We report a rare case of a vertebral arteriovenous fistula presenting with subarachnoid hemorrhage (SAH). A 60-year-old man was admitted to our hospital with a sudden onset of headache and neck pain. A neurological examination showed no abnormalities. Computed tomography scans revealed SAH in the pontine cistern and cistern magna. Although the first cerebral angiogram failed to depict the cause of bleeding, the second angiogram on day 15 demonstrated an arteriovenous fistula in the left vertebral artery at C4-5, which was draining into the internal vertebral venous plexus and forming a venous

BACKGROUND: Predicting the success of endovascular recanalization in non-acute internal carotid artery occlusion (NICAO) remains a challenge.
OBJECTIVE: To examine the hypothesis that the concentric ring sign observed on high-resolution magnetic resonance vessel wall imaging (MR-VWI) could serve as a potential imaging biomarker to improve the accuracy of predicting recanalization success and guide treatment decisions.
METHODS: A retrospective analysis was conducted on patients with NICAO who underwent endovascular treatment at our institution between January 2020 and December 2023. Baseline

INTRODUCTION: Dural arteriovenous fistula (DAVF) is a vascular malformation. Although rare, DAVFs can represent a potentially reversible cause of dementia. Bilateral white matter changes can be detected on the magnetic resonance imaging of patients with DAVFs. Familiarity with symptoms of DAVFs can reduce the incidence of misdiagnosis of dementia syndrome. This study aimed to explore the clinical manifestation, imaging characteristics, and prognosis of dementia with bilateral white matter changes caused by DAVFs.

BACKGROUND: Glioblastoma is the most common primary malignant tumor of the brain. Common radiologic findings using initial computed tomography (CT) reveal an intra-axial lesion with perifocal edema. Here, we present a rare case of diffuse subarachnoid hemorrhage (SAH) detected on an initial CT image in a patient without intracranial aneurysm in whom the final diagnosis was glioblastoma.
CASE DESCRIPTION: We report the rare case of a 57-year-old man with glioblastoma in the right temporal lobe who presented with a sudden onset of disturbance of consciousness as an initial manifestation. Initia

OBJECTIVE: New vessels formation (NVF) along the steno-occlusive middle cerebral artery (MCA) is often observed in digital subtraction angiography (DSA) imaging. In the study, we aim to explore the clinical relevance of NVF in young ischemic stroke patients with isolated middle cerebral artery (MCA) stenosis.
METHODS: We retrospectively reviewed the digital subtraction angiography (DSA) images of 93 young ischemic stroke patients (age ≤ 45 years old) in our center from January 2006 to June 2016. All the patients were diagnosed with isolated steno-occlusive middle cerebral artery (MCA) disease

BACKGROUND: High-flow priapism is uncommon, and its association with bilateral cavernous pseudoaneurysms has seldom been reported.
CASE DESCRIPTION: This paper presents an exceedingly rare case of high-flow priapism resulting from bilateral cavernous pseudoaneurysms that developed following a straddle injury. The diagnosis was established through blood gas analysis of cavernosal aspirate, ultrasound, and magnetic resonance imaging (MRI). Digital subtraction angiography (DSA) revealed that both cavernous pseudoaneurysms were irrigated by the right internal pudendal artery. Following three mont

Digital subtraction angiography (DSA) is the most useful technique for diagnosing spinal arteriovenous malformations (AVM). In recent years, with the improvement of imaging capabilities, the usefulness of three-dimensional (3D) imaging by fusing various modalities has been recognized. The use of 3D fusion imaging with a workstation connected to an angiography system has been reported in many cases of intracranial disease, but less frequently for spinal AVM. In this article, we describe two illustrative cases of spinal AVM in which 3D fusion imaging was useful for treatment. Although 3D fusion

BACKGROUND: Periventricular collaterals are associated with high risk of hemorrhagic stroke in adult moyamoya disease (MMD). However, the clinical significance of the periventricular collateral enhancement sign (PCES), which indicates wall enhancement of periventricular collaterals on contrast-enhanced vessel wall imaging (VWI), has yet to be determined.
METHODS: Thirty-seven patients with MMD with acute neurological symptoms were consecutively recruited. Periventricular collaterals including lenticulostriatal artery, thalamic artery, and choroidal artery collaterals were evaluated on digital

