Pars plana lentectomy for treatment of congenital cataract.

BACKGROUND: Congenital cataract surgery can be performed using a pars plicata/plana or a limbal approach, if placement of an intraocular lens at the time of cataract removal is not a consideration. Because of the high incidence of secondary cataract formation in children the operation should be combined with capsulotomy and anterior vitrectomy. METHODS: The series consisted of 30 eyes from 20 consecutive children who suffered from congenital cataract and underwent cataract surgery between May 1995 and June 2000. The inclusion criterion was congenital cataract affecting the visual axis. We per

Management of monocular congenital cataracts.

Monocular congenital cataracts have been treated for the past two decades with early surgery, contact lens correction and patching of the unaffected eye. While an occasional patient has had a good visual outcome with this treatment regiment, the majority end up being legally blind in the aphakic eye. Intraocular lenses (IOLs) are increasingly being used as an alternative means of optically correcting aphakia during infancy. A growing body of literature suggests that an IOL correction is associated with an improved visual outcome and a relatively low incidence of post-operative complications. A

Early treatment of congenital unilateral cataract minimizes unequal competition.

PURPOSE: Dense congenital unilateral cataracts may compromise visual development through visual deprivation and biased interocular competition, whereas dense congenital bilateral cataracts compromise visual development primarily through visual deprivation alone. Differences in sensory deficits between the two patient groups with these disorders may reflect the specific effects of unequal competition. To determine whether early treatment (at <8 weeks of age) minimizes the adverse effects of unequal competition, grating acuity deficits during the immediate posttreatment period and contrast sensi

The effectiveness of daily wear contact lenses for the correction of infantile aphakia.

OBJECTIVE: To evaluate the effectiveness and safety of daily wear contact lenses in the treatment of infantile aphakia. DESIGN: A study of prognosis using a cohort followed up for a mean of 58 months. SETTING: All operations, contact lens care, and follow-up were conducted at a university referral center. PATIENTS: Of 111 children undergoing surgery for cataract between 1980 and 1990, 51 (68 eyes) met the criteria of age younger than 2 years at the time of surgery and of cataract of nontraumatic origin. Patients were evaluated for visual acuity, complications, number of contact lenses worn

Secondary IOL power calculation. A comparison of an optical and a biometric method.

In 40 aphakic eyes undergoing secondary IOL implantation, the axial length was calculated optically as well as measured by ultrasound, and the two methods of axial length determination were compared to evaluate the accuracy in the calculation of secondary IOL power. The mean refractive prediction error (+/- SD) was 0.18 diopters (+/- 0.96) and -0.37 diopters (+/- 0.83) using the optical and the biometric method, respectively. The difference in mean value was significant, whereas the variation around the mean value was not found to be significantly different between the two methods. A calculate

Liquefaction of human vitreous in model aphakic eyes by 300-nm UV photolysis: monitoring liquefaction by fluorescence.

After the lens is removed in cataract surgery, the vitreous presumably receives all of the ambient 300-nm light that has filtered through the cornea. Using this model for aphakic eyes, we progressively irradiated intact vitreous samples of a 49-year-old human with 300-nm light and monitored changes in absorption, fluorescence, and circular dichroism (CD) properties. CD and fluorescence measurements of unirradiated vitreous samples showed a) a strong tryptophan fluorescence band of non-collagenous protein at 336 nm and a very weak band around 430 nm due to N-formylkynurenine (N-FK), a photoprod

Visual acuity in monocular pediatric aphakia: does epikeratophakia facilitate occlusion therapy in children intolerant of contact lens or spectacle wear?

Twenty-two children had a monocular cataract extracted between 1981 and 1986. Twelve of these patients eventually tolerated a monocular contact lens or anisometropic spectacle wear. Eleven patients initially would not tolerate conventional therapy, and epikeratophakia was performed. Of the 12 patients tolerating a contact lens or spectacles, eight (67%) now see 20/60 or better, and six (50%) see 20/40 or better. Only one (10%) of the patients maintaining the epikeratophakia graft developed vision as good as 20/70. Epikeratophakia did not facilitate occlusion therapy for amblyopia. The critical

Fuchs' heterochromic iridocyclitis in blacks.

