ETHNOPHARMACOLOGICAL RELEVANCE: The Chinese formula Guben-Jiannao Ye (GBJNY) formula has a long history of usage in traditional Chinese medicine (TCM) for the treatment of learning and memory disorders as well as senile insomnia. This formulation is derived from Sun Simiao's five tonic pills. Furthermore, modern pharmacological investigations have revealed its ability to improve cognitive impairment and ameliorate sleep-wake circadian rhythm disorders. However, the precise mechanism underlying its efficacy remains elusive.
AIM OF THE STUDY: The current research explored the modulatory effects

OBJECTIVE: Although the classical Cox-Maze III is the gold standard surgical therapy with a proven efficacy in the treatment of atrial fibrillation (AF), complexity of this procedure has resulted in a search for a simpler, less invasive and more cost-effective method. In this study we evaluated the results of cryosurgical ablation in the treatment of chronic AF in patients undergoing concomitant mitral valve surgery.
METHODS: Ninety patients (mean age: 50.9+/-12 years) with chronic AF who were having mitral valve surgery as the main procedure underwent cryoablation with a newly designed N(2)O

Ophthalmologic surgical interventions are performed in numerous cases under general anaesthesia with halothane, many of which requiring a preoperatory mydriasis. Neosinerfin is alpha-adrenoceptive sympathomimetic. Local use, in a 10% concentration produces a strong mydriasis, resistant to the opening of the aqueous chamber during the intervention. Its systemic effects are remarkable: it induces a peripheral blood vessel constriction which can unleash a reflex bradycardia followed by a shortcoming of the cardiac flow. The halothane has various effects upon the heart: reduces the cardiac frequen

BACKGROUND: During recent years, systematic reviews of observational studies have compared digoxin to no digoxin in patients with atrial fibrillation or atrial flutter, and the results of these reviews suggested that digoxin seems to increase the risk of all-cause mortality regardless of concomitant heart failure. Our objective was to assess the benefits and harms of digoxin for atrial fibrillation and atrial flutter based on randomized clinical trials.
METHODS: We searched CENTRAL, MEDLINE, Embase, LILACS, SCI-Expanded, BIOSIS for eligible trials comparing digoxin versus placebo, no interven

A 68-year-old woman presented with palpitations, though ECG monitoring showed sinus rhythm. Multimodal imaging (cardiac magnetic resonance and echocardiography) showed asymmetric septal hypertrophy with left ventricular outflow tract obstruction (resting gradient of up to 87 mmHg). Echocardiography showed severe mitral regurgitation with multiple regurgitant jets and systolic anterior motion of the mitral valve. Mitral valve anatomy was abnormal, with anterior leaflet restriction and a clefted posterior leaflet. Symptoms persisted despite medical therapy (beta-blockers), and following multidis

We report the case of a 68-year-old male, presenting with persistent atrial fibrillation (Pe AF) refractory to anti arrhythmic medications and cardioversion, on a background history of ischaemic heart disease. Pre and post standard pulmonary vein isolation (PVI), left atrial (LA) voltageanalyses wereperformed, followed by dynamic local activation time (DLAT) mapping in addition to focal activity identification.Thisdemonstrated a heavily scarred LA, and a number ofareas of focal activity. The patient remained in atrial fibrillation (AF) post rotor (focal activity) targeting, howevernotable chang

Electrical storm (ES) is not a rare clinical entity. Sympathetic hyperactivity may play critical role in development and continuation of ES. Some recent reports have demonstrated that catheter-based renal sympathetic denervation (RSD) may reduce sympathetic activation and have a potential role in reducing arrhythmic burden. A 46-year-old man was admitted to clinic with frequent implantable cardioverter defibrillator shocks and ES related to catecholaminergic polymorphic ventricular tachycardia (VT). Tachycardia was unresponsive to administration of beta-blockers, verapamil, and flecainide. Cat

BACKGROUND: Propranolol is a highly lipid-soluble beta-blocker. We describe a case of severe propranolol overdose, with atypical dysrhythmia--wide complex tachycardia--which was successfully treated with lipid emulsion.
CASE REPORT: A 31-year-old woman ingested approximately 3.6 g of propranolol along with ethanol. Clinical manifestations of poisoning included coma, seizures, respiratory failure, hypoglycaemia, circulatory shock, and dysrhythmias. An ECG revealed nonspecific intraventricular conduction delay, followed by wide complex supraventricular tachycardia. Toxicological analysis of blo

Clinical practice guidelines recommend cardiac resynchronization therapy (CRT) for ambulatory New York Heart Association (NYHA) class IV patients with a QRS duration >or= 120 ms and a left ventricular ejection fraction <or= 35%. Only two prospective, randomized trials have compared outcomes after CRT in NYHA class III and IV patients. CRT improved mortality, exercise capacity, and quality of life in class IV patients, but the 1-year mortality remained high. Patients in these trials were in sinus rhythm at randomization and most patients had a left bundle branch block. Less data are available f

