Laparoscopic appendectomy: Which factors are predictors of conversion? A high-volume prospective cohort study.

UNLABELLED: Appendicitis represents one of the most frequent condition requiring surgery. In Italy almost 0.2% of the population will be affected by acute appendicitis every year. Laparoscopic appendectomy (LA) has gained acceptance over the past years and despite several meta-analyses, randomized studies and retrospective studies have been conducted, the indications and results are still conflicting especially in cases of complicated appendicitis. The aim of our study is to evaluate which factors are related to conversion to open appendectomy (OA) during laparoscopic appendectomy (LA). MATHE…

Perforated appendix with abscess: Immediate or interval appendectomy? Some examples to explain our choice.

INTRODUCTION: There are no clear guidelines in the treatment of a perforated appendicitis associated with periappendiceal abscess without generalized peritonitis. PRESENTATION OF CASES: We retrospectively studied six examples of treated children in order to discuss the reasons of our team's therapeutic approach. Some children were treated with a conservative antibiotic therapy to solve acute abdomen pain, planning a routine interval appendectomy after some months. Others, instead, underwent an immediate appendectomy. DISCUSSION: By examining these examples we wanted to highlight how the firs…

Negative Appendectomy: an Audit of Resident-Performed Surgery. How Can Its Incidence Be Minimized?

Chronic hepatitis C virus (HCV) infection is a major cause of morbidity and mortality worldwide. Progression to cirrhosis and hepatocellular carcinoma occurs in 20% of infected adults. The natural history following childhood infection is less well defined, although cirrhosis in children is described. Since blood product screening for HCV infection was introduced in 1990, most children who acquire HCV do so by vertical transmission from an infected mother. Transmission to offspring occurs in approximately 5%. Most children with HCV infection are asymptomatic. Diagnosis is made by testing those

Interval appendectomy after perforated appendicitis: what are the operative risks and luminal patency rates?

BACKGROUND: The need for interval appendectomy after nonoperative management of a perforated appendicitis is being questioned owing to recent studies that estimated recurrence rates as low as 5% because of obliteration of the appendiceal lumen. We review our experience with interval appendectomy in this subset of patients to determine the postoperative outcomes and luminal patency rates. METHODS: A retrospective review was conducted of all children treated nonoperatively for a perforated appendicitis followed by elective interval appendectomy during the past 10 years. The data collected inclu…

Pain after laparoscopic appendectomy: a comparison of transumbilical single-port and conventional laparoscopic surgery.

PURPOSE: Conventional laparoscopic appendectomy is performed using three ports, and single-port appendectomy is an attractive alternative in order to improve cosmesis. The aim of this study was to compare pain after transumbilical single-port laparoscopic appendectomy (SA) with pain after conventional three-port laparoscopic appendectomy (TA). METHODS: From April to September 2011, 50 consecutive patients underwent laparoscopic appendectomy for simple appendicitis without gangrene or perforation. Patients who had undergone appendectomy with a drainage procedure were excluded. The type of surg…

Laparoscopic versus open appendectomy in adults with complicated appendicitis: systematic review and meta-analysis.

BACKGROUND: The goal of the present study was to critically review and identify the strength of available evidence in the literature on the use of laparoscopic appendectomy (LA) in complicated appendicitis (CA). METHODS: The Cochrane Library and Controlled Trials Registry, MEDLINE (Ovid), PubMed, Web of knowledge, and SCOPUS databases were electronically searched, using the keywords "appendectomy," "laparoscopy," "appendicitis." "complicated appendicitis." "gangrenous appendicitis," "perforated appendicitis," with English language as a limit. Backward chaining was also employed. The NHS Publi…

Incidental pathologic findings in open resectional gastric bypass specimens with routine cholecystectomy and appendectomy.

BACKGROUND: Morbid obesity has become a public health epidemic in the United States. With this epidemic, the demand for surgical intervention has led to rapid growth in the field of bariatric surgery. Although various procedures exist, one of the procedures offered at our institution is resectional Roux-en-Y gastric bypass with incidental cholecystectomy and appendectomy. This procedure allows for pathologic assessment of otherwise normal viscera routinely removed as a part of the gastric bypass. The purpose of this study was to determine the incidence of abnormal findings of the extirpated, g…

Risk factors in multiple sclerosis: tuberculin reactivity, age at measles infection, tonsillectomy and appendectomy.

The purpose of the present study was to investigate whether risk of multiple sclerosis (MS) is associated with the occurrence of viral or ba … or bacterial infections of childhood, nor to routine vaccinations, tonsillectomy, adenoidectomy or appendectomy. Our findings suggest a difference between MS patients and controls with respect to environment before school age, the former being less exposed to air-borne infections.

