Endoscopic resection of lipoma of the patellar tendon.

Synovial lipoma of the patellar tendon is a very rare entity. It can be associated with rupture of the patellar tendon. We present a case of synovial lipoma that was successfully resected endoscopically. The other indications for patellar tendoscopy include chronic patellar tendinitis and tendinosis, recalcitrant bursitis around the tendon, Osgood-Schlatter disease, and jumper's knee. The major potential danger of this endoscopic procedure is iatrogenic damage to the patellar insertion during endoscopic debridement in patients with jumper's knee or the tibial insertion during endoscopic debrid…

Endoscopic calcaneoplasty: an improved technique.

Haglund's syndrome is a triad of posterosuperior calcaneal prominence, retrocalcaneal bursitis, and insertional Achilles tendonitis. Endoscopic treatment of patients with this triad allows for earlier weight bearing, direct visualization of the Achilles insertion, and less soft tissue disruption. In addition, it allows excellent visualization of the pathology. The authors present a technique of endoscopic calcaneoplasty in the prone position with the monitor at the foot of the bed and the surgeon standing beside the patient. Prone positioning allows for easier access to the Haglund's deformity…

MR imaging of overuse injuries of the hip.

The aim of this article is to emphasize the importance of MR imaging in the evaluation of chronic hip pain and overuse injuries. Image interpretation of the hip can be difficult because of the complex anatomy and the varied pathology that athletes can present with, such as labral and cartilaginous injuries, surrounding soft tissue derangement involving muscles or tendons, and osseous abnormalities. The differential diagnosis in adults is diverse and includes such common entities as stress fracture, avulsive injuries, snapping-hip syndrome, iliopsoas bursitis, femoroacetabular impingement syndr…

MRI and US of gluteal tendinopathy in greater trochanteric pain syndrome.

Greater trochanteric pain syndrome is commonly due to gluteus minimus or medius injury rather than trochanteric bursitis. Gluteal tendinopathy most frequently occurs in late-middle aged females. In this pictorial review the pertinent MRI and US anatomy of the gluteal tendon insertions on the greater trochanter and the adjacent bursae are reviewed. The direct (peritendinitis, tendinosis, partial and complete tear) and indirect (bursal fluid, bony changes and fatty atrophy) MRI signs of gluteal tendon injury are illustrated. The key sonographic findings of gluteal tendinopathy are also discussed…

Extension of lumbar spine infection into osteoarthritic hip through psoas abscess.

We present a case of pyogenic lumbar discitis and septic hip arthritis, accompanied by a psoas abscess and pyogenic iliopsoas bursitis, for which the correct diagnosis was delayed. The patho-mechanism was speculated to be initial hematogenous infection in the lumbar spine that spread along the psoas muscle as a psoas abscess and then extended into the hip joint via the iliopsoas bursa. For an early correct diagnosis, clinicians should be aware that the lumbar spine and hip joint regions communicate through the psoas muscle space and iliopsoas bursa, making it possible for infection to spread.

Complications of arthroscopy of the hip.

Although arthroscopy of the hip is being carried out increasingly, little is known about the rate of associated complications. We describe a prospective study of 640 consecutive procedures in which a consistent technique was used. The overall complication rate was 1.6%. Complications, none of which was major or long-term, included transient palsy of the sciatic and femoral nerves, perineal injury, bleeding from the portal wounds, trochanteric bursitis and intra-articular breakage of the instrument. We believe that it is possible to undertake this operation safely using the technique described.

Shoulder pain: the role of diagnostic injections.

Many different shoulder disorders cause similar symptoms and pain patterns. An accurate diagnosis can generally be made by obtaining a detailed history, performing a comprehensive, directed physical examination and obtaining selected radiographs. Occasionally, shoulder injections can be of great assistance in establishing a clear diagnosis and providing relief of symptoms. Subacromial space injection, acromioclavicular joint injection, intra-articular injection and injection of the biceps tendon are helpful in identifying such disorders as subacromial bursitis, acromioclavicular arthritis, inj…

Brachial plexus neuropathy.

Brachial plexus neuropathy is a common syndrome that, although a distinct clinical entity, is often unrecognized. The illness is characterized by the acute onset of shoulder girdle pain without constitutional symptoms. The pain persists for 1 to 2 weeks, then subsides, with the development of weakness and atrophy in the affected muscles. Good recovery usually occurs but may take up to three years. There is no effective treatment. Since this condition may masquerade as bursitis, cervical disc disease, or other conditions commonly seen by general physicians, it is important to recognize this dis…

Diagnosis and treatment of popliteal cysts.

The authors reviewed 122 patients affected with popliteal cysts and treated between 1975 and 1985 with or without surgery; follow-up ranged from 1 to 7 years (average 3 years and 2 months). The patients were subdivided into 3 age groups, and observed for the clinical, anatomo-pathological and therapeutic differences among them. Cysts affecting children and adults were considered to be acute, or chronic bursitis, and those affecting elderly patients true cysts deriving from the joint cavity. The diagnostic effectiveness of arthrography and sonography is emphasized.

Shoulder impingement syndrome: radiographic evaluation.

