[Resection of posterior mediastinal cyst under thoracoscopy].

A 52-year-old woman with left posterior mediastinal cyst underwent surgical removal using thoracoscopy. Under general anesthesia by use of o … Residual cyst-wall was burned by electric knife. A pathological diagnosis probed that the tumor was bronchogenic cyst without malignancy. Post operative pain markedly reduced. The patient rapidly recovered and was discharged on the 6th postoperative day. Thoracoscopic resection of mediastinum cyst may be a useful approach.

[Müllerian Cyst of the Posterior Mediastinum].

Müllerian cyst is rarely reported in the literature. Here, we reported a rare case of Müllerian cyst of the posterior mediastinum. The patient was a 46-year-old woman who was found to have an abnormal shadow on chest X-ray film at medical examination and was followed up as suspected of bronchogenic cyst. One year later, the lesion increased and she was referred to our hospital for surgery. Chest computed tomography demonstrated a 36 mm diameter cystic tumor in front of the right 4~5 thoracic vertebra level. We performed thoracoscopic surgery for diagnosis and treatment. Histopathological exami…

[Air-filled cysts of tracheal origin: nosological problems and actual frequency].

Air-filled paratracheal bronchogenic cysts are extremely rare. They are lined by respiratory epithelium. We have operated on 2 patients whose cysts were diagnosed by CT scan: one presenting with bloody sputum and the other with dysphagia. Both patients had a history of neck surgery. Because such cases are rare in the literature, their exact origin, whether tracheocele, diverticulum or other form of air-filled cyst, remains a matter of debate. However, their frequency is probably underestimated because most cause few symptoms and are well tolerated. In a recent radiologic study their prevalence…

Mediastinal cysts: diagnosis by CT and needle aspiration.

Five patients with bronchogenic or enteric cysts were diagnosed by using a combination of CT and transbronchial or transesophageal needle aspiration. Serous or mucoid fluid was successfully aspirated in each case. Cultures of the fluid were negative for pathogens, and cytologic examinations showed no malignancy. Mediastinoscopy and thoracotomy were avoided in all patients, including one symptomatic patient whose symptoms resolved after needle aspiration. CT with transbronchial or transesophageal needle aspiration represents a practical alternative to surgery in the evaluation of congenital med

Confronting Upside-Down Video Assisted Thoracic Surgery for Posterior Mediastinal Müllerian Duct Cysts.

A Müllerian duct cyst is a rare epithelial-lined cyst originating from remnants of the Müllerian (paramesonephric) duct. While these cysts are typically found along the lateral or posterior vaginal wall in females or near the prostate in males, posterior mediastinal occurrence is exceedingly rare. Accurate preoperative diagnosis is challenging, as imaging findings often resemble those of more common posterior mediastinal lesions such as neurogenic tumors or bronchogenic cysts. Surgical resection is usually required for definitive diagnosis and treatment. Video-assisted thoracic surgery (VATS) …

Posterior mediastinal Müllerian cyst in a patient with Mayer-Rokitansky-Küster-Hauser syndrome: a case report.

Posterior mediastinal cysts are rare lesions accounting for ~5%-10% of mediastinal cysts. Most are bronchogenic, enteric, or neurogenic in origin; however, Müllerian cysts have recently been recognized as a distinct entity thought to arise from displaced Müllerian duct tissue. Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome results from incomplete development of Müllerian ducts and is characterized by congenital absence of the uterus and upper vagina. A shared embryological origin suggests a possible association between MRKH syndrome and mediastinal Müllerian cysts. We report the first known ca

Clinicopathologic demonstration of complex bronchopulmonary foregut malformation.

Bronchopulmonary foregut malformations are rare and often prove to be extremely difficult to diagnose. The presented case is a 44-year-old man complaining about cough and hemoptysis. These symptoms continued despite medical treatment. A computed thorax tomographic scan depicted a paravertebral cystic lesion. The patient underwent surgical exploration, and communication between the cyst and the esophagus was found. A left lower lobectomy was performed. The pathologic result confirmed a duplication cyst of mixed bronchogenic and esophageal type with bronchiectasis of the lower lobe. We present t

Mediastinal Epidermoid Cyst in a 5-Year-Old Girl.

A 5-year-old girl was referred to our unit with an incidental finding of a lesion on the right hemithorax situated within the right atrial shadow. Computed tomography thorax showed a well-defined soft tissue lesion felt to be consistent with a bronchogenic cyst. The lesion was located in the posterior mediastinum, adherent to the diaphragm and inferior vena cava, but did not extend within the wall of the esophagus. It was entirely excised via video-assisted thoracoscopy converted to open thoracotomy. Histopathology confirmed an encapsulated nodular tissue measuring 2.5 × 2.5 × 2 cm lined by sq…

Congenital lung malformations.

Congenital lung malformations (CLMs) are rare developmental anomalies of the lung, including congenital pulmonary airway malformations (CPAM), bronchopulmonary sequestration, congenital lobar overinflation, bronchogenic cyst and isolated congenital bronchial atresia. CLMs occur in 4 out of 10,000 live births. Postnatal presentation ranges from an asymptomatic infant to respiratory failure. CLMs are typically diagnosed with antenatal ultrasonography and confirmed by chest CT angiography in the first few months of life. Although surgical treatment is the gold standard for symptomatic CLMs, a con…

Bronchogenic cysts always require resection because of their high complication rate (66%) after conservative treatment. In these cases complete resection is necessary due to the probability of recurrence. Ninety-eight percent of neurogenic tumors in adults are benign …

Thoracoabdominal duplication cyst: US, CT and MR findings.

A rare case of thoracoabdominal duplication is reported. Radiological workup including US, CT and MR provided the correct diagnosis. The US revealed the cystic nature of the lesions, whereas CT and MR showed their relationship with the diaphragm and each other. Coronal- and sagittal-plane MR images were helpful in demonstrating the extension of the lesions. The patient was discharged after an uneventful surgery and recovery. The histological evaluation revealed the cyst lined with single-layered columnar epithelium and the cyst wall contained submucosal mucous glands with underlying smooth mus

Cysts of the third ventricle.

Cysts of the third ventricle, predominantly being of congenital origin, are often summarized under the heading of ependymal or neuroepithelial cysts, if they contain an epithelial covering. Though surely they form a heterogeneous group, even by histochemistry and electron microscopy a differentiated classification on a histogenetic basis is difficult because of overlaps in structural details. The aim of this study is checking by immunohistochemistry with antibodies to cytokeratins (KL 1), epithelial membrane antigen (EMA), S 100 protein (S 100), and glial fibrillary acidic protein (GFAP). Flexible fibreoptic bronchoscopy in Basra, Iraq: a 20-month experience.

This prospective study represents our experiences in using fibreoptic bronchoscopy (FOB) in the evaluation of different thoracic lesions. Over a 20-month period, 203 patients (151 males and 52 females) (age range: 15-100 years) underwent bronchoscopies. The patients had a wide range of symptoms and/or radiographic abnormalities. The majority had cough and shortness of breath; haemoptysis was a common symptom. In all, 148 patients had neoplasms and 55 had non-neoplastic lesions. The most common malignancy was bronchogenic carcinoma (91 confirmed, 33 suspected). Other neoplasms included pulmonar
