We report the results of 36 femoral lengthenings in 30 consecutive patients using the Ilizarov technique. Patient age at surgery in 19 boys and 11 girls averaged 13.4 years (range, 5-18). Minimum follow-up was 2 years. The etiology of femoral shortening was congenital in 21 femora and acquired in 15. The orthoplastic treatment of post-traumatic bone infections is complex and requires a multidisciplinary approach using both orthopedic and … membrane technique (MIMT), and soft tissue coverage with free flap. The Bridle procedure is a tritendon anastomosis between the posterior tibialis, anterior tibialis, and peroneus longus, combined with an Achilles tendon lengthening for treating equinus and equinovarus deformities. The technique avoids problems of tendon attachment to bone and tendon placement for balance. One hundred seven procedures were performed on patients with cerebral palsy with 74% excellent and good results overall. The average follow-up was 5 years 9 months. The procedure was also performed with mixed results, on smaller groups of patients with other neuromuscular diseases.

This retrospective case series evaluates the technique of transverse debridement, acute shortening and subsequent distraction histogenesis in the management of open tibial fractures with bone and soft tissue loss, thereby avoiding the need for a soft tissue flap to cover the wound. Thirty-one patients with Gustilo grade III open tibial fractures between 2001 and 2011 were initially managed with transverse wound extensions, debridement and shortening to provide bony apposition and allowing primary wound closure without tension, or coverage with mobilization of soft tissue and split skin graft.

Congenital femoral deficiencies remain a surgical challenge. There is no common classification: both radiological and clinical methods are recommended. A one-moment correction of associated defects of the lower extremities was performed in 45 patients. An analysis of treatment of such patients enabled us to elaborate indications to operative interventions concerned and surgical technic. The treatment was terminated in 38 patients. In 33 of them weight-bearing of the extremity was restored by means of ankylosis of the coxa with simultaneous lengthening of the extremity and correction of concomitant deformities. In 3 patients no ankylosis in the joint developed, in two-pseudarthrosis at the site of osteotomized femoral bone was formed.
