Neurolytic celiac plexus blocks (NCPB) have been performed for many years for the treatment of cancer and some non-cancer pain conditions associated with the upper gastrointestinal tract. The block can provide adequate pain relief from the area of the distal esophagus to the transverse colon, and can be approached from a variety of ways. This is a review of the anatomy, patient selection, technique, medications used, possible complications, and efficacy of the treatment.

OBJECTIVE: The main cause of dissatisfaction after sympathetic trunk blocking surgery (T2 sympathectomy, sympathetic clipping) for craniofacial hyperhidrosis is compensatory sweating. Preserving sympathetic trunk may decrease the incidence of compensatory sweating, and we introduce T2 ramicotomy, which may better preserve the sympathetic nerve trunk in order to reduce compensatory sweating.
METHODS: From January 2000 to November 2002, video-assisted thoracoscopic (VAT) T2 sympathetic clipping and VAT ramicotomy were performed in 44 patients suffering from craniofacial hyperhidrosis. Twenty-tw

OBJECTIVE: The purpose of this study was to determine the factors associated with false-positive medial branch blocks (MBB), the nerve blocks used to diagnose facet arthropathy, in soldiers and retirees.
METHODS: The study subjects were 78 patients with chronic low back pain who underwent diagnostic MBB to determine whether or not the facet joints were pain generators. Radiofrequency denervation of these nerves was performed in all patients with positive responses. Patients who failed to obtain pain relief after the blocks (negative blocks) and those who obtained temporary pain relief after M

The effect and physiologic role of enkephalins on the sphincter of Oddi was studied in the cat. Leucine- and methionine-enkephalin caused an initial sphincter of Oddi contraction followed by a more prolonged relaxation. The excitatory effect of leucine-enkephalin was antagonized partially by atropine and by methysergide infusion. It was completely blocked by a combination of atropine and methysergide infusion, by tetrodotoxin, by 5-hydroxytryptamine depletion induced by reserpine, and by 5-hydroxytryptamine tachyphylaxis. Naloxone infusion decreased sphincter of Oddi motor activity, which was

OBJECTIVES: This study aimed to evaluate the effect of vagotomy, when associated with splenectomy, on adiposity and glucose homeostasis in Wistar rats.
METHODS: Rats were divided into 4 groups: vagotomized (VAG), splenectomized (SPL), VAG + SPL, and SHAM. Glucose tolerance tests were performed, and physical and biochemical parameters evaluated. Glucose-induced insulin secretion and protein expression (Glut2/glucokinase) were measured in isolated pancreatic islets. Pancreases were submitted to histological and immunohistochemical analyses, and vagus nerve neural activity was recorded.

OBJECTIVE: This study aimed to compare the efficacy of lidocaine versus ropivacaine in stellate ganglion block (SGB) for insomnia treatment, including sleep quality improvement, symptom relief, and autonomic nervous system regulation. METHODS: Sixty patients aged 18–75 years with insomnia were randomly divided into the ropivacaine group (Rop group) and lidocaine group (Lido group). Patients in Lido group received a 10-day SGB treatment using 0.8% lidocaine/3 ml. Patients in Rop group were treated with 0.2% ropivacaine/3 ml for SGB for 10 consecutive days. All patients received standard cogniti

Age-related changes of the sinus node (SN) function and the autonomic influence on the SN function were evaluated in 65 patients with sick sinus syndrome (range 14 to 84 years). Heart rate (HR), corrected SN recovery time and sinoatrial conduction time were measured before (basic) and after (intrinsic) autonomic blockade (propranolol 0.2 mg/kg plus atropine 0.04 mg/kg intravenously). Percent of autonomic chronotropies of the SN function were calculated by the following formulas: (1)--(intrinsic HR--basic HR/intrinsic HR) X 100; (2) (intrinsic corrected SN recovery time--basic corrected SN reco

AIMS: Autonomic dysfunction can promote atrial fibrillation (AF) and results from AF-related remodelling. N-type Ca2+-channels (NTCCs) at sympathetic nerve terminals mediate Ca2+-entry that triggers neurotransmitter release. AF-associated remodelling plays an important role in AF pathophysiology but the effects of NTCC inhibition on such remodelling is unknown. Here, we investigated the ability of a clinically available Ca2+-channel blocker (CCB) with NTCC-blocking activity to suppress the arrhythmogenic effects of AF-promoting remodelling in dogs.
METHODS AND RESULTS: Mongrel dogs were kept

Endotoxemia and/or systemic inflammation may lead to disturbances in the cardiac autonomic nervous system and consequent arrhythmia. The underlying mechanism remains unclear. Therefore, we investigated the expression of nerve growth factor (NGF) and its association with cardiac sympathovagal balance in a rodent model of self-limited peritonitis. Male Wistar rats were randomized into the following groups: normal control, sham, gastric perforation (GP), and GP treated with methylprednisolone. Cardiac expression of NGF, growth-associated protein 43 (GAP43), along with other nerve markers were eva

BACKGROUND AND OBJECTIVE: Desflurane has been shown to increase sympathetic activity and heart rate (HR) in a concentration-dependent manner. Nevertheless, desflurane, like all other volatile anaesthetics, increased HR in parallel to vagal inhibition in a previous study. Therefore, our hypothesis is that desflurane elicits tachycardia by vagal inhibition rather than by activation of the sympathetic nervous system.
METHODS: Six dogs were studied awake and during desflurane anaesthesia (1 and 2 MAC) alone, after pretreatment with propranolol (2 mg kg(-1) followed by 1 mg kg(-1) h(-1)), or after

OBJECTIVE: Blockade of the stellate ganglion is an established and highly effective diagnostic and therapeutic procedure for management of certain acute and chronic pain syndromes or other disorders. The paratracheal injection of a local anaesthetic is the simplest and most frequently used approach for blocking the cervicothoracic sympathetic nerves (ganglion stellate). However, since serious complications can occur during or following the anterior paratracheal technique including accidental intravenous or intraarterial injection, non-invasive methods for blockade of the stellate ganglion have

BACKGROUND: Advances in minimally invasive techniques, enhanced recovery protocols, and regional pain management have improved postoperative outcomes. However, visceral pain and postoperative nausea and vomiting (PONV) continue to be challenging after minimally invasive procedures. Intraoperative autonomic neural blockade (ANB), which interrupts the afferent nerves transmitting these impulses, has effectively addressed these challenges, as demonstrated in previous publications. This randomized trial evaluates its efficacy in laparoscopic cholecystectomy.
METHODS: In this double-blind, randomi

The coccyx, the last segment of the spine, joins the sacrum at the base. It has three to five vertebrae, which are typically fused. In front of the sacrococcygeal junction is the ganglion impar, the only unpaired autonomic ganglion. The two sympathetic chains come to a close there. The impar ganglion is traversed by sympathetic nerves carrying pain fibers from the perineum, distal sections of the rectum, the vagina and the urethra. The painful disorder known as coccygodynia, which affects the tail end of the spine, is frequently brought on by birth trauma or is caused by an unidentified factor
