BACKGROUND: Left atrial appendage (LAA) closure reduces stroke in patients with atrial fibrillation.
CASE SUMMARY: A 63-year-old man with chronic atrial fibrillation had undergone mitral valve replacement and endocardial suture closure of the LAA 5 years before presentation. One year postoperatively, a giant LAA was noted, and coronary angiography revealed a newly developed coronary-LAA fistula. Progressive enlargement of the LAA was confirmed 5 years postoperatively, with expansion of the fistulous tract on coronary angiography, ultimately leading to surgical LAA resection.
DISCUSSION: Inco…

OBJECTIVE: Thrombus formation in atrial fibrillation is caused by blood flow stagnation within the left atrial appendage (LAA). It is believed that blood flow tends to stagnate within the LAA even in patients with sinus rhythm and high risk of stroke. This study evaluated blood flow within the LAA using four-dimensional flow magnetic resonance imaging among patients stratified by the congestive heart failure, hypertension, age, diabetes mellitus, prior stroke, vascular disease, age, sex category (CHA2DS2-VASc) score.
METHODS: Ninety-eight participants with sinus rhythm (70 patients, 28 contro…

PURPOSE: Left atrial appendage (LAA) closure has become the standard of care for patients with atrial fibrillation (AF) at high risk of thromboembolism who are intolerant or unwilling to take anticoagulants. LAA occlusion device infection is a challenging complication, and there is a paucity of data on the management and outcomes of LAA occlusion device infection. We aimed to summarize the existing literature and highlight the knowledge gap in this area.
METHODS: A detailed search was conducted through the databases PubMed/MEDLINE, EMBASE, Web of Science, CINAHL, and Cochrane Central using th…

OBJECTIVES: Left atrial appendage occlusion (LAAO) has emerged as an alternative to anticoagulation for reducing stroke risk in patients with atrial fibrillation (AF). However, patients with heart failure (HF) may have an enhanced risk of stroke and elevated risk for this procedure. To further investigate this population, the authors present a retrospective analysis on LAAO in patients with HF.
METHODS: The authors performed a retrospective review of all hospitalizations for LAAO using the National Readmissions Database between September 2015 and November 2019. From these, patients with ICD-1…

BACKGROUND: Advances in imaging have led to procedural optimization of left atrial appendage closure (LAAC). Contrast-free approaches, guided merely by echocardiography, have been established, however data on this topic remains scarce. In this analysis, we assessed contrast-free procedural results with the LAMBRE LAAC device.
METHODS: The multicenter retrospective BoBoMa (Bonn/Bordeaux/Mainz)-Registry included a total of 118 patients that underwent LAAC with LAMBRE devices omitting contrast-dye. Baseline and echocardiographic characteristics as well as intra- and postprocedural complications …

INTRODUCTION: The Centers for Medicare & Medicaid Services (CMS) required a shared decision-making (SDM) interaction, with an "independent" physician, before left atrial appendage closure (LAAC). The purpose of this study is to better understand how this requirement is implemented in clinical practice.
METHODS: We surveyed LAAC-performing centers. The characteristics of respondent and nonrespondent hospitals were compared using the CMS Provider of Services File for 2017.
RESULTS: We received 86 responses out of 269 surveys mailed (32%). Respondent and nonrespondent hospital affiliations were…

IMPORTANCE: Patients with atrial fibrillation (AF) who have ischemic stroke despite taking oral anticoagulation therapy (OAT) have a very high risk of recurrence. Left atrial appendage occlusion (LAAO) is a mechanical stroke prevention strategy that may provide additional protection in patients with thromboembolic events under OAT.
OBJECTIVE: To compare percutaneous LAAO with continuing OAT alone regarding stroke prevention in patients with AF who had a thromboembolic event despite taking OAT.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study was a propensity score-matched comparison of t…

The standard approach to catheter based left atrial appendage occlusion (LAAO) involves trans-esophageal echocardiography (TEE) guided screening and placement, and procedural general anesthesia requiring overnight stay. In pursuit of improved patient experience and reduced cost, streamlined approaches in each phase of care have been explored. However, the safety and clinical outcomes for a completely protocolized minimalist approach utilizing computed tomography angiogram (CTA), intracardiac echocardiography (ICE), conscious sedation, and same-day discharge are lacking. We retrospectively stud…

BACKGROUND: Transesophageal echocardiography (TEE) is the gold standard test for the diagnosis of left atrial appendage (LAA) thrombus. Nonetheless, computerized tomography angiography (CTA) is readily used to exclude LAA thrombus before pulmonary vein isolation (PVI) and LAA closure procedures. We aimed to assess the comparability of LAA thrombus diagnosis using chest CTA scans in patients with atrial fibrillation who underwent TEE.
METHODS: Retrospective collection of consecutive patients with atrial fibrillation who underwent TEE and chest CTA within 30 days and had evidence of spontaneous…

PURPOSE OF REVIEW: Intracardiac echocardiography (ICE) has emerged as a powerful imaging tool to guide percutaneous left atrial appendage closure (LAAC). Herein, we review an imaging protocol for ICE-guided LAAC and discuss the evidence for its use.
RECENT FINDINGS: Standardized imaging protocols have been proposed but have not been fully validated. ICE imaging yields similar procedural and clinical outcomes when compared to transesophageal echocardiography (TEE) to guide LAAC. Despite benefits of avoiding general anesthesia, TEE, and multiple physicians for LAAC procedures, ICE imaging remai…

Peri-device leak after left atrial appendage closure (LAAC) is often treated with endovascular coils, plugs, or second occluders. This is the first study reporting the Amulet device used for peri-device leak. An 80-year-old male with paroxysmal atrial fibrillation and recurrent falls with head trauma who underwent LAAC with a 24 mm Watchman 2.5 device 3 years ago at another institution was referred to our clinic for management of the peri-device leak. Transesophageal echocardiogram showed persistent residual peri-device leak with 5 mm width along the Coumadin ridge aspect of the device. Comput…
