Preventing stroke in patients with atrial fibrillation.

CONTEXT: Atrial fibrillation, a common disorder that affects nearly one sixth of the population aged 75 years and older, is a major risk factor for stroke. OBJECTIVES: To review and evaluate the evidence supporting the use of warfarin and/or aspirin for stroke prevention in patients with atrial fibrillation. DATA SOURCES: Prospective, randomized trials of patients with atrial fibrillation evaluating either warfarin or aspirin or both, from MEDLINE from January 1, 1966, to February 23, 1999. STUDY SELECTION: Five primary prevention placebo-controlled studies, which had been formally pooled, …

Dispersion of repolarization in paroxysmal atrial fibrillation.

Widened dispersion of refractoriness has been considered to lead to an increase in atrial vulnerability. In this study, we obtained simultaneous monophasic action potential recordings by 2 special catheters from 2 different loci in the right atrium in 9 normal subjects (Group 1) and in 7 patients with paroxysmal atrial fibrillation (Group 2). We measured action potential duration at 50% (APD50) and 90% (APD90) repolarization from both loci in the right atrium during steady-state through one of the two catheters. The differences between APD50 and APD90 recorded by the two catheters were designa…

The peri-operative management of atrial fibrillation.

Atrial fibrillation is a common arrhythmia frequently seen in surgical patients. The onset of new atrial fibrillation during the peri-operative period is less common. There are many possible precipitating factors, although volatile agents themselves may have an antifibrillatory action. The management of atrial fibrillation includes removal of any precipitating factors and treatment of the arrhythmia itself. Immediate management of acute-onset atrial fibrillation is usually direct current cardioversion. Alternatively, anti-arrhythmic drugs can be used to achieve cardioversion. In patients with …

Electrophysiologic mechanisms of perpetuation of atrial fibrillation.

The presence of an excitable gap during atrial fibrillation (AF), although short and variable, may be of potential importance for the development of alternative techniques for termination of AF by rapid pacing. Also the notion that perpetuation of AF may be partly dependent on macroreentry around the natural atrial orifices, may provide a new therapeutic option for the permanent cure of AF by interrupting the anatomical circular pathways in the atria by radiofrequency ablation. In our opinion the rapidly growing understanding of the electrophysiologic mechanisms of AF certainly warrants some o…

[Atrial fibrillation and apoplexy--risks and prevention].

The annual incidence of ischemic stroke among patients with chronic non-valvular atrial fibrillation is about 4.5 percent. In five controlled trials, oral anticoagulant therapy with warfarin reduced the annual incidence of stroke by 68 percent to 1.4 percent. The effect of aspirin has not been unequivocally determined. Aspirin reduced the annual risk of stroke by 18 percent (n.s.) in one trial, and by 44 percent in another, though the two trials differed both in mean age of the patients and in aspirin doses. Direct comparison of warfarin and aspirin revealed no difference in efficacy. Advanced…

Regional entrainment of atrial fibrillation in man.

INTRODUCTION: The feasibility of entrainment of macroreentrant atrial arrhythmias such as atrial flutter is well documented. Recently, it has been shown that regional entrainment of atrial fibrillation is feasible in dogs. METHODS AND RESULTS: Three patients with chronic atrial fibrillation underwent electrophysiologic evaluation with attempted entrainment of atrial fibrillation prior to successful endocardial atrial defibrillation. A 16-pole catheter was positioned in the trabeculated right atrium, and in two patients a multipolar catheter was positioned along the septum. In addition, two la…

Newer developments in the management of atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia encountered in clinical practice. Unlike reentrant supraventricular tachycardia and malignant ventricular tachyarrhythmias, for which highly effective and safe nonpharmacologic therapies are available, the treatment of AF remains controversial and often problematic. Whereas electrical cardioversion restores sinus rhythm in most patients with AF, the maintenance of sinus rhythm often requires membrane-active antiarrhythmic drugs that may increase mortality by inducing ventricular proarrhythmia. The control of ventricular r…

[The use of digitalis glycosides in atrial fibrillation].

The role of cardiac glycosides for conversion of atrial fibrillation to simus rhythm is controversially discussed. In a prospective study, 45 patients with paroxysmal atrial fibrillation were randomly assigned to one of three treatment groups (of 15 patients each). Group I received oral digoxin, three times 0.125 mg up to twice 0.25 mg daily; group II oral digoxin twice 0.125 mg and quinidine hydrogen sulphate 750-1000 mg daily; group III oral digoxin three times 0.125 mg and flecaimide 200-300 mg daily. During a mean observation period of 11 months, digoxin alone was significantly less effect…

The management of atrial fibrillation: current perspectives.

