Bone destruction in arthritis.

Rheumatoid arthritis (RA) is characterised by the presence of an inflammatory synovitis accompanied by destruction of joint cartilage and bone. Destruction of cartilage matrix results predominantly from the action of connective tissue proteinases released by RA synovial tissues, chondrocytes, and pannus tissue. Several lines of evidence in RA and in animal models of arthritis support a role for osteoclasts in the pathogenesis of bone erosions. RA synovial tissues produce a variety of cytokines and growth factors that may increase osteoclast formation, activity, and/or survival. These include i…

Neuropathy in rheumatoid arthritis.

BACKGROUND: Paucity of Indian literature on rheumatoid neuropathy creates a lacuna in the critical evaluation and discussion of the subject. We did this study to find out the incidence and pattern of neuropathy and to correlate it with disease parameters and other extra-articular involvement. MATERIAL AND METHODS: We studied 31 patients of rheumatoid arthritis (RA) classified by ACR criteria. Electromyography and nerve conduction studies (EMG/NCV) were done in all the patients apart from routine laboratory and radiological investigations. Electrocardiograph (ECG), pulmonary function tests (PF…

Comorbidity in rheumatoid arthritis.

It is increasingly clear that coexistent disease plays a pivotal role in RA outcome and that efforts aimed at specifically addressing these comorbidities need to be aggressively sought, investigated, and implemented once proven effective. RA-associated costs are currently increasing at twice the rate of the medical care index. Comorbidity in the setting of RA independently predicts disease-associated disability (a major cost component) and mortality, underscoring the need for a more comprehensive approach to RA, one that adequately addresses disease-specific comorbidities. At present, many pri

Pathogenesis of reactive arthritis.

There is good evidence that bacteria persist in vivo in patients with reactive arthritis (ReA). While Chlamydia seem to hide inside the joint, other areas such as gut mucosa or lymph nodes seem to be more likely places for Salmonella and Yersinia. T-helper (Th) 1 cells secreting cytokines such as IFN gamma and TNF alpha are crucial for an effective elimination of these bacteria. An inhibited Th1-response could be demonstrated in ReA, probably contributing to bacterial persistence. While HLA-B27 is found in only approximately 50% of patients with acute ReA, HLA-B27 seems to be crucial for the d…

Whipple disease and arthritis.

Whipple disease is a chronic, multisystem, curable, bacterial infection that usually affects middle-aged men and has a wide range of clinical manifestations. The most common symptoms are weight loss and diarrhea, preceded in three quarters of cases by arthritis for a mean of 6 years. In most patients, periodic acid-Schiff staining of proximal small bowel biopsy specimens reveals inclusions within the macrophages, corresponding to bacterial structures. However, patients with various manifestations of the disease may have no gastrointestinal symptoms and negative jejunum biopsy results. Before t…

Cyclophosphamide for rheumatoid arthritis.

OBJECTIVES: To assess the short-term effects of cyclophosphamide for the treatment of rheumatoid arthritis. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Group's Register, the Cochrane Controlled Trials Register (issue 3, 2000), Medline and Embase up to and including August 2000. We also carried out a handsearch of the reference lists of the trials retrieved from the electronic search. SELECTION CRITERIA: All randomized controlled trials (RCTs) and controlled clinical trials (CCTs) comparing oral cyclophosphamide against placebo (or an active drug at a dosage considered to be ine…

Rheumatoid arthritis and osteoporosis.

Some controversial issues in the current literature in relation to osteoporosis and rheumatoid arthritis are updated and discussed. Because most studies agree that osteoporosis in postmenopausal women and in men with RA is more evident at the hip and radius than at the spine, and that the most important determinants of bone loss are disability, local disease activity, and cumulative corticosteroid dose, osteoporosis is not a common systemic extra-articular manifestation of RA. In early arthritis, periarticular osteoporosis does indeed reflect disease activity because it is closely related to t…

Prognosis in juvenile arthritis.

Juvenile arthritis implies an onset of disease under 16 years with arthritis persisting in one or more joints for at least six weeks, and with the active exclusion of well defined illnesses, such as systemic lupus erythematosus. Prognosis implies the ability to predict outcome. Its accuracy depends on many factors with early recognition and appropriate care being important. However, response to treatment may be variable. In general, those with involvement of a few joints do better than those with systemic disease or seropositive juvenile rheumatoid arthritis both with regard to persistence of …

[Osteoporosis in rheumatoid arthritis].

In cases of rheumatoid arthritis, osteoporosis may be local or general. The aetiology is multifactorial. Reduced bodily function, synovial inflammation, steroids and menopause are important risk factors. Studies have shown that, in cases of primary osteoporosis, bone mineral density measurements in the distal radius may predict risk of fracture at other sites, such as the neck of the femur and the dorsal vertebrae. Such a connection is not found for rheumatoid arthritis. Bone density measurements in the distal radius may overestimate the risk of fractures due to localised periarticular osteopo…

Rheumatoid Arthritis of the Hip.

The hip joint may be affected in 15% to 28% of all patients with rheumatoid arthritis. Radiographic evidence of involvement includes periarticular osteopenia, cystic changes, and a variable amount of progressive protrusio acetabuli. Histomorphometric study has shown increased bone turnover in acetabular biopsy specimens from rheumatoid patients undergoing total hip arthroplasty. Due to the relative fragility of bone in these patients, there is an increased risk of fracture of the proximal femur due to minor trauma, and a high rate of loss of fixation has been reported. Total hip arthroplasty h…

[Etiopathogenesis of rheumatoid arthritis].

Rheumatoid arthritis is a disease, which etiopathogenesis depends on genetic and environment factors. Little is the progress of knowledge the environment factors but the strong is association with HLA DR4/DR1. The hypothesis being proposed is that RA is a chronic immune-mediated disease whose initiation and perpetuation is dependent on T-cell CD4 helper is connected by activation monocyte/macrophage cells which per production proteolytic enzymes and prostaglandins way to local inflammation and destruction articular cartilage and bone. Lymphocytes B proliferate to plasmocytes, which produce rhe…

Treatment of Lyme arthritis.

The efficacy of different therapeutic regimens for Lyme arthritis is reviewed. The first treatment for Lyme arthritis, intramuscular benzathine penicillin 2.4 million units weekly for 3 weeks, had a success rate of 35%. Another study employed intravenous penicillin G at a dosage of 20 million units daily for 10 days, which cured 55% of patients. Intravenous ceftriaxone has been shown to be superior to penicillin with a response rate of 94%. However, these results have been challenged in recent reports. Oral doxycycline or amoxicillin in association with probenecid seems to work equally well al…

Minocycline for rheumatoid arthritis.

Minocycline may prove to be a valuable agent in adjunctive treatment of RA. The use of minocycline is attractive because of its relatively benign adverse effect profile in common dosages, although vestibular toxicity has occurred frequently when doses of 400 mg/d have been used. Adverse effects that do occur usually subside after discontinuation of the drug. Currently, the studies available offer no definitive conclusion concerning the use of tetracyclines for this purpose. These trials do show promise, however, and suggest that larger, controlled, double-blind studies with prolonged use of mi
