[Clinical and genetic analysis for activated PI3K-δ syndrome by PIK3CD gene mutation].

OBJECTIVE: To analyze clinical and genetic features of activated PI3K-δ syndrome (APDS), a new form of immunodeficiency disease caused by PIK3CD gene mutation. METHOD: Data of two patients diagnosed as APDS at Second Department of Respiratory Medicine of Beijing Children's Hospital Affiliated to Capital Medical University in 2015 were retrospectively reviewed. Pathogenetic genes were screened by whole exome sequencing, and identified by first generation sequencing. The identified pathogenetic genes were further verified in patients' parents. Then the gene sequencing results were analyzed. RE

Hyperadrenocorticism Caused by a Pituitary Ganglioglioma in a Dog.

An 11-year-old castrated male Chihuahua dog was presented with complaints of polyuria, polydipsia, abdominal enlargement, and alopecia. Hyperadrenocorticism was diagnosed on the basis of clinical signs, blood tests, adrenocorticotropin-stimulation test results, and an elevated serum adrenocorticotropin concentration. Contrast-enhanced magnetic resonance imaging showed that the pituitary gland was enlarged, compatible with a pituitary macroadenoma. Pituitary-dependent hyperadrenocorticism was suspected, and transsphenoidal hypophysectomy was thus performed for complete resection of the tumor. A

A Multicentric T-Cell Lymphoma with a Plasmacytoid Morphology in a Dog.

An 8-year-old male (neutered) Labrador with a history of erythematous skin lesions and exercise intolerance for a prolonged period was suddenly found dead. Necropsy findings revealed an infiltrative, focally extensive mass which occupied 25% of the cardiac interventricular septum. Severe endocardiosis was also found on the bicuspid and tricuspid valves. The submandibular lymph nodes and kidneys were bilaterally enlarged, and the pre-hepatic lymph node and spleen were also enlarged. Multiple dermal pustules were present around the mouth and on the ear, and small ulcers were present on the tongu

[Early Gastric Cancer with Many Enlarged Lymph Nodes as Sarcoid Reaction - A Case Report].

The patient was a 57-year-old man. Upper gastrointestinal endoscopy revealed a 0- II c lesion at the anterior wall of the greater curvature in the lower third of the stomach, and the biopsy of this tumor gave the diagnosis of tubular adenocarcinoma. Abdominal contrast-enhanced computed tomography revealed many enlarged regional and non-regional lymph nodes. Because there was no definite evidence of lymph node metastasis, distal gastrectomy with D2 was performed. Histopathological examinations found the sarcoid reaction(SR)in lymph nodes, but could not demonstrate any cancer cells. SR is the ep

Thyroid involvement in Langerhans cell histiocytosis: a report of two cases and review of the literature.

Langerhans cell histiocytosis (LCH) is a disorder with unclear etiology and pathogenesis characterized by abnormal clonal proliferation and accumulation of antigen presenting Langerhans' cells at various tissues and organs. Almost all organs or systems may be involved, and the prognosis depends on the involved sites and the presence of organ dysfunction. Thyroid tissue is a rarely affected site in children, and without histopathological evaluation it may be difficult to distinguish from other thyroid disorders because of the similar physical examination, laboratory and imaging findings. Here w

[A case of giant perirenal malignant lymphoma: a case report].

We report a case of malignant lymphoma arising as a giant perirenal mass in a 62-year-old woman who presented with an enlarging abdominal mass. Computed tomography (CT) revealed a large homogeneous mass with a diameter of 20 cm arising in the retroperitoneum. It surrounded the left kidney without renal infiltration and accompanied with para-aortic lymph node enlargement. Compared with the renal cortex, the tumor showed a homogeneous isodense mass by plain CT and only slight enhancement by the contrast-enhanced CT. On the basis of these imaging studies, the tumor was diagnosed as lymphoma. Then

Light and electron microscopic features of surgically excised left atrial appendage in rheumatic heart disease patients with atrial fibrillation and sinus rhythm.

INTRODUCTION: There are few studies comparing the pathology of the remodeled substrate in patients of rheumatic heart disease with atrial fibrillation (AF) and normal sinus rhythm (NSR). METHODS: The study group comprised 30 patients with rheumatic heart disease undergoing mitral valve replacement. Excised left atrial appendages of these patients [17 with persistent AF and 13 NSR (control group)] were subjected to light and electron microscopic examination. RESULTS: The histopathological findings of the myocardium were characterized by cardiomyocyte hypertrophy (CH), nuclear enlargement (NE)

Dermatosurgical pearls: melolabial advancement flap for tension-free closure of a basal cell carcinoma defect under the eyelid.

A 73-year-old man presented to the dermatology department with a history of a tumor in the right infraorbital region, present for approximately 2-3 years, which had shown progressive enlargement in recent months. Dermatological examination revealed a nodular, tumor-like lesion, elevated above the surrounding skin, with visible telangiectasias and crusts, clinically suspected to be basal cell carcinoma. Family history of skin cancer was unremarkable. The patient denied frequent sun exposure during child- or adulthood. Routine blood tests showed mild anemia, dyslipidemia, and hyperuricemia. Surg

Peripheral T-cell lymphoma invasion of the liver: The underappreciated hypoechoic periportal cuffing on ultrasound-A case report and literature review.

Liver involvement in lymphoma often manifests as nonoccupying diffuse infiltration, posing challenges in distinguishing it from primary liver disorder. Herein, we present the case of a 21-year-old female who underwent two separate diagnoses within a nine-month interval before being ultimately diagnosed with peripheral T-cell lymphoma, not otherwise specified. Our review of this case identified an ultrasound imaging feature, the hypoechoic periportal cuffing. When combined with associated increased lymphocyte count and liver enlargement, it can serve as a noninvasive suggestion for malignant di

Simultaneous granular cell tumor and seminoma in the descended testis of a cryptorchid rabbit.

