Management of cerebral ischemia due to Takayasu's arteritis.

OBJECTIVE: To explore the management of cerebral ischemia caused by Takayasu's arteritis. METHODS: Ninety-three cases treated from June 1984 to September 1999 at the General Post & Telecom Hospital, the Sir Run Run Shaw Hospital, the First Affiliated Hospital of Zhejiang University, the Second Medical College of Beijing University, Beijing An Zhen Hospital, and the Beijing Union Medical College Hospital, including 10 men and 83 women, were reviewed. Of the 93 cases, bypasses from the ascending aorta to the axillary or subclavian artery and from graft to the carotid artery were performed in 47…

[Intestinal necrosis as clinical presentation of Takayasu arteritis].

A 32 years old female was admitted to hospital due to acute abdominal pain, nausea, vomiting and liquid stools. Physical examination was nor … udication and superior limb acrocyanosis disappeared. This is an unusual case of type IV Takayasu's arteritis with acute abdominal signs as the first manifestation.

Sex differences in temporal arteritis and polymyalgia rheumatica.

OBJECTIVE: Sex-specific differences in treatment outcomes have been observed in polymyalgia rheumatica (PMR) and temporal arteritis (TA), with a significantly longer course of treatment in women than in men. We analyzed whether these sex differences are related to differences in disease presentation and severity of the inflammatory response. METHODS: The records of 163 cases of PMR and/or TA diagnosed over a 15 year period were reviewed. A comparative study of clinical and laboratory features between men and women was performed. RESULTS: Of 163 patients, 90 had isolated PMR and 73 had TA. Am…

[Pitfalls in diagnosis of polymyalgia rheumatica/temporal arteritis].

Polymyalgia rheumatica as well as Horton's disease (arteriitis temporalis) are often diagnosed by exclusion. When either disease is suspected because of certain findings a therapy with glucocorticoids is usually started and initial therapeutic success is commonly seen as indirect proof of diagnosis. Clinical experience, however, shows that both diagnoses may conceal a malignancy. Based on this experience the cases of three patients are reported who's clinical and laboratory findings suggested first manifestations of polymyalgia rheumatica or Horton's disease respectively. During treatment, how

The pathogenesis of giant cell arteritis: morphological aspects.

The light-microscopic, electron-microscopic and immunocytochemical characteristics of giant cell arteritis (GCA) have been investigated in a number of studies on temporal arteries. Arterial atrophy and calcification of the internal elastic membrane appear to be prerequisites for the evolution of the inflammatory process. Foreign body giant cells form close to calcifications, apparently without connection with other inflammatory cells and probably by the fusion of modified vascular smooth muscle cells. The foreign body giant cells attack the calcifications. Lymphocytes accumulate around them an…

(A)symptomatic necrotizing arteritis of the female genital tract.

AIMS: The vasculitides represent a heterogenous set of disorders that differ in prognosis and response to therapy. Beside systemic vasculitides, the development of localized forms of arteritis is well known though uncommon and the etiopathogenesis is not yet definitely clear. METHODS: Patients with necrotizing arteritis of the female genital tract proven by histology are studied in a retrospective analysis. RESULTS: Three cases of necrotizing arteritis with histological features of panarteritis nodosa apparently confined to the female genital tract are presented. None of these patients had p…

Doppler ultrasonography in the diagnosis of giant cell arteritis.

Ultrasonography of the temporal arteries detects characteristic signs of vasculitis with a high sensitivity and specificity: a hypoechoic halo due to an oedema of the artery wall and stenoses. The use of modern scanners with high resolution is essential. A halo has to be displayed in two planes. It is always circumferential. The colour intensity must not be too strong, otherwise the colour may cover the halo. A hypoechoic halo is also seen in large vessel giant cell arteritis at the axillary and brachial arteries and at the temporal arteries in polyarteritis nodosa.

Cranial arteritis: a medical emergency with orofacial manifestations.