Fuchs' heterochromic iridocyclitis (FHI) is a frequently overlooked cause of anterior uveitis. Improper diagnosis may lead to unnecessary therapy. Dark brown irides may demonstrate heterochromia poorly. We believe that FHI may be overlooked in blacks because of a frequent lack of obvious heterochromia and the frequent presence of iris nodules. Heterochromia is not necessary for the diagnosis of FHI if other clinical features of the syndrome are present. In our series of 54 whites and 13 blacks with FHI, heterochromia occurred in 50 (92%) whites and ten (76%) blacks (nonsignificant difference).

Fluorophotometric evaluation of ocular barriers and of the vitreous body in the aphakic eye.

8 aphakic patients were studied with fluorophotometry and 3-mirror examination of the vitreous body one month after cataract extraction. 4 were extracapsular with posterior chamber pseudophakos (ECCE) and 4 were intracapsular with anterior chamber pseudophakos (ICCE). A mathematical model is used to calculate blood-ocular fluorescein permeability (P) and vitreous diffusion coefficient (D) from fluorophotometric scans. All 30-minutes scans in the ECCE group and 2 in the ICCE showed a profile resembling that in phakic eyes, and P- and D-values were calculated. In the remaining two patients from

The potential damaging effects of light on the eye (Part II).

The electromagnetic spectrum consists of radiant energy that is classified according to specific wavelengths. Man is constantly exposed to a portion of the electromagnetic spectrum, particularly to radiation in the ultraviolet, visible and infrared regions. The tissues of the eye and skin are particularly sensitive to the non-ionizing wavelengths of radiant energy. Although both tissues possess endogenous protective mechanisms to minimize the effects of such exposure, problems still occur due to the absorption of radiation. The cornea, lens and retina have been shown to be susceptible to damag

[Complications and after care in aphakic and pseudophakic patients].

The aim of every cataract operation is optical rehabilitation. With the greater social demands made today by aphakic and pseudophakic patients, the recognition and treatment of diseases with early or late onset after cataract extraction assumes greater importance. The early aims are rapid mobilization, complete absence of irritation, and normalization of pressure. Regular administration of corticosteroids, essential in the post-operative treatment of pseudophakia, may mask a bacterial or mycotic infection. The sole indication for removal of an implant is the uveitis-glaucoma-hyphema syndrome.

Histopathology of a case of epikeratophakia (aphakic epikeratoplasty).

An epikeratoplasty button was removed by penetrating keratoplasty 31 months following an aphakic epikeratoplasty. The epithelium over the donor cap was thickened with numerous surface abnormalities. Bends and fractures in the donor Bowman's membrane were found throughout the length of the specimen. A significantly decreased number of keratocytes were found in the anterior half of the donor lenticule; the posterior half of the lenticule was acellular. The interfibrillar collagen distance of the donor lenticule was significantly increased compared with that of the recipient host cornea. The cent

Gram-negative corneal ulcers in elderly aphakic eyes with extended-wear lenses.

We report 13 cases of corneal ulcers caused by gram-negative bacteria. These cases occurred in aphakic eyes wearing hydrophilic contact lenses on an extended-wear basis. The risks for infection in the elderly aphakic eye with extended-wear hydrophilic lenses are increased by delay in removal of the lens. Caution in the use of these lenses is urged. The ability of either the patient or an attendant to remove the lens as soon as redness, diminished vision, or pain presents itself is essential in minimizing the risks for serious infection.

Neodymium-YAG laser therapy to the anterior hyaloid in aphakic malignant (ciliovitreal block) glaucoma.

In three aphakic eyes and two eyes with intraocular lens implants (five patients, four women and one man, ranging in age from 65 to 76 years) persistent shallow or flat anterior chamber was observed despite multiple patent laser or surgical iridectomies. In each case direct application of the neodymium-YAG laser (3 to 11 mJ) to the anterior hyaloid face resulted in immediate deepening of the anterior chamber. This deepening was sustained in four of the cases. These results confirm previous observations suggesting that the anterior hyaloid can be the locus of abnormality in aphakic malignant (c

Corneal edema accompanying aphakic extended lens wear.

Corneal swelling was monitored for nine hours in 14 unilateral aphakic patients who wore +13.25-diopter extended-wear lenses. After closed-eye wear, the corneal swelling was 6.9% and 11.4% for aphakic and phakic eyes, respectively. To determine if aphakic corneal swelling could be predicted from either corneal epithelial oxygen uptake or hypoxic corneal swelling, we measured epithelial oxygen uptake and corneal swelling from hypoxia. These were 3.67 mm Hg/s and 3.96 mm Hg/s and 8.0% and 12.1% for the aphakic and phakic eyes, respectively. Differences in these measurements between aphakic and p
