The most common cause of nail bed deformity is trauma, but other causes are infection, tumor, ischemia, or congenital anomalies. This article includes discussions of nonadherence, split nail, reconstruction of the eponychium, crooked and hooked nail, bony irregularity, pachyonychia, ischemic deformities, and absence of the nail. New problems are encountered daily and other deformities have no method of correction recorded in the literature. For these cases, we provide suggested treatments based on the anatomy and physiology of the nail.

BACKGROUND: Intramedullary nailing of tibial shaft fractures has been common practice for decades. Nevertheless, complications occur frequently, and subsequent surgery is often required. To improve our understanding on how we may improve trauma care for patients with tibial shaft fractures, this study systematically reviewed all currently available evidence to assess the incidence of complications and rate of re-operations following intramedullary nailing of traumatic tibial fractures.
METHODS: Trip Database, Medline, Scopus and Cochrane Library were searched on September 7th, 2018. Searches

BACKGROUND: There are various distal locking options for the repair of tibia distal diametaphyseal fractures with intramedullary nails. There is no consensus about the superiority of any of these distal locking options. In our study, comparing nails with distal bolt locking screw (DSBLS) and conventional nails; we aimed to compare the clinical and radiological results of intramedullary nail models in fixing tibial distal diametaphyseal fractures.
METHODS: In our orthopedics and traumatology department, 117 tibial distal diametaphyseal fractures of 116 patients treated with intramedullary nail

We report a case of knee fusion after a failed total knee arthroplasty (TKA) with severe osteolysis including the epicondyle and ipsilateral total hip arthroplasty (THA) with long Wagner revision stem (Sulzer Orthopedics, Baar, Switzerland). The conventional devices for arthrodesis were unavailable in this case because of the long Wagner revision stem and poor bone stock. A connector was made between the long Wagner revision stem and an intramedullary nail (IM nail; Solco, Seoul, Korea). The custom-made connector was coupled with a femoral stem by cylindrical taper fit with additional cement a

Marrow nailing developed by Gerhard Küntscher for long tubular bones is an excellent surgical procedure for fractures of the lower leg shaft. It yields outstanding results both with the typical good indications in respect of shape of fracture, localisation and soft tissue conditions and even with atypical borderline and emergency indications where the healing results can be said to be good and satisfactory. Of course long-standing experience is required especially when employing the Küntscher intramedullary nail in the atypical range of indications. This method produces good results even in cr

BACKGROUND: The removal of implants such as intramedullary nails is one of the most common operations in orthopedic surgery. The indications for orthopedic implants removal will always remain a subject of conversation and hardly supported by literature. The aim of this study to report injuries of treatment in tibial nail removal and to determine if there are fracture characteristics, patient demographics, or surgical details that may predict a complication.
METHODS: This is a retrospective seven-year (2010-2016) study including a total of 389 tibial intramedullary nail removals at the Helsink

Psoriatic onycho-pachydermo periostitis (POPP) is characterized by psoriatic onychodystrophy, connective tissue thickening, and periostitis of the distal phalanges (DPs), producing a drumstick-like deformity. Our aim was to present the first case of POPP treated successfully with an IL-17 inhibitor, perform a literature review of its characteristics and treatment, and explore the possible pathogenesis. We conducted a systematic review of previously presented POPP cases. We present a patient with methotrexate (MTX)-resistant treatment POPP, who had significant resolution of symptoms and inflamm

BACKGROUND: This study aims to investigate the effects of early weightbearing after intramedullary fixation of trochanteric fractures.
METHODS: Femurs with different types of trochanteric fractures were modeled according to AO/OTA classification. Fractures were ideally reduced with one mm gap between fragments and fixed with intramedullary nails. Forces were applied simulating single- (Body weight: 60 kg, joint reaction force: 1999.2 N, abductor muscle force:1558.8 N) and double-leg standing positions (Joint reaction force: 196 N). In another model, a 500 Nm rotational force was applied as a

UNLABELLED: Proximal tibial deformities, particularly varus deformities, are relatively common in adolescents and young adults. The etiology of these deformities is often untreated or undercorrected infantile and adolescent Blount disease. Other less common etiologies include metabolic bone disease-associated deformities, posttraumatic and iatrogenic growth disturbance, and deformity related to surgical treatment or radiation for tumors1-3. We apply the principles of fixator-assisted acute deformity correction, mostly described for use at the distal aspect of the femur, as well as the principl

We report a difficult healing process after a femoral shaft fracture in childhood. We present surgical correction options of femoral shortening due to pseudarthrosis after elastic stable intramedullary nailing. First, we tried to establish distraction using an external fixator, followed by plate osteosynthesis. After material failure of plate osteosynthesis, we treated the refracture with intramedullary nailing, after which bone healing occurred.

