[The significance and problem of cyclosporine blood level monitoring].

Cyclosporine, a potent immunosuppressive agent used to prevent rejection of transplanted organs, has a narrow therapeutic range and various toxic effects, mostly concentration-dependent. The kinetics of this drug present a large intra- and interindividual variability due to many factors, resulting in marked variations of blood cyclosporine concentrations, and in a poor correlation between administered dose and concentration. The knowledge of cyclosporine peculiarities and of factors affecting blood concentrations can provide a rational basis for establishing an adequate therapy for the individ

Gender Performativity Norms and Friendship Networking Processes: An Epidemiological Perspective From a Longitudinal Cohort of Adolescents.

INTRODUCTION: Adolescence is a stage of development in which sensitivity to social norms and peer influences becomes more pronounced, and individuals are expected to conform to social group identities and adopt the normative behaviors of their peers, including gender role norms. However, little is known about the reciprocal effects between gender performativity and the friendship networking process in Taiwanese adolescents. Therefore, this study aimed to adopt a social network analysis to study the relationship mentioned above using a representative cohort study. METHODS: Empirical data were

Genomic variability in Zika virus in GBS cases in Colombia.

Major clusters of Guillain-Barré Syndrome (GBS) emerged during the Zika virus (ZIKV) outbreaks in the South Pacific and the Americas from 2014 to 2016. The factors contributing to GBS susceptibility in ZIKV infection remain unclear, although considerations of viral variation, patient susceptibility, environmental influences, and other potential factors have been hypothesized. Studying the role of viral genetic factors has been challenging due to the low viral load and rapid viral clearance from the blood after the onset of Zika symptoms. The prolonged excretion of ZIKV in urine by the time of

Inequity in biological DMARD prescription for spondyloarthritis across the globe: results from the ASAS-COMOSPA study.

OBJECTIVES: The value of biological disease-modifying antirheumatic drugs (bDMARDs) in spondyloarthritis (SpA) is well recognised, but global access to these treatments can be limited due to high costs and other factors. This study explores country variation in the use of bDMARDs in SpA in relation to country-level socioeconomic factors. METHODS: Patients fulfilling the Assessment in SpondyloArthritis International Society (ASAS) SpA criteria in the multinational, cross-sectional ASAS Comorbidities in Spondyloarthritis study were studied. Current use of bDMARDs or conventional synthetic DMARD

Applications of artificial intelligence in personalized nutrition interventions.

Unhealthy dietary patterns remain a major modifiable contributor to the global burden of obesity, type 2 diabetes mellitus (T2DM), cardiovascular disease, and cancer. However, individuals can respond markedly differently to identical foods, limiting the effectiveness of one-size-fits-all dietary guidance. This review examines the application of artificial intelligence (AI) to personalized nutrition interventions across three interconnected layers. At the biological level, inter-individual variability can be characterized by three mechanistic dimensions: nutrigenomic variation, which influences

Development and validation of risk prediction algorithms based on cone beam computed tomography and intraoral scanning fusion data to estimate whether biological width is at a low value: a cross-sectional study.

BACKGROUND: Biological width (BW) safeguards the periodontal ligament and alveolar bone. A reduced BW value may lead to periodontal complications. This study aims to develop and validate digital risk prediction algorithms to assess whether BW is low in adults with good oral hygiene. METHODS: Four thousand four hundred twenty sites of 740 teeth from 55 adults with good oral hygiene were used for model development and validation. A mixed-effects logistic regression model was used to derive a risk prediction equation for low BW, incorporating CBCT, intraoral scanning (IOS), and periodontal probi

Refining Diagnosis of Schistosoma haematobium Infections: Antigen and Antibody Detection in Urine.

Background: Traditional microscopic examination of urine or stool for schistosome eggs lacks sensitivity compared to measurement of schistosome worm-derived circulating antigens in serum or urine. The ease and non-invasiveness of urine collection makes urine an ideal sample for schistosome antigen detection. In this study several user-friendly, lateral-flow (LF) based urine assays were evaluated against a composite reference that defined infection as detection of either eggs in urine or anodic antigen in serum. Method: In a Tanzanian population with a S. haematobium prevalence of 40-50% (S. ma

Longitudinal flux balance analyses of a patient with episodic colonic inflammation reveals microbiome metabolic dynamics.

We report the first use of constraint-based microbial community modeling on a single individual with episodic inflammation of the gastrointestinal tract, who has a well documented set of colonic inflammatory biomarkers, as well as metagenomically-sequenced fecal time series covering seven dates over 16 months. Between the first two time steps the individual was treated with both steroids and antibiotics. Our methodology enabled us to identify numerous time-correlated microbial species and metabolites. We found that the individual's dynamical microbial ecology in the disease state led to time-v

Telomere Length in Metabolic Syndrome: Mechanisms, Epidemiology and Clinical Implications: A Narrative Review.

Metabolic syndrome (MetS) is a major and growing global public health challenge, affecting a substantial proportion of adults worldwide and contributing markedly to the rising burden of type 2 diabetes, cardiovascular disease, metabolic dysfunction-associated steatotic liver disease, and premature mortality. Despite its high prevalence and systemic consequences, the biological mechanisms linking chronic metabolic stress to cellular ageing remain incompletely resolved. Telomere length (TL), particularly leukocyte telomere length (LTL), has emerged as a candidate integrative biomarker of biologi

Identification of novel biomarkers based on lipid metabolism-related molecular subtypes for moderately severe and severe acute pancreatitis.

