Hemangioma of the facial nerve may occur more frequently than previously recognized. This benign vascular tumor most often arises in the area of the geniculate ganglion, although the reason for this site of predilection is not known. Using silicon injection and cross-sectional vessel counts, we recently demonstrated the presence of a geniculate capillary plexus (GCP) in the cat. Two patients with systemic capillary leak syndrome (SCLS) were followed up clinically for 5 and 2 years, respectively. Muscle biopsy was performed 1 week after the end of an acute crisis in one patient, and after 3 years in the other. Reported are results for 23 Haitian patients with capillary Wuchereria bancrofti microfilaremias greater than 30 mf/20 cmm blood. In capillary blood, the number of microfilariae was 0.9 to 7.8 times greater than expected when compared with that in venous blood. When the values for venous blood and capillary blood from these high count patients were plotted, the simple linear regression y = a + bx (r2 = .36) and the exponential function y = aebx (r2 = .34) both represented the relationship. A 43-year-old man reported a 2-month history of numerous widespread vascular lesions that had developed suddenly on his scalp, trunk, and extremities. The lesions were 2 to 6 mm in diameter, firm, dark red papules that bled easily with minor trauma. Capillary microscopy was performed on 19 patients with eosinophilic fasciitis. These patients were compared with 13 individuals with progressive systemic sclerosis (scleroderma). A case of severe hypovolaemic shock related to idiopathic oedema was observed in a 37 year old woman. Large plasma volume expansion (nearly 12 1 over 9 hours) did not change the clinical status. Haemodynamic studies showed low cardiac index (1.1 1/min/m2), decreased left ventricular stroke work index (6.7 gm/m2), and high systemic arterial resistance (52 mmHg/1/min/m2). A 21 years old female patient with lipoatrophic diabetes, a distinct syndrome of insulin resistant diabetes mellitus, loss of subcutaneous fat, hepatosplenomegaly, hyperlipidemia, increased basal metabolic rate, subvalvular aortic stenosis and cystic bone lesions is described. She exhibited all clinical signs of diabetic microangiopathy. Quantitative estimation revealed severely thickened basement membranes of muscle capillaries. It is concluded that the extent of her microangiopathy is a consequence of her excessive metabolic abnormalities.
