[Imaging diagnosis of brucella spondylitis and tuberculous spondylitis].

Objective: To compare the characteristics of X-ray, CT and MRI of Brucella spondylitis and tuberculous spondylitis and its significance for differential diagnosis. Methods: A total of 10 cases of Brucella spondylitis and 20 cases of tuberculous spondylitis confirmed from the clinical, laboratory or pathological department were enrolled between January 2014 and August 2017 in the Fifth Affiliated Hospital of Sun Yat-sen University and the Third Affiliated Hospital of Southern Medical University. The CT, MRI findings were retrospectively analyzed to improve the differential diagnosis of these tw

Klippel-Feil anomaly with associated rudimentary cervical ribs in a human skeleton: case report and review of the literature.

Anomalies of the cervical spine are intriguing anatomically and often come to clinical attention. Fusion of one or more cervical vertebral segments, the Klippel- Feil anomaly (KFA), often causes increased motion at the vertebral segments superior to and inferior to the fused level with a resultant premature wear of these joints. We report an adult male skeleton with fusion of his C6 and C7 vertebral bodies (Type II KFA). A remnant of the intervertebral disc space was noted and bilateral rudimentary cervical ribs were observed emanating from the C7 vertebrae. Excessive joint degeneration was no

Use of halo fixation therapy for traumatic cranio-cervical instability in children: a systematic review.

PURPOSE: Traumatic cranio-cervical instability in childhood is rare and constitutes a challenge for the treating surgeon. The aim of therapy is to restore cervical stability without limiting the range of motion. The goal of this systematic review was to find out whether, over the last 10 years, halo fixation (HF) could still be considered a successful treatment option without major risks or complications. METHODS: We analyzed studies describing the use of HF in traumatic injuries of the cranio-cervical junction in children under the age of 17. Searches were performed in PubMed, MEDLINE and Em

Occipitocervical arthrodesis in children. A new technique and analysis of results.

A new wiring technique for occipitocervical arthrodesis was used in sixteen consecutive children between 1985 and 1992. The twelve boys and four girls had an average age of nine years and six months (range, two years and five months to nineteen years and three months) at the operation. The arthrodesis was performed between the occiput and the second cervical vertebra in ten patients and between the occiput and the third cervical vertebra in six. The instability was related to congenital anomalies (six patients), decompression for cervical stenosis (four patients), Down syndrome (three patients

The surgical treatment of instability of the upper part of the cervical spine in children and adolescents.

In a retrospective review of the cases of thirteen skeletally immature children and adolescents (four to eighteen years old) with instability of the upper part of the cervical spine (occiput to fifth cervical vertebra), we determined the efficacy of posterior arthrodesis and halo-cast immobilization in the management of this condition. The patients were divided into two groups: those with congenital vertebral anomalies alone (fusion or structural defects, or both) and those with cervical anomalies and systemic disorders (dwarfism, juvenile rheumatoid arthritis, Down syndrome, and cerebral pals

[Cervical spondylolysis--a case report and review of the literature].

Spondylolysis of the sixth cervical vertebra was reported in a thirty-seven-year-old man. He complained of sleeplessness and had no history of trauma. Plain cervical spine films and CT scan showed separation of the bilateral pars interarticularis and hypoplasia of the superior and inferior articular processes of C6. And they showed compensated hyperplasia and upward deviation of the right superior articular process of C7, but there was no instability of cervical vertebra. Spina bifida occulta at C6 and deviation of the spinous process of C7 to the left side were observed clearly on CT scan. We

Nasotracheal intubation awake in a patient with multiple fractures of the maxilla and cervical spine: A case report.

RATIONALE: Nasotracheal intubation is an advanced airway modality used in specific contexts, such as limited mouth opening, macroglossia, cervical spine instability. In addition, it can be performed with the patient awake, especially when predictors a difficult airway are unknown. PATIENT CONCERNS: A 41-year-old male with a lesion in the C1 cervical vertebra, associated with a fracture of the right maxilla, was intubated through the nasopharyngeal route while awake. The forms of induction were discussed. DIAGNOSES: Based on the mechanism of trauma and on the report of pain, associated with i

Dynamics of spinal cord and vertebral column development during the postnatal ontogenesis of the cat.