The Impact of Body Mass Index and Presenting Institution on Diagnostic Imaging for Children Undergoing Appendectomy.

PURPOSE: Children with excess weight experience disparities in healthcare delivery. We sought to investigate whether pediatric obesity is associated with differences in appendicitis workup and whether these vary by presenting institution. METHODS: The American College of Surgeons National Surgical Quality Improvement Program-Pediatrics (NSQIP-P) database was retrospectively surveyed for patients undergoing appendectomy from 2018 to 2022. Patients < 2 or > 18 years, non-binary sex, missing body mass index (BMI) data, or those with biologically implausible BMI percentiles were excluded. 72,749 p…

A Rare Complication of Bladder Injury Following Laparoscopic Appendectomy: A Case Report and Literature Review.

Acute appendicitis is widely recognized as the leading cause of acute abdominal pain in patients seeking hospital treatment. The standard of care and golden approach has traditionally involved performing an appendicectomy, which nowadays is more often performed using laparoscopic techniques. As with any laparoscopic procedure, there is a risk of iatrogenic organ injury, particularly during trocar insertion; one such organ at risk is the urinary bladder (0.17-0.73%). Most reported cases of urinary bladder injuries occur due to trocar insertion in the suprapubic area. In this report, we present

Appendectomy and colorectal cancer: a mini review from the perspective of gut microbiota and mucosal immunity.

The vermiform appendix was long considered a vestigial organ, yet accumulating evidence now supports its role as a component of gut associated lymphoid tissue and as a niche involved in microbial homeostasis and mucosal immune regulation. Against this background, whether appendectomy influences colorectal cancer (CRC) development and progression has become an important question at the intersection of oncology, microbiology, and immunology. Current epidemiological evidence does not support a simple model in which appendectomy uniformly increases long term overall CRC risk. Earlier observational…

Assessment of frailty and inflammatory burden index as predictors of postoperative IAA in elderly appendectomy patients.

BACKGROUND: Postoperative intra-abdominal abscesses (IAA) remain a significant complication after laparoscopic appendectomy for acute appendicitis. This study investigates the roles of the 5-item modified Frailty Index (mFI-5) and Inflammatory Burden Index (IBI) in predicting IAA risk. METHODS: This retrospective study analyzed elderly patients who underwent laparoscopic appendectomy from 2015 to 2025. Patients were assessed for frailty using mFI-5 and systemic inflammation using IBI. We collected demographic, clinical, and laboratory data, and univariate and multivariate logistic regression …

Incidental Neoplasm and Histopathological Spectrum of Suspected Acute Appendicitis in Appendectomy Specimen: An Observational Study.

INTRODUCTION: Acute appendicitis is the most common surgical emergency and histopathological studies are the gold standard for confirmation of clinical diagnosis and key methods of discovering appendiceal neoplasm. Appendiceal neoplasms are uncommon and are mostly identified by pathologic examination after appendectomy for presumed appendicitis as an incidental finding. METHODS: An observational cross-section study was conducted at a Medical College from 1st August 2022 to 31st July 2024. Appendectomy specimens were included in the study after obtaining consent. Ethical approval was taken fro…

Patients' attitudes toward negative appendectomies and surgery for suspected appendicitis: a qualitative interview study.

INTRODUCTION: From a surgeon's perspective, appendicitis is treated with appendectomy and sometimes a normal appendix is removed. This study aimed to investigate the patients' perspectives on having surgery but not appendicitis and their involvement in treatment decisions. METHODS: This study is reported according to the COnsolidated criteria for REporting Qualitative research (COREQ) guideline. Eligible participants either had a normal diagnostic laparoscopy with no resection of the appendix or a negative appendectomy confirmed by histopathology. Interviews were conducted using a semi-struct…

Exposure to Operative Anesthesia in Childhood and Subsequent Neurobehavioral Diagnoses: A Natural Experiment Using Appendectomy.

BACKGROUND: Observational studies of anesthetic neurotoxicity may be biased because children requiring anesthesia commonly have medical conditions associated with neurobehavioral problems. This study takes advantage of a natural experiment associated with appendicitis to determine whether anesthesia and surgery in childhood were specifically associated with subsequent neurobehavioral outcomes. METHODS: This study identified 134,388 healthy children with appendectomy and examined the incidence of subsequent externalizing or behavioral disorders (conduct, impulse control, oppositional defiant, …