ACTH and lipotropins (beta- and gamma-LPH) are synthesized from a common precursor by the pituitary corticotropic cell. We have measured LPH plasma levels under physiological and pathological conditions and we have compared them with ACTH plasma levels in the same circumstances. Spontaneous variations (nycthemeral rhythm) in LPH, ACTH and cortisol plasma levels were parallel, while responses to Dexamethasone freination test and stress (Insulin induced hypoglycemia) or more specific stimulation (Metopirone, lysine-vasopressin) were parallel and superimposable. LPH levels were always higher than

BACKGROUND: Neural oscillations support perception, attention, and higher-order decision making. Aberrations in the strength or consistency of these oscillations in response to stimuli may underlie impaired visual perception and attention in schizophrenia. Here, we examined the phase and power of alpha oscillations (8-12 Hz) as well as aspects of beta and theta frequency oscillations during a demanding visual sustained attention task.
METHODS: Patients with schizophrenia (n = 74) and healthy control participants (n = 68) completed the degraded stimulus continuous performance task during elect

OBJECTIVES: To examine circadian changes in the sympathovagal balance, the activity of the renin-angiotensin system and hemostatic variables in patients with stable coronary artery disease, and the effects of beta-adrenoceptor blockade and angiotensin-converting enzyme inhibition.
BACKGROUND: Sympathovagal balance and key components of the fibrinolytic system show circadian variability. The effects of beta-adrenergic blocking agents and angiotensin-converting enzyme inhibitors on these autonomic and hemostatic rhythms are not well defined.
METHODS: Twenty patients with coronary artery diseas

BACKGROUND: P-wave dispersion (PWD), has been defined as the difference between maximum and minimum P-wave duration. Prolonged P-wave duration and increased PWD have been reported to be related with increased risk for atrial fibrillation (AF). AF is the most common sustained arrhythmia encountered in patients with rheumatic mitral stenosis (MS). Beta-blockers are the mainstay of therapy in patients with rheumatic MS to control ventricular rate both during sinus rhythm and AF. In the present study, we aimed to evaluate the effect of long-term beta-blocker therapy on P-wave duration and PWD in p

Postoperative atrial fibrillation (POAF) after cardiac surgery is associated with elevated morbidity and mortality. Although current prediction models have limited efficacy, several perioperative interventions can reduce patients' risk of POAF. These begin with preoperative medications, including beta-blockers and amiodarone. Moreover, patients should be screened for preexisting atrial fibrillation (AF) so that concomitant surgical ablation and left atrial appendage occlusion can be performed in appropriate candidates. Intraoperative interventions such as posterior pericardiectomy can reduce m

INTRODUCTION: Sleep disturbances are highly prevalent across most major psychiatric disorders. Alterations in the hypothalamic-pituitary-adrenal axis, neuroimmune mechanisms, and circadian rhythm disturbances partially explain this connection. The gut microbiome is also suspected to play a role in sleep regulation, and recent studies suggest that certain probiotics, prebiotics, synbiotics, and fecal microbiome transplantation can improve sleep quality.
METHODS: We aimed to assess the relationship between gut-microbiota composition, psychiatric disorders, and sleep quality in this cross-sectio

Serotonin N-acetyltransferase (NAT) activity and melatonin show a daily rhythm with high levels at night. Although the rhythmic properties of NAT and melatonin are similar in pineal gland and retina, great differences in the light perception and transmission mechanisms exist. We have analyzed the effects of adrenergic and dopaminergic agents on greenfrog (Rana perezi) eyecup culture, in order to identify the receptors involved in the regulation of retinal melatonin synthesis. A D2-like receptor is directly involved in the regulation of NAT activity and melatonin release in R. perezi retina. Qu

Interpersonal movement coordination is a foundational feature of human sociality, scaffolding communication, affiliation, and joint action from infancy onward. Whether this behavioral coupling is accompanied by genuine coupling between brains - rather than one manufactured by shared sensory input or motion artifact - has proven difficult to establish. We recorded dual-brain EEG with synchronized head-mounted accelerometry from participant-partner dyads (16 dyads, 155 dyad-condition recordings) across six conditions grading interpersonal coordination from stillness to fully reciprocal joint mov

Small cell lung carcinoma (SCLC) is a high-grade pulmonary neuroendocrine carcinoma strongly associated with smoking and commonly presents with advanced intrathoracic disease. Atrial fibrillation (AF) is common in acutely ill patients and in patients with cancer; however, causal attribution in an individual patient is often difficult because multiple arrhythmogenic stressors coexist. A 72-year-old man with hypertension, morbid obesity, peripheral arterial disease, heavy prior tobacco exposure, and no known prior cardiac arrhythmia or structural heart disease presented with progressive dyspnea,
