The unexpected death of a loved one and risk for onset of PTSD and depression in the Nepal Chitwan Valley Family Study.

BACKGROUND: Adults who experience the unexpected death of a loved one are at elevated risk for post-traumatic stress disorder (PTSD) and maj … xpected death of a loved one. We used time-varying Cox proportional hazards models to estimate post-bereavement onset of PTSD and MDD. We stratified models by age at the first unexpected loss, as 0 to 19, 20 to 29, 30 to 39, and 40 to 65 years, compared to no loss, and stratified by gender in an interaction analysis. RESULTS: Bereaved adults demonstrated a h …

Unraveling attachment - A network analysis of the cognitive pathways linking attachment and prolonged grief.

BACKGROUND: Attachment style is widely recognized as influential in shaping responses to bereavement and prolonged grief disorder (PGD). Although theorized extensively, empirical clarity regarding how attachment styles specifically impact PGD symptoms and therapeutic implications remains limited. This study aimed to identify cognitive-behavioral mechanisms linking attachment styles to PGD symptoms. METHODS: Data were collected from a community sample of 695 bereaved adults. Network analysis explored interactions between attachment styles (anxious and avoidant) and various cognitive-behavioral…

Family Involvement in Learning From Expected Child Deaths: A Qualitative Study of UK Parents.

BACKGROUND: Bereaved parents often have questions about their child's illness and care even when the cause was established prior to death. Child Death Review (CDR) seeks to understand the full reasons for each child's death to help improve care. In the United Kingdom, parents should be informed of CDR, asked for questions or feedback and outcomes shared with them. They should be allocated a keyworker for support with bereavement and CDR. This study aims to explore parents' experiences of CDR following expected child deaths. METHODS: Parents whose children died in England during 2021-2022, in …

Rumination, worry and negative and positive affect in prolonged grief: A daily diary study.

After bereavement, a significant minority experiences severe, persistent, and disabling grief, termed prolonged grief or complicated grief. Prolonged grief treatments may be enhanced by improving understanding of malleable risk factors in post-loss psychological adaptation. Repetitive negative thought (e.g., rumination, worry) constitutes such a risk factor. Rumination and worry are both theorized to be maladaptive through interrelations with affect, yet this assumption has not been systematically investigated in the bereaved. We aimed to fill this gap in knowledge with a baseline survey and 1…

Prolonged grief in refugees, parenting behaviour and children's mental health.

BACKGROUND: Many refugees experience bereavement, and as a result they suffer elevated rates of prolonged grief disorder. Evidence also indicates that elevated rates of psychological disturbance in refugee children can be associated with parental mental health. This study examined the extent to which prolonged grief disorder in refugees is associated with their parenting behaviour and in turn with their children's mental health. METHODS: This study recruited participants from the Building a New Life in Australia prospective cohort study of refugees admitted to Australia between October 2013 a…

Autobiographical memory specificity and symptoms of complicated grief, depression, and posttraumatic stress disorder following loss.

This study examined the specificity and content of autobiographical memories among bereaved individuals. Self-report measures of bereavement-related distress and a standard and trait version of the Autobiographical Memory Test (AMT) were administered to 109 bereaved people. We examined associations of memory specificity with (a) demographic and loss-related variables and with (b) symptom-levels of complicated grief (CG), depression, and posttraumatic stress disorder (PTSD), (c) associations of the content of memories (related vs. unrelated to the loss/lost person) with symptoms, and (d) the de…

Empathic Communication in Pediatric Palliative Care: A Mixed-Methods Systematic Review.

BACKGROUND: Effective communication between healthcare providers and parents facing their child's end of life is critical yet complex. A comprehensive synthesis of how empathic communication influences clinical and bereavement outcomes in pediatric palliative care is lacking. OBJECTIVE: To synthesize qualitative and quantitative evidence on parent-provider empathic communication in pediatric palliative care and to develop an integrative theoretical-conceptual model explaining its proposed mediating role in symptom management and family bereavement. METHODS: A mixed-methods systematic review …

Prolonged Grief Disorder: Addressing Misconceptions With Evidence.