The management of atrial fibrillation is evolving in response to recently published data. Low-risk patients with new-onset atrial fibrillation may not need to be hospitalized. Beta blockers may be the most effective drugs for controlling the heart rate. When a patient does not respond to drug therapy, it is still appropriate to search for the cause of the arrhythmia and to use direct-current cardioversion. Clear evidence now exists that patients with chronic atrial fibrillation should be given anticoagulant drugs to reduce the risk of stroke. Antiarrhythmic drugs should be used cautiously, bec…

[A-V conduction in atrial fibrillation and flutter].

The assessment of A-V conduction in the presence of atrial fibrillation is based upon analysis of the R-R intervals. This is because in atrial fibrillation it is impossible both to identify the impulse that has been conducted to the ventricles, and to measure the A-V conduction time. The first step is, therefore, to evaluate whether the QRS complexes are the expression of conducted atrial impulses, or they are A-V junctional or ventricular in origin. In other words, it is necessary to distinguish between A-V conduction and A-V dissociation. Conduction in atrial fibrillation commonly results in…

Atrial fibrillation after stroke in the elderly.

To examine the relationship between atrial fibrillation and mortality after stroke, we studied 186 men and 167 women from the Waikato Stroke Registry whose mean age was 75.2 +/- 7.5 years. Twenty-three percent (82 of 353) had atrial fibrillation or flutter on their admission electrocardiogram. This group differed significantly from that with sinus rhythm in three respects: 1) They were older (p less than 0.01); 2) they had more severe current stroke deficit as evidenced by lower limb power (p less than 0.05) and Mini-Mental State Score (p less than 0.001), higher incidence of homonomous hemian…

Usefulness of labetalol in chronic atrial fibrillation.

Beta-adrenergic blocking agents are useful in controlling excessive ventricular rate in chronic atrial fibrillation (AF) but often reduce exercise capacity. To investigate the advantage of labetalol--a unique beta blocker with alpha-blocking property--in chronic AF, 10 patients without underlying structural heart disease were studied with treadmill test, 12-minute walk and 24-hour ambulatory electrocardiographic monitoring. Patients were randomized and crossed over to receive 4 phases of treatment (placebo, digoxin, digoxin with half-dose labetalol, and full-dose labetalol). Exercise durations…

Diagnosis and characteristics of lone atrial fibrillation.

Thirty-two patients with LAF were studied. Nineteen (59 percent) were male subjects. Mean age was 46.8 years. They were followed up for 2 to 16 years. On admission, most patients (94 percent) were symptomatic and had palpitations, nine had nonspecific chest pain, and five had syncope or near syncope. Left atrial size measured by echocardiography was less than 40 mm in all patients. During the follow-up period, only one patient had a complication, ie, cerebral embolism. We conclude that LAF may be regarded as a relatively benign form of AF. According to our study, one of its features is left at

Arterial embolism in thyrotoxicosis with atrial fibrillation.

The incidence of atrial fibrillation (AF) in 142 patients with thryotoxicosis was 21% and that of thromboembolic disease was 8.5%. Male sex, increasing age, and associated rheumatic or hypertensive heart disease were significantly associated with the presence of AF. Thromboembolic episodes occurred in 12 (40%) of the patients with AF and in none of the 112 patients without AF. Cerebral emboli accounted for 53% of the episodes. In view of the grave prognosis of the embolic episodes, patients with thyrotoxicosis should be treated with heparin during attacks of AF and with oral anticoagulants whe…

Atrial fibrillation and arterial embolism in hyperthyroidism.

Atrial fibrillation or flutter was present in 70 of 381 patients with uncontrolled hyperthyroidism; return to stable sinus rhythm occurred in 39 with antithyroid and antiarrhythmic treatment. One third of the patients who reverted did so in the first week of treatment while still hyperthyroid. As expected, reversion was more likely in younger patients, and in those with arrhythmia of recent onset, without evidence of other heart disease. Eight patients with arrhythmia had proven (five) or probable (three) major arterial embolic episodes. Four of these eight patients died. Embolism tended to oc…
