Impaired trophic interactions between the cerebellum and the cerebrum among preterm infants.

BACKGROUND: Advanced neuroimaging techniques have brought increasing recognition of cerebellar injury among premature infants. The developmental relationship between early brain injury and effects on the cerebrum and cerebellum remains unclear. OBJECTIVES: To examine whether cerebral parenchymal brain lesions among preterm infants are associated with subsequent decreases in cerebellar volume and, conversely, whether primary cerebellar injury is associated with decreased cerebral brain volumes, with advanced, 3-dimensional, volumetric MRI at term gestational age equivalent. METHODS: Total cer

Magnetic resonance spectroscopy of diffuse brain trauma in the pig.

The acute metabolic events linked to the evolution of selective axonal pathology in the white matter following diffuse brain injury have not previously been evaluated due to the paucity of relevant experimental models. Here, we utilized a new model of inertial brain injury in the pig that selectively damages axons in the white matter, and applied proton and phosphorous magnetic resonance spectroscopy (MRS) to noninvasively monitor the temporal course of metabolic changes following trauma. Evaluating four pigs with MRS prior to injury, within 1 h and 3 and 7 days postinjury, we found that wides

Cerebellar white matter abnormalities following primary blast injury in US military personnel.

Little is known about the effects of blast exposure on the human brain in the absence of head impact. Clinical reports, experimental animal studies, and computational modeling of blast exposure have suggested effects on the cerebellum and brainstem. In US military personnel with isolated, primary blast-related 'mild' traumatic brain injury and no other known insult, we found diffusion tensor MRI abnormalities consistent with cerebellar white matter injury in 3 of 4 subjects. No abnormalities in other brain regions were detected. These findings add to the evidence supporting the hypothesis that

Biomarkers of neurological status in HIV infection: a 3-year study.

PURPOSE: To evaluate circulating cytokines and chemokines as correlates of the degree of brain injury in individuals with advanced human immunodeficiency virus (HIV) infection. EXPERIMENTAL DESIGN: Study participants included ten well-characterized subjects in advanced stage HIV infection. High-throughput multiplexed analysis was used to quantify markers of interest at baseline and 3 years later in the clinical course. Objective measurements of the brain were derived in vivo with quantitative magnetic resonance segmentation algorithms and with diffusion tensor imaging. RESULTS: Of the marker

Shaken baby syndrome: re-examination of diffuse axonal injury as cause of death.

The discussion surrounding shaken baby syndrome (SBS) arose from the lack of evidence implicating diffuse axonal injury (DAI) as a cause of death. It was assumed instead that injury to the cervical cord, medulla, and nerve roots played a causal role. The present pathomorphological study examines 18 selected infants (<1-year-old) whose deaths were highly suspicious for SBS, exhibiting the classical SBS triad of acute subdural hemorrhage (SDH), retinal bleeding, and encephalopathy. Gross autopsy and microscopic findings of these infants were compared with those of 19 victims of sudden infant dea

Glaucoma of the brain: a disease model for the study of transsynaptic neural degeneration.

The identification of mechanisms precipitating neuronal death and injury is an intense area of investigation requiring reliable models to assess the effects of neuroprotective agents. Most are suboptimal since the effects of initial damage are diffuse and may not be reproducible or easily quantifiable. The ideal laboratory model should have the ability to (a) clearly detect evidence of neuronal injury and recovery, (b) accurately measure morphologically the extent of these changes, and (c) provide functional evidence for damage and recovery. Glaucoma is a disease of visual neurons in the eye a

Intraventricular Hemorrhage and White Matter Injury in Preclinical and Clinical Studies.

Germinal matrix-intraventricular hemorrhage (IVH) occurs in nearly half of infants born at less than 26 weeks' gestation. Up to 50% of survivors with IVH develop cerebral palsy, cognitive deficits, behavioral disorders, posthemorrhagic ventricular dilatation, or a combination of these sequelae. After the initial bleeding and the primary brain injury, inflammation and secondary brain injury might lead to periventricular leukomalacia or diffuse white matter injury. Potential factors that are involved include microglia and astrocyte activation, degradation of blood components with release of "tox

Nitrosative and oxidative injury to premyelinating oligodendrocytes in periventricular leukomalacia.

Periventricular leukomalacia (PVL), the major substrate of cerebral palsy in survivors of prematurity, is defined as focal periventricular necrosis and diffuse gliosis in immature cerebral white matter. We propose that nitrosative and/or oxidative stress to premyelinating oligodendrocytes complicating cerebral ischemia in the sick premature infant is a key mechanism of injury interfering with maturation of these cells to myelin-producing oligodendrocytes and subsequent myelination. Using immunocytochemical markers in autopsy brain tissue from 17 PVL cases and 28 non-PVL controls, we found in t

Controlled cortical impact: a new experimental brain injury model.

