Determinants of oxidant stress in extremely low birth weight premature infants.

Early in life, premature neonates are at risk of oxidant stress. They often require total parenteral nutrition (TPN), which is, however, contaminated with oxidation products. Coadministration of parenteral multivitamins (MVP) with a lipid emulsion (LIP) prevents lipid peroxidation. We hypothesized that LIP+MVP induces a lower oxidant load compared to preparations in which MVP is administered with an amino acid solution (AA+MVP). The aim of this study was to compare markers of oxidant stress in premature neonates receiving LIP+MVP, either exposed to or protected from light, or AA+MVP. Antioxida

Outcome of low birth weight infants in Diyala province of Iraq.

OBJECTIVES: To measure birth weight-specific mortality rates in the Diyala Province of Iraq and to determine if the causes of neonatal death could identify interventions needed to reduce neonatal mortality rates. METHOD: We retrospectively compared the outcome of 196 neonates with birth weight 500-2499 g admitted in 2003 with 252 such neonates admitted in 2009. RESULTS: The mortality rate in very low birth weight infant (VLBWI) increased from 12/80 (15%) in 2003 to 51/152 (33.6%) in 2009, (p = 0.003). In LBWI, 10/116 (8.6%) died in 2003 compared to 33/152 (13%) in 2009 (p = 0.29). Sepsis acc…

Economic outcomes in young adulthood for extremely low birth weight survivors.

OBJECTIVE: The goal was to compare educational attainment and labor market outcomes in young adulthood (21-26 years of age) for a Canadian, population-based cohort of 149 extremely low birth weight (ELBW) (<1000 g) survivors and a normal birth weight (NBW) cohort of 133 young adults from the same geographic area who were matched to the ELBW cohort in childhood. METHODS: We estimated the effects of ELBW status, according to gender, on continuous outcomes through least-squares regression and those on binary outcomes through logistic regression. We controlled for family background and considered…

Birth weight is a significant risk factor for incident atrial fibrillation.

BACKGROUND: Few if any studies have assessed the relationship between birth weight and incident atrial fibrillation (AF). METHODS AND RESULTS: From 1993 to 2009, we prospectively followed 27 982 women who were >45 years of age and free of cardiovascular disease and AF at baseline. Information on birth weight was categorized into 5 different categories: <2.5, 2.5 to 3.2, 3.2 to 3.9, 3.9 to 4.5, and >4.5 kg. The primary outcome was time to incident AF. During 14.5 years of follow-up, 735 AF events occurred. Age-adjusted incidence rates for incident AF from the lowest to the highest birth weight…

Pregnancy outcome after age 40 and risk of low birth weight.

A historical cohort study was conducted to examine the pregnancy outcome in women aged 40 or older and determine the effect of age on low birth weight. The pregnancy outcomes of 789 mothers aged 40 years or older were analysed and compared with those of 20,852 mothers aged 20-34 years. There were differences in socioeconomic status and obstetric characteristics between the two groups. The older group had more medical and obstetric complications (diabetes mellitus, chronic hypertension, malpresentation, pregnancy-induced hypertension, placenta praevia, multiple pregnancies, pre-term labour, fet…

Aggressive vs. conservative phototherapy for infants with extremely low birth weight.

BACKGROUND: It is unclear whether aggressive phototherapy to prevent neurotoxic effects of bilirubin benefits or harms infants with extremely low birth weight (1000 g or less). METHODS: We randomly assigned 1974 infants with extremely low birth weight at 12 to 36 hours of age to undergo either aggressive or conservative phototherapy. The primary outcome was a composite of death or neurodevelopmental impairment determined for 91% of the infants by investigators who were unaware of the treatment assignments. RESULTS: Aggressive phototherapy, as compared with conservative phototherapy, signific…

EEG functional connectivity in term age extremely low birth weight infants.

OBJECTIVE: The hypothesis is tested that electrocortical functional connectivity (quantified by coherence) of extremely low birth weight (ELBW) infants, measured at term post-menstrual age, has regional differences from that of full term infants. METHODS: 128 lead EEG data were collected during sleep from 8 ELBW infants with normal head ultrasound exams and 8 typically developing full term infants. Regional spectral power and coherence were calculated. RESULTS: No significant regional differences in EEG power were found between infant groups. However, compared to term infants, ELBW infants h…

Early treated hypotension and outcome in very low birth weight infants.

BACKGROUND: Early hypotension is a common problem among preterm infants. Studies have shown conflicting data regarding the definition of hypotension, the way to treat it and the correlation to outcome. OBJECTIVES: To investigate the risk factors for developing hypotension and its relations to short- and long-term outcomes. METHODS: Medical charts of all surviving very low birth weight infants were retrospectively reviewed during a 4-year period. The data of infants suffering from early hypotension and needed treatment were compared with those of a control group with 'normal' blood pressure. …

Analysis of birth weight in Metu Karl hospital: South West Ethiopia.

BACKGROUND: Birth weight is a sensitive indicator for predicting the chances of both infant survival and healthy child growth and development. Low birth weight continues to remain a major public health problem related to poverty, social factors such as lower status of women, malnutrition, lack of anti-natal care, etc., in Ethiopia in contrast to what is observed in many developing countries. The objective of this study was to determine birth weight distribution and its association with some risk factors among newborns delivered in Metu Karl hospital Metu town (South-West Ethiopia) from Februar…

Gestational age, birth weight, intrauterine growth, and the risk of epilepsy.

The authors evaluated the association between gestational age, birth weight, intrauterine growth, and epilepsy in a population-based cohort of 1.4 million singletons born in Denmark (1979-2002). A total of 14,334 inpatients (1979-2002) and outpatients (1995-2002) with epilepsy were registered in the Danish National Hospital Register. Children who were potentially growth restricted were identified through two methods: 1) sex-, birth-order-, and gestational-age-specific z score of birth weight; and 2) deviation from the expected birth weight estimated based on the birth weight of an older siblin…

[Chorioamnionitis and lung damage in the extremely low birth weight infant].

UNLABELLED: Some experimental work suggests that exposure to intrauterine infection is associated, not only, with lung maturation and a reduced risk of respiratory distress syndrome, but also, with delayed alveolarization and increased risk of bronchopulmonary dysplasia. AIM: To evaluate the association between histological chorioamnionitis and lung disease in extremely low birth weight preterm infants. METHODS: A retrospective chart review of 63 less than 1000 g birthweight, appropriated for gestational age neonates, delivered at three tertiary medical centers in the north of Portugal, betw…

Postnatal outcome in gastroschisis: effect of birth weight and gestational age.

INTRODUCTION: Early elective delivery of antenatally diagnosed gastroschisis has been proposed as a strategy to minimize postnatal morbidity. This hypothesis was tested by analyzing outcome in relationship to gestational age and birth weight at delivery. METHODS: Single-center retrospective review of infants born with gastroschisis over a 13-year period (January 1993-December 2005). Standard outcome measures were compared using nonparametric methods. Data are quoted as median values (range). RESULTS: The study population consisted of 110 infants with gastroschisis. They were divided accordin…

Memory performance in a sample of very low birth weight adolescents.

Prematurely born participants with very low birth weight (VLBW) are at high risk of brain injury in the perinatal period and of later cognitive impairment. Studies of long-term memory sequelae in VLBW participants are scarce and focus on verbal and visual memory assessed by standard clinical memory tests. There is even less research into everyday memory, and the results obtained are contradictory. This study explores long-term memory deficits in VLBW adolescents using 2 standard clinical memory tests and 1 everyday memory test. Results show impairment only in everyday memory. These memory defi…