Pulmonary arteries involvement is well described in Takayasu's arteritis (TA), a condition which is mainly associated with involvement of the systemic arteries. We report a case of TA with documented isolated pulmonary arteries involvement. Symptoms were quite similar to those encountered in chronic thromboembolic disease. A pulmonary angiogram showed bilateral stenosis and occlusion of pulmonary arteries. Diagnosis of TA was suspected, and as such a complete aortogram was made but proved to be normal. Massive haemoptysis suddenly occurred, which resulted in death. Autopsy disclosed characteri

Moyamoya syndrome is characterized by the reticulated collateralization of the intracranial vasculature distal to an occlusion of proximal intracranial vessels. In the present study this pathology was visualized in 2 patients, aged 29 and 32 years, using transcranial color Doppler imaging (TCDI). Digital subtraction angiography in both patients revealed stenosis of the intracranial portion of the internal carotid arteries, occlusion and stenosis of several cerebral arteries, and a bilateral reticulated collateral network particularly in the region of both basal ganglia, typical of moyamoya syn

Diagnostic accuracy of arteriovenous malformations (AVMs) is imperative for delineating management. The current standard is digital subtraction angiography (DSA). Arterial spin labeling (ASL) is an understudied noninvasive, non-contrast technique that allows angioarchitecture visualization and additionally quantifies cortical and AVM cerebral blood flow and hemodynamics. This meta-analysis aims to compare ASL and DSA imaging in detecting and characterizing cerebral AVMs. EMBASE, Medline, Scopus, and Cochrane databases were queried from inception to July 2022 for reports of AVMs evaluated by DS

BACKGROUND Symptoms caused by developmental venous anomalies (DVAs) are usually mild and unspecific. Despite the benign nature of DVAs, they can occasionally be symptomatic. CASE REPORT A 67-year-old woman presented with sudden diplopia and left eyelid ptosis for 10 days. A neurologic examination revealed left complete oculomotor nerve palsy. Other neurologic deficits, including eye pain or pulsatile tinnitus, were not detected. Furthermore, the visual acuity was normal. Additionally, no retinal hemorrhage, venous dilatation, or fundus tortuosity were observed. No ischemia lesions or neoplasms

The authors demonstrate the radiological anatomy and review the accepted embryological theories in a case of total craniopagus. These 24-year-old female cranially conjoined twins were studied with computerized tomography (CT) and CT angiography, magnetic resonance (MR) imaging and MR angiography as well as selective arterial digital subtraction (DS) angiography to clarify whether surgical separation was possible. The neuroradiological findings are discussed, taking into consideration both the embryological and surgical literature. The malformation was classified as a total parietooccipitotempo

PURPOSE: A persistent trigeminal artery is the most common persistent carotid-vertebrobasilar anastomosis. Persistent trigeminal artery variants (PTAVs) terminate in the cerebellar arteries without connecting to the basilar artery; of these, the anterior inferior cerebellar artery (AICA) is the most common. AICA duplication is frequently observed. However, to our knowledge, there have been no previous reports of PTAVs associated with duplicated AICAs. Here, we report a case of PTAV as a duplicate AICA, associated with the early bifurcation of the superior cerebellar artery and the aortic arch

INTRODUCTION: Subdural hematoma without subarachnoid hemorrhage secondary to intracranial aneurysm rupture is rare and may complicate patient management due to delay in diagnosis and subsequent treatment. Herein, we describe a case presenting with pure SDH secondary to the rupture of a posterior communicating artery infundibular dilatation (PcoA-ID). To the best of our knowledge, this is the first case of rupture of a PcoA-ID with SDH in the convexity and tentorium, which also tracked into the upper cervical spine along the subdural space. Additionally, we briefly discuss the previously publis

BACKGROUND: Fibrolipoma is an uncommon subtype of lipoma. On the basis of the results of a survey of the PubMed database, only approximately a dozen cases in children have been described in which the histological diagnosis of fibrolipoma has been confirmed. In children, fibrolipomas have been reported in the eyelids, ears, lips, nasopharynx, mouth, and other locations but not in the thoracic cavity. We present the case of a 15-year-old girl with a giant fibrolipoma in the left side of her thoracic cavity.