Testicular tumors are rarely reported in rabbits. In this case study, a 4-year-old Holland lop rabbit, previously diagnosed with unilateral cryptorchidism, was presented because of enlargement of the descended testis. The rabbit was clinically normal. Following unilateral orchiectomy and scrotal ablation, histopathological analysis revealed 2 distinct types of testicular tumor in the descended testis: a granular cell tumor and a seminoma. To the best of the author's knowledge, this is the first documented report of simultaneous testicular tumors in the testis of a rabbit with unilateral crypto

Pulmonary large cell neuroendocrine carcinoma with adrenal oligorecurrence successfully treated by adrenalectomy.

A 63-year-old man suspected of having lung cancer underwent right upper lobectomy and was diagnosed with large cell neuroendocrine carcinoma (LCNEC). Eleven months after surgery, he developed an oligorecurrence in the adrenal gland and underwent left adrenalectomy. The specimen revealed LCNEC metastasis. Forty-one months after surgery, enlargement of a lesion near the surgical site was seen. Biopsy showed LCNEC metastasis and he is currently undergoing radiotherapy for the recurrent lesion. We report a case of LCNEC with adrenal gland oligorecurrence treated by adrenalectomy, which led to long

[Macroglossia].

A 63-year-old woman was referred to a department of oral and maxillofacial surgery because of slowly increasing enlargement of the tongue. Histopathologic examination of a biopsy showed the presence of amyloid. Further physical and laboratory studies disclosed the presence of multiple myeloma (Kahler's disease). A few years before, the patient had been operated for metacarpol syndrome of both hands, most likely because of amyloid deposition. The patient underwent a stam cell transplantation. After one year follow-up the patient is still alive. There was no distinct regression of the macrogloss

Histiocytic lymphophagocytic panniculitis (Rosai-Dorfman disease): a case report.

A 68-year-old woman presented with a symmetrical enlargement of the face and submandibular area associated with essential thrombocytosis. Biopsy of the lesions revealed a septal and lobular panniculitis disclosing a lymphohistiocytic and plasma cell proliferation with lymphophagocytosis by histiocytes, compatible with Rosai-Dorfman disease. The lesions regressed spontaneously without therapy over a period of 4 months. Diagnosis was confirmed by electron-microscopic examination and immunocytochemical presence of S-100 antigen within the histiocytic cells. A 2-year follow-up did not demonstrate

Albumin metabolism in chronic fasciola hepatica infections of cattle.

The metabolism of I131-albumin was examined in chronically fluke- infected animals in which anaemia and hypoalbuminaemia were prominent features. Animals with high fluke-burdens exhibited increased turnover rates of albumin, presumably due to a loss of blood into the bile ducts. Despite pronounced hepatic fibrosis the animals had a normal or only slightly decreased synthetic rate of albumin. This was in keeping with results from liver-function tests and is presumably explained by enlargement of the liver, resulting in maintenance of a near-normal parenchymal cell mass.

ANG II modulation of cardiac growth and remodeling in immature fetal sheep.

ANG II increases fetal blood pressure and stimulates fetal heart growth; however, little is known regarding its direct effects on cardiomyocytes in vivo. We sought to determine whether ANG II stimulates heart growth and cardiomyocyte hypertrophy and/or hyperplasia in utero in the immature fetal heart independent of the effects on cardiac afterload. In twin gestation, fetal sheep at ∼100 days gestation (term 145 days), one fetus received a chronic (6 days) infusion of ANG II alone (50 μg·kg(-1)·min(-1)) or ANG II plus nitroprusside (NTP) to attenuate the increase in blood pressure; noninstrumen

Testicular mixed germ cell tumor: A case report.

BACKGROUND: Testicular mixed germ cell tumors (TMGCTs) are rare malignant tumors that are more common in men aged 20-40 years. TMGCTs comprise two or more types of germ cell tumors that primarily affect the testis. Their onset is undetectable; thus, early diagnosis is challenging. However, early recognition and diagnosis substantially improve patient prognosis. CASE SUMMARY: We evaluated a rare case of TMGCT in a male patient presenting with recurrent fever and left supraclavicular lymphadenectasis instead of testicular enlargement and pain, which may easily lead to misdiagnosis. We report th

A case of Lambert-Eaton myasthenic syndrome associated with atypical bronchopulmonary carcinoid tumor.

The Lambert-Eaton myasthenic syndrome (LEMS) is typically recognized as a paraneoplastic syndrome associated with a small cell lung carcinoma (SCLC), whereas LEMS with other neuroendocrine lung tumors, including carcinoids or large cell lung carcinoma, are highly unusual. Here, we report a rare case of LEMS with atypical bronchopulmonary carcinoid tumor: a 65-yr-old man presented with progressive leg weakness and a diagnosis of LEMS was made by serial repetitive nerve stimulation test. Chest CT revealed a lung nodule with enlargement of paratracheal lymph nodes, and surgically resected lesion

Pancreatic T-cell lymphoma with high level of soluble interleukin-2 receptor.

An 82-year-old man was admitted to hospital with symptoms of abdominal fullness and loss of appetite. Abdominal computed tomography (CT) scan and ultrasonography showed enlargement of the whole pancreas with para-aortic lymphadenopathy. Endoscopic retrograde pancreatography (ERP) showed diffuse narrowing of the main pancreatic duct (MPD), and brushing cytology from the MPD was non-neoplastic. Differential diagnosis between lymphoma and other exocrine and endocrine pancreatic malignancies was needed, and the level of serum soluble interleukin-2 receptor (17 751 U/ml) was revealed to be signific