An intercalary segmental allograft is an option for limb salvage in bone tumours. Stable and congruent intercalary reconstructions are a prerequisite for achieving host-graft union. However, a too rigid fixation could increase the risk of late complications correlated with negative bone remodelling. This study compared the reconstruction stiffness achieved by three different host-graft junctions, namely, end-to-end, modified step-cut, and taper. A low-stiffness bone plate was used as the fixation method, except for the taper junction where a low-stiffness intramedullary nail was also used to i

INTRODUCTION: Chronic osteomyelitis (Chr OM) may result as a sequel of acute hematogenous infection or following open fractures or orthopedic surgeries. Among various osteoarticular infections, Chr OM is one of the most challenging in terms of treatment. In the present study, we evaluate the outcome of antibiotic cement-coated nails in the treatment of chronic pyogenic osteomyelitis and also discuss the future innovations in field of antibiotic-compatible biomaterials for coating the implants.
MATERIALS AND METHODS: Twelve cases of Chr OM (5 hematogenous and 7 exogenous following trauma or su

We present a new option for treatment of post-total knee arthroplasty periprosthetic infection associated with bone destruction and massive loss-use of an antibiotic-impregnated cement rod-spacer. This rod-spacer can be custom- made, at time of surgery, with Steinmann pins, intramedullary nails, Rush rods, Harrington spine rods, bone cement (polymethylmethacrylate), and antibiotics. We used this technique in 9 cases of periprosthetic infection over a 6-year period. The rod-spacer provided stable fixation across the knee, local antibiotic delivery, maintenance of the joint space, and preservati

Non-union following intramedullary nailing of diaphyseal long bone fractures represents a serious complication necessitating timely and effective intervention. The situation becomes considerably more complex in the presence of a broken femoral nail, which poses additional technical challenges for the operating surgeon. Conventional management strategies typically entail removal of the existing nail, followed by re-fixation with a larger intramedullary nail, plate fixation, or external fixation constructs. Our case describes a 34-year-old male patient who presented with aseptic femur non-union

Twenty-seven patients with complex femoral fractures (combined shaft and proximal femoral fractures) were treated with a modified Grosse-Kempf slotted locking nail (cephalomedullary nail), wherein two screws were inserted in the hip. Four types of complex, multifocal femoral fractures were represented in the series. Eleven of the femoral shaft fractures were secondary to a previous, internally fixed, not yet united hip fracture (type I). Ten comminuted peritrochanteric fractures occurred in normal bone (type II). Three similar fractures were pathologic because of metastasis. Two patients had a

INTRODUCTION: Both bone forearm fractures are common injuries in children. Most can be treated with reduction and casting. Those that fail nonoperative management can be treated with a plate or intramedullary fixation; however, refracture remains a problem. The goal of this study is to evaluate the refracture rate in both bone forearm fractures based on the mode of fixation.
METHODS: Institutional board review approval was obtained. A retrospective chart review from 2010 to 2021 at a single tertiary care institution was conducted for all operative patients <18 years who sustained a both bone

Children's limb fractures are commonly seen in general practice and emergency departments. Diagnostics relies on relevant clinical examination and good quality radiographs and will be supplemented with CT scanning, when necessary. The treatment of fracture must not interfere with the child's normal bone growth. Conservative treatment with reduction under anesthesia is the treatment of choice. With operative treatment, rigid fixation of the fracture is not necessary; K-wires and elastic intramedullary nails are widely used. Although the aim of the treatment is exact positioning of the fracture,

Dorsal compound traumatic thumb loss can be reconstructed in selected young and well-motivated patients with a custom-made partial great toe transfer. The transfer, including the nail complex and vascularized piece of bone, can be fashioned by longitudinally cutting the distal phalanx to approach the appearance of the opposite thumb and to allow for more normal nail appearance. Ten patients illustrate this technique and emphasize its reconstructive potential as well as cosmetic acceptance.