The impingement syndrome is an important source of shoulder pain that occurs when the bony and soft-tissue structures of the superior aspect of the shoulder encroach upon the coracoacromial ligamentous arch during abduction of the arm. Subacromial bursitis, bicipital tendinitis, and rotator cuff disruptions are common sequelae of this abnormality. The presence of bony excrescences arising from the anteroinferior aspect of the acromion and of flattening and sclerosis of the greater tuberosity of the humerus is an important plain radiographic sign of the impingement syndrome. Fluoroscopy, should…

Morton's metatarsalgia.

In a series of 25 patients (predominantly women) with Morton's metatarsalgia, the most likely site for a neuroma is the third cleft of the left foot. If a swelling is present or if radiographic examination shows toe divergence then the diagnosis in unlikely to be a simple digital neuroma. A review of the literature suggests that there are other causes of pain in the forefoot. Usually the pain is diffuse by when it is localized, Morton's metatarsalgia is diagnosed. However, causes other than a digital neuroma, e.g. traumatic bursitis and rheumatoid arthritis, epithelial cyst, and foreign body s…

MR imaging of sports injuries of the hip.

While MR imaging studies of the hip have been directed routinely toward the diagnosis of atraumatic osteonecrosis (ON), interest has increased in the use of MR imaging to diagnose other disorders of the hip and surrounding soft tissues, especially in athletic injuries. In addition to reviewing MR imaging technical considerations and image appearance, this article discusses applications of MR imaging to diagnosing pelvic and hip fractures, muscle contusion and strain, tendon injuries, acetabular labral tears, bursitis, and osteitis pubis and chronic symphyseal injury.

[Contribution of ultrasonography in inflammatory rheumatic disorders (rheumatoid arthritis, polymyalgia rheumatica and spondyloarthropathies)].

Ultrasonography may be very useful for the positive and differential diagnosis of inflammatory rheumatic disorders (rheumatoid arthritis [RA], polymyalgia rheumatica [PMR] and spondylarthropathies) and if necessary, for the treatment response evaluation (RA). Later in the course of the disease, ultrasonography may help in diagnosing some peripheral complications (tendinous ruptures, synovial cysts, superficial bursitis...) and is useful to guide corticosteroid injections (RA, spondylarthropathies). This article addresses the main ultrasonographic features of inflammatory rheumatic disorders an…

Hoof wall surgery in the horse: approaches to and underlying disorders.

Because of the hoof capsule, surgery of the equine foot is often perceived to be quite difficult. Knowledge of the specific disease entities that require surgical intervention as well as an in-depth understanding of the anatomy of the tissues beneath the hoof capsule is a definite prerequisite to successful surgical treatment. This article details the surgical approaches used to treat septic navicular bursitis, septic pedal osteitis, infection of the collateral cartilages, and keratomas. The principles used to treat these conditions can be applied to a variety of other conditions for which acc…

Osteomyelitis of the patella.

Osteomyelitis of the patella is an uncommon infection that is primarily a disease of childhood. The diagnosis frequently is delayed because of its rarity and variable presentation, which may be acute with systemic signs or insidious with mild local signs. The diagnosis should be considered in patients with persistent peripatellar pain and swelling, cellulitis, septic prepatellar bursitis, and septic arthritis of the knee that does not respond to standard treatment. Treatment is identical to the treatment of osteomyelitis at other locations. The outcome is favorable in the child regarding range…

[Mycobacterium szulgai infection in a patient with hemophilia and AIDS].

Mycobacterium szulgai is a scotochromogenic species recently recognized as a human pathogen. Twenty-nine cases of disease caused by M. szulgai in humans have been reported. Pulmonary disease indistinguishable from that caused by M. tuberculosis was the commonest type of infection caused by M. szulgai. Olecranon bursitis was reported in 3 cases and disseminated infection was noted in 3 cases occurring in immunocompromised patients without AIDS. The authors report the first case of pulmonary disease caused by M. szulgai in Brazil and the first case in patient with AIDS in the world literature.

Shoulder pain. Guidelines to diagnosis and management.

The cause of shoulder pain can usually be diagnosed after a pertinent history has been obtained and a relevant physical examination performed. To carry out such an evaluation, the examiner must understand the salient anatomic relationships of the shoulder. Causes of shoulder pain include supraspinatus tendinitis (the most common), bicipital tendinitis, impingement syndromes, supraspinatus rupture, subacromial bursitis, arthritis, frozen shoulder, and various conditions that refer pain to the shoulder. Treatment in most cases consists of rest and administration of a nonsteroidal antiinflammator…

Protothecosis algaemia in a patient presenting with septic arthritis: A rare case of Prototheca zopfii isolated from Malaysia.

Prototheca species have been reported to cause infections in human. Typically, clinical symptoms of protothecosis include cutaneous infection, olecranon bursitis, tenosynovitis and disseminated systemic disease. We report a case of septic arthritis in which Prototheca zopfii was isolated from blood. Joint aspirate was also sent for cultures but did not yield any growth. No other organisms were isolated from this patient during his admission. The blood isolate was identified to species level via Polymerase Chain Reaction (PCR) method. The patient improved with administration of intravenous itra…
