV was assessed. Additionally, PONV incidence was assessed for all combinations of gabapentin, dexamethasone, and/or ondansetron (in addition to propofol infu

[Comparison of five video laryngoscopes and conventional direct laryngoscopy : Investigations on simple and simulated difficult airways on the intubation trainer].

INTRODUCTION: Securing the airway with a tracheal tube is essential in emergency situations, in the intensive care setting as well as during anesthesia for surgery and other interventions. Current methods of airway assessment are poor screening tests for predicting difficult direct laryngoscopy due to a generally low positive predictive value; therefore, successful endotracheal intubation requires a high level of expertise, regular training and practice and sometimes additional tools. Currently, several video laryngoscopes (VL) with different designs are commercially available and have been in

Implications for postoperative visual loss: steep trendelenburg position and effects on intraocular pressure.

Postoperative visual loss (POVL) is a rare but catastrophic complication after nonocular surgery. Previously POVL has been reported in lengthy, prone, lateral, or cardiopulmonary cases, with extreme blood loss, hemodilution, and hypotension. The author's index case of POVL following a lengthy operation in steep Trendelenburg position (ST) prompted study of the relationship between intraocular pressure (IOP), mean arterial pressure (MAP), and time spent in ST. A 3-year investigation of the relationship between IOP and ST procedures is reported. Ocular perfusion pressure (OPP) was calculated fro

The nurse anaesthetist.

With the ongoing debate on New Ways of Working in the National Health Service, the role of nurses and other perioperative practitioners in anaesthetics is becoming an increasingly prominent issue. The opening up of new anaesthetic roles to nurses and their non-medical colleagues raises questions as to their own acceptance of these new roles and their acceptance by their medical colleagues. In this article, the author addresses the question as to whether perioperative practitioners are ready for the nurse anaesthetist, and whether the UK as a whole is ready for this initiative.