There are many misconceptions about Prolonged Grief Disorder (PGD). We show with data that PGD is a diagnosis that applies to a rare few of mourners who are at risk of significant distress and dysfunction. Those mourners who meet criteria for PGD have been shown to benefit from specialized, targeted treatment for it. The case against PGD is empirically unsubstantiated, and the need for scientific examination of effective treatments is warranted.

This inspired the concept of different subtypes based on symptom pr … t variety of rather common hallucinations possibly related to heightened alertness, and 4. possibly bereavement hallucinations. The bereavement subtype showed an increase in emotional loneliness and the presence of delusions. The relationship between mortality and time since divorce, widowhood or remarriage in Norway.

The chance of dying within any given year probably depends not only on marital status in that year but also on earlier partnership history. There is still not much knowledge about such effects, however. Our intention is to see how mortality is associated with time since divorce, bereavement and remarriage and time between marital disruption and remarriage. We use register data that include the entire Norwegian population aged 40-89 from 1970 to 2008 (70,701,767 person-years of exposure and 1,484,281 deaths). The excess mortality of divorced men compared to their married counterparts increases …

Restricted End-of-Life Care and Mourning During COVID-19: A Longitudinal Study on Grief, Mental Health, and Rumination.

This longitudinal study examined the mental health impact of COVID-19 restrictions on individuals bereaved during the pandemic, focusing on the mediating role of grief-specific rumination. A total of 141 adults who lost a significant person during the COVID-19 pandemic completed questionnaires at two time points, assessing perceived restrictions, psychological impact, trauma, anxiety, depression, prolonged grief, and rumination. Results showed that high levels of restrictions' psychological impact were associated with elevated symptoms of trauma, anxiety, depression, and prolonged grief. Altho

A prospective examination of catastrophic misinterpretations and experiential avoidance in emotional distress following loss.

This prospective study examined the role of experiential avoidance and catastrophic misinterpretations of grief reactions in emotional distress following the death of a loved one. Eighty-two bereaved individuals completed measures of experiential avoidance, catastrophic misinterpretations, and symptom of prolonged grief disorder (PGD) and depression within the first half year of bereavement, and again completed measures assessing PGD and depression severity 1 year later. The findings showed that experiential avoidance was significantly correlated with endorsement of catastrophic misinterpretat…

National survey of obstetrician attitudes about timing the subsequent pregnancy after perinatal death.

OBJECTIVE: After stillbirth or early infant death, parents often query when they can try for another pregnancy. We conducted a national survey of US obstetricians to assess attitudes about optimal timing of next pregnancy and advice given to parents. STUDY DESIGN: The study was an anonymous mail survey of 1500 randomly selected US obstetricians asking about physician experiences with perinatal death. RESULTS: In all, 804 of 1500 obstetricians completed the survey for a 54% usable response rate. Two-thirds of respondents endorsed a waiting time <6 months for parents bereaved by stillbirth who

End-of-life care planning for ethnically diverse kidney patients in the COVID-19 era.

BACKGROUND: End-stage kidney disease (ESKD) is most prevalent amongst people from minority ethnic groups and people receiving haemodialysis were considered to be ‘clinically extremely vulnerable’ during the Covid- 19 pandemic. Covid-19 impacted the health of those from ethnically diverse backgrounds disproportionally. The Covid-19 pandemic brought death and dying to the forefront of public discourse, yet few studies have explored the impact of the Covid-19 pandemic on individual’s understanding of and willingness to engage in end-of-life care planning conversations. In addition, few studies ha

First responders' experiences and training needs at the scene of a suspected suicide in Ireland: A mixed methods study.

BACKGROUND: First responders are categorised as those who are the first to respond to emergency calls and attend the site of an emergency. They include, but are not limited to police, emergency medical technicians, firefighters, and water safety personnel. First responders play a critical role in suicide postvention, supporting those bereaved by suicide and preventing adverse outcomes. However, many responders feel uncertain about how to effectively support those bereaved by suicide and often lack formal postvention training. This study aimed to explore and understand the experiences and train