A new experimental model of mechanical brain injury was produced in the laboratory ferret (Mustela putorius furo) using a stroke-constrained pneumatic impactor. Cortical impacts were made on vertex to the intact dura mater overlying the cerebral cortex with contact velocities ranging from 2.0 to 4.0 m/sec and with deformations of 2.0 to 5.0 mm. The dwell time of the impact and the stability of the skull during impact were verified with high speed (1000 to 3000 frames/sec) cineradiography. Systemic arterial blood pressure, heart rate, and respiration were monitored, and postinjury changes were

The Complexity of Secondary Cascade Consequent to Traumatic Brain Injury: Pathobiology and Potential Treatments.

According to the World Health Organization, Traumatic brain injury (TBI) is the major cause of death and disability and will surpass the other diseases by the year 2020. Patients who suffer TBI face many difficulties which negatively affect their social and personal life. TBI patients suffer from changes in mood, impulsivity, poor social judgment and memory deficits. Both open and closed head injuries have their own consequences. Open head injury associated problems are specific in nature e.g. loss of motor functions whereas closed head injuries are diffused in nature like poor memory, problem

Inflicted traumatic brain injury in infants and young children.

This article will discuss the subject of inflicted or abusive head injury in infants and young children. Inflicted neurotrauma is a very common injury and a frequent problem in attempting to distinguish between inflicted and accidental injury. Inflicted head injury occurs usually in the home in the presence of the individual who has inflicted the injury outside the view of unbiased witnesses. Distinguishing between inflicted and accidental injury may be dependent upon the pathological findings and consideration of the circumstances surrounding the injury. The most common finding in an inflicte

The Role of MRI in Debunking the Fallacy of "Mild" Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) is a prevalent yet often overlooked public health concern due to the absence of detectable abnormalities on CT or conventional MRI scans. Approximately 18.3%-31.3% of mTBI patients experience persistent symptoms 3-6 months post-injury, despite normal imaging results, making diagnosis and treatment challenging. In recent years, advanced neuroimaging modalities have emerged with the potential to reveal subtle physiological and structural brain changes that are invisible to traditional imaging. Diffusion MRI (dMRI), for instance, is particularly valuable for det

[Traumatic, infectious or degenerative pathology].

CRANIO-ENCEPHALIC TRAUMAS: Scanography remains the examination of choice. However, MRI can be useful in diagnosis of diffuse axional lesions, not clearly visualized with scanography, and for screening the subsequent lesions. INFECTIOUS OR INFLAMMATORY LESIONS: Some are very evocative with MRI: cerebral abscesses, notably herpetic encephalitis and Creutzfeldt-Jacob's disease. If multiple sclerosis is suspected, MRI is considered as the principle para-clinical examination able to confirm the diagnosis with the first episode. It also supplies data for the diagnosis of metabolic, toxic and degener

Brain Injury and Microstructural Brain Development in Very Preterm Infants with Patent Ductus Arteriosus.

INTRODUCTION: Evidence regarding the association between patent ductus arteriosus (PDA) and brain development in very preterm infants is inconclusive. The aim of the current study was to systematically evaluate brain injury and microstructural brain maturation as assessed by magnetic resonance imaging (MRI) at term-equivalent age in a contemporary cohort of very preterm infants with and without PDA. METHODS: This was a retrospective, single-center study. Preterm infants born at <32 weeks' gestation with PDA and cerebral MRI were eligible for this study. They were matched 1:1 according to gest

Craniomaxillofacial Complex Injuries Sustained During Training in Alpine Environments: Multimodal MRI-Based Analysis of Injury Patterns and Acute-Phase Assessment.

OBJECTIVE: Training activities in alpine environments, characterized by low temperatures, hypoxia, and high physical demand, predispose individuals to craniomaxillofacial (CMF) complex injuries with potential involvement of the central nervous system in the event of accidents. However, the imaging manifestations and multimodal features of brain injury within this specific environmental context remain insufficiently characterized. This study aimed to retrospectively analyze the brain injury patterns in patients with CMF injuries sustained during alpine training, utilizing multimodal magnetic re

Vibrational spectroscopy methods reveal biochemical changes associated with the glial scar formation after traumatic brain injury.

Traumatic brain injury (TBI) is a serious clinical and social problem. Millions of TBI cases, that require hospitalization and consequently burden social security systems, are reported each year. Analysis of the time course of changes that occur in the brain after primary injury may help indicate therapeutic goals and treatment directions that will minimize severe secondary effects of TBI. Existing animal models simulating the development of TBI in human are divided into two main groups, namely into diffuse and local models. Diffuse injury models are ideal for studying concussions and long-ter

Brain Tissue Oxygen Monitoring and the Intersection of Brain and Lung: A Comprehensive Review.

Traumatic brain injury is a problem that affects millions of Americans yearly and for which there is no definitive treatment that improves outcome. Continuous brain tissue oxygen (PbtO2 ) monitoring is a complement to traditional brain monitoring techniques, such as intracranial pressure and cerebral perfusion pressure. PbtO2 monitoring has not yet become a clinical standard of care, due to several unresolved questions. In this review, we discuss the rationale and technology of PbtO2 monitoring. We review the literature, both historic and current, and show that continuous PbtO2 monitoring is f