BACKGROUND: Cranial arteritis, or CA, a vascular disease affecting primarily elderly people, may result in permanent blindness if untreated. Since it frequently mimics temporomandibular joint, myofascial or odontogenic pain, dentists must be familiar with this condition. CASE DESCRIPTION: The authors present reports of two patients who had signs and symptoms of CA, some of which were suggestive of other head and neck pain disorders. In both cases, the diagnosis of CA was confirmed by temporal artery biopsy, and treatment with systemic steroids resulted in rapid resolution of symptoms. CLINIC…

Surgical treatment for annuloaortic ectasia in Takayasu arteritis.

Annuloaortic ectasia (AAE) associated with aortic regurgitation (AR) is a rare involvement in Takayasu arteritis. There are many difficult problems in surgical treatment of this lesion because of its inflammatory nature. We developed a composite graft repair technique to avoid complications associated with Bentall type procedures for treatment of AAE+AR. The technique consists of a double proximal suturing and direct coronary artery reimplantation as a Carrel patch without wrapping. Eight patients (one male, seven females) received this composite graft repair for treatment of AAE+AR due to Tak…

Takayasu arteritis--a case report of aortic aneurysm.

Aortic pseudo-aneurysm is a rare manifestation of Takayasu arteritis. We present a 16-year-old girl who first complained of multiple arthritis, recurrent abdominal pain and malaise at the age of 15 years. The initial working diagnosis was juvenile rheumatoid arthritis. Follow-up abdominal ultrasonography for her hepatomegaly incidentally revealed an aortic aneurysm. Total aortography showed diffuse aortic narrowing and an infra-renal aortic pseudo-aneurysm. Vascular reconstruction with an interposition Dacron graft was performed with uneventful recovery. Early non-specific presentation of Taka…

Color duplex ultrasonography in the diagnosis of temporal arteritis.

BACKGROUND: The diagnosis of temporal arteritis usually requires a biopsy of the temporal artery. We examined the usefulness of color duplex ultrasonography in patients suspected of having temporal arteritis. METHODS: In this prospective study, all patients seen in the departments of rheumatology and ophthalmology from January 1994 to October 1996 who had clinically suspected active temporal arteritis or polymyalgia rheumatica were examined by duplex ultrasonography. The final diagnoses, made according to standard criteria, were temporal arteritis in 30 patients, 21 with biopsy-confirmed dise…

Localized necrotizing arteritis of the central nervous system.

We describe a case of necrotizing arteritis of the brain in a 26-year-old man who presented as organic brain syndrome. The lesion has been confused clinically with acute encephalitis. At necropsy, no other abnormalities suggestive of systemic arteritis were found. Such CNS-restricted necrotizing arteritis of medium and small arteries in the absence of granuloma is extremely uncommon.

Invasive aspergillosis. A complication of treatment of temporal arteritis.

Temporal arteritis is a systemic necrotizing vasculitis for which the only effective treatment is systemic corticosteroids. A review of the literature suggests that there is a significant mortality rate in untreated patients but that those patients who receive adequate corticosteroid treatment rarely experience a reduced life expectancy. I had the opportunity to evaluate a patient with temporal arteritis who eventually died from disseminated aspergillosis 6 weeks after beginning corticosteroid treatment. A high index of suspicion for infections with opportunistic organisms should be maintained…

Temporal arteritis. Comparison of histological and clinical findings.

The aim of this investigation was to correlate the clinical and histological findings in 85 consecutive patients with temporal arteritis. Particular attention was given to a possible correlation between optic nerve involvement and the presence of giant cells upon histological examination. Severe ischemia as in anterior ischemic optic neuropathy and central retinal arterial occlusion was presented in 37 patients (43%). Giant cells were definitely observed in 32 biopsy specimens (37.6%) and suspected in 11 additional specimens (13%): In 42 specimens (49.3%) no giant cells were present. Our study…

Cancer in association with polymyalgia rheumatica and temporal arteritis.

In our prospective controlled study, a total of 185 patients with polymyalgia rheumatica (PMR) and temporal arteritis (TA) diagnosed during 1978-83 and their 925 matched controls were cross checked with the data files at the Cancer Registry of Norway at the end of 1987. Malignancy was registered in 27 patients (14.6%) and 131 controls (14.2%) between 1953 and the end of 1987. Malignancy was registered in 16 (24.6%) of the patients with biopsy demonstrating arteritis temporalis. The hazard rate for developing malignancy after diagnosis for the whole patient population was not significantly diff…