BACKGROUND: Acute pancreatitis (AP) is an unpredictable and potentially fatal disorder. A derailed or unbalanced immune response may be the root of the disease's severe course. Disorders of lipid metabolism are highly correlated with the occurrence and severity of AP. We aimed to characterize the contribution and immunological characteristics of lipid metabolism-related genes (LMRGs) in non-mild acute pancreatitis (NMAP) and identify a robust subtype and biomarker for NMAP. METHODS: The expression mode of LMRGs and immune characteristics in NMAP were examined. Then LMRG-derived subtypes were

The Chromosome Passenger Complex (CPC) Components and Its Associated Pathways Are Promising Candidates to Differentiate Between Normosensitive and Radiosensitive ATM-Mutated Cells.

BACKGROUND: Sensitivity to ionizing radiation differs between individuals, but there is a limited understanding of the biological mechanisms that account for these variations. One example of such mechanisms are the mutations in the ATM (mutated ataxia telangiectasia) gene, that cause the rare recessively inherited disease Ataxia telangiectasia (AT). Hallmark features include chromosomal instability and increased sensitivity to ionizing radiation (IR). OBJECTIVES: To deepen the molecular understanding of radiosensitivity and to identify potential new markers to predict it, human ATM-mutated an

Genetic Regulation of Transcription in the Endometrium in Health and Disease.

The endometrium is a complex and dynamic tissue essential for fertility and implicated in many reproductive disorders. The tissue consists of glandular epithelium and vascularised stroma and is unique because it is constantly shed and regrown with each menstrual cycle, generating up to 10 mm of new mucosa. Consequently, there are marked changes in cell composition and gene expression across the menstrual cycle. Recent evidence shows expression of many genes is influenced by genetic variation between individuals. We and others have reported evidence for genetic effects on hundreds of genes in e

Reference change value of global longitudinal strain in clinical practice: A test-rest quality implementation project.

BACKGROUND: Reference change value (RCV) is used to assess the significance of the difference between two measurements after accounting for pre-analytic, analytic, and within-subject variability. The objective of the current study was to define the RCV for global longitudinal strain (GLS) using different semi-automated software in standard clinical practice. METHODS: Using a test-retest study design, we quantified the median coefficient of variation (CV) for GLS using AutoStrain and Automated Cardiac Motion Quantification (aCMQ) by Philips. Associations of FTO and CLOCK Genetic Variants with Emotional Eating and Reward-Related Appetite Regulation Among Healthy Young Adult Males: An Exploratory Secondary Analysis.

Background: Patterns of dysregulated eating, including overeating, frequent snacking, and heightened food cravings, are associated with an increased risk of obesity and metabolic disease. Eating behaviors are multidimensional and can influence many factors, including social, cultural, and biological processes. Emerging evidence suggests that genetic variation may contribute to inter-individual differences in appetite regulation and reward-related eating, potentially influencing susceptibility to dysregulated eating patterns and behaviors. Objectives: This exploratory, secondary analysis invest

Variation in Methylmercury Metabolism and Elimination in Humans: Physiological Pharmacokinetic Modeling Highlights the Role of Gut Biotransformation, Skeletal Muscle, and Hair.

The biological half-life (t1/2) of methylmercury (MeHg) shows considerable individual variability (t1/2 < 30 to > 120 days), highlighting the importance of mechanisms controlling MeHg metabolism and elimination. Building on a prior physiologically based pharmacokinetic (PBPK) model, we elucidate parameters that have the greatest influence on variability of MeHg t1/2 in the human body. Employing a dataset of parameters for mean organ volumes and blood flow rates appropriate for man and woman (25-35 years) and child (4 - 6 years), we demonstrate model fitness by simulating data from our prior co

Intra-patient potassium variability after hypothermic cardiac arrest: a multicentre, prospective study.

BACKGROUND: To date, the decision to set up therapeutic extra-corporeal life support (ECLS) in hypothermia-related cardiac arrest is based on the potassium value only. However, no information is available about how the analysis should be performed. Our goal was to compare intra-individual variation in serum potassium values depending on the sampling site and analytical technique in hypothermia-related cardiac arrests. METHODS: Adult patients with suspected hypothermia-related refractory cardiac arrest, admitted to three hospitals with ECLS facilities were included. Blood samples were obtained

Dynamic changes in the proteome of human peripheral blood mononuclear cells with low dose ionizing radiation.

Humans are continually exposed to ionizing radiation from natural as well as anthropogenic sources. Though biological effects of high dose radiation exposures have been well accepted, studies on low-to-moderate dose exposures (in the range of 50-500 mGy) have been strongly debated even as researchers continue to search for elusive 'radiation signatures' in humans. Proteins are considered as dynamic functional players that drive cellular responses. However, there is little proteomic information available in context of human exposure to ionizing radiation. In this study, we determined differenti

Sex differences in DNA methylation assessed by 450 K BeadChip in newborns.

BACKGROUND: DNA methylation is an important epigenetic mark that can potentially link early life exposures to adverse health outcomes later in life. Host factors like sex and age strongly influence biological variation of DNA methylation, but characterization of these relationships is still limited, particularly in young children. METHODS: In a sample of 111 Mexican-American subjects (58 girls , 53 boys), we interrogated DNA methylation differences by sex at birth using the 450 K BeadChip in umbilical cord blood specimens, adjusting for cell composition. RESULTS: We observed that ~3% of CpG

Age-induced changes in the oral mucosa and their therapeutic consequences.

The oral mucosa is subject to specific changes with increasing age but these do not by themselves impair the function and well-being of the patient. The dentist should, however, devote particular attention to the reactions of the oral mucosa when it has undergone age-induced changes, to the effects, extending over years and even decades, of foreign bodies such as prostheses and of other agents--in particular, smoking. Changes are brought about mainly by arteriosclerotic processes, the progressive obliteration of the capillaries and the reduction of cell metabolism. Nerves and end organs in the