Data on the structure and postnatal ontogenesis of the vertebral column and spinal cord are of great importance for neurobiology. The present study is devoted to a morphometric analysis of the spinal cord and vertebral column of the cat during the period from birth to adulthood. A complex pattern of postnatal development of both structures was revealed. It was established that the growth rate of vertebral length depends on the rostrocaudal position of the vertebrae: it is similar in VC3-VT10 and increases in the caudal direction, reaching a maximum in VL5. The growth rate of segment length dem

Effect of posterior instrumented fusion on three-dimensional volumetric growth of cervical ossification of the posterior longitudinal ligament: a multiple regression analysis.

BACKGROUND CONTEXT: Despite the fact that ossification of posterior longitudinal ligament (OPLL) is a three-dimensional disease, conventional studies have focused mainly on a two-dimensional measurement, and it is difficult to accurately determine the volume of OPLL growth and analyze the factors affecting OPLL growth after posterior decompression (laminoplasty or laminectomy and fusion). PURPOSE: The present study aimed to investigate the factors affecting OPLL volume growth using a three-dimensional measurement. STUDY DESIGN/SETTING: This was a retrospective case study. PATIENT SAMPLE: Ei

Cervical tuberculosis in early childhood.

CASE HISTORY: We present a case of spinal tuberculosis in a 4-year-old girl that grounds severe vertebral destruction involving four cervical vertebrae and a large abscess with retropharyngeal expansion. Presenting symptoms were neck and right arm pain, torticollis and weakness of the right arm. We drained the abscess and removed the infected bone and reconstructed the cervical spine with a fibular allograft. The neurological deficits were recovered and the torticollis was straightened up. CONCLUSION: We emphasize the value of early diagnosis of spinal tuberculosis and early radical surgery w…

Radiology of fracture-dislocation of the cervical spine during delivery.

Four infants with fracture-dislocation of the cervical spine after traumatic delivery are described. In one patient with tetraparesis and complete dislocation of the cervical spine, transection of the cord at C6 was shown at autopsy. Two patients had fractures through the superior cartilaginous plates of the vertebral body with anterior subluxation of the spine. A fourth patient had a vertical fracture through the body of C5 with tracheal compression secondary to anterior dislocation of the spine. With conservative therapy, these three patients had good clinical recovery with healing and remod

Klippel-Feil syndrome: a case report and current understanding of molecular genetic background.

A case of Klippel-Feil syndrome in a 12-year-old boy presentingwith the features of low set posterior hairline, short webbed neck, scoliosis and Sprengel's deformity associated with upper eyelid coloboma and pre-auricular appendages is described. Radiologically there was evidence of maldeveloped cervical and upper thoracic vertebrae associated with elevated scapula. The association of the eyelid defect and pre-auricular appendages has not been documented in the past. The current literatures based on the recent advances in understanding of molecular genetic control over embryonic development of

Klippel-Feil syndrome with other associated anomalies in a medieval Portuguese skeleton (13th-15th century).

Klippel-Feil syndrome, or synostosis of the cervical spine, is the result of an abnormal division of somites during embryonic development. This report analyses an adult male (exhumed from a Portuguese graveyard dating from the 13th to the 15th century) with malformations in the cranium and vertebral column. Besides the lesions that are typical of Klippel-Feil syndrome type II, other defects usually linked to this pathology are described (occipito-atlantal fusion, hemivertebrae, butterfly vertebrae, cervical rib, changes in normal number of vertebral segments and a possible Sprengel deformity).

Anterior transvertebral herniotomy for cervical disk herniation.

This study analyzed the results of anterior transvertebral herniotomy for cervical disk herniation to assess the utility of this procedure. Anterior transvertebral herniotomy was performed in 24 patients who had cervical disk herniation without spinal canal stenosis. In most patients, a good result was obtained, but simultaneous or subsequent anterior intervertebral fusion was necessary in four patients. In one (4%) patient, the two adjacent vertebrae had fused spontaneously. The best indication for this treatment judging from the postoperative results is a large disk hernia associated with ei

Transverse posterior element fractures associated with torsion.

Six examples of a previously undescribed class of transverse vertebral element fractures are presented. These fractures differ from Chance and Smith fractures and their variants in the following respects: (1) the etiology is torsion and not flexion; (2) there is neither distraction of posterior ring fragments nor posterior ligament tears; (3) in contrast to Chance and Smith fractures, extension of the fracture into the vertebral body is absent or minimal; (4) the transverse process of the lumbar vertebra is avulsed at its base with a vertical fracture, not split horizontally. These fractures o
