A half-century of experience in the management of colon injuries: changing concepts.

A total of 773 patients who sustained colon injuries during the period 1927-1974 at Charity Hospital of Louisiana in New Orleans was reviewed. For purposes of comparison the patients were grouped into three 15-year periods. The first period (1927-1942) represented patients treated in the pre-antibiotic era exclusively with exteriorization. The second period (1943-1958) included patients in whom a program of selective surgical treatment of colon injuries was instituted, and included the advent of antibiotics and improved resuscitation. The final period (1959-1974) represented the increased use

Christian and Indian religions on palliative sedation: a scoping study.

BACKGROUND: Though especially in the last decade, much work has been done on various aspects of palliative sedation, little attention has been given to the role played by religious beliefs and traditions. Since the attitudes and decisions of patients, family members, and caregivers can be deeply influenced by religion when faced with severe suffering at the end of life, it is important to know the views of these traditions regarding palliative sedation. This study discusses and compares normative views in Christian denominations (Anglicanism, Catholicism, Eastern Orthodoxy, Protestantism) and

Plasma glial fibrillary acidic protein (GFAP) is a biomarker for central nervous system involvement in infantile-onset Pompe disease.

BACKGROUND: Pompe disease (PD), caused by acid α-glucosidase (GAA) deficiency, leads to glycogen accumulation in various tissues including the central nervous system. While enzyme replacement therapy (ERT) is lifesaving, it does not cross the blood-brain barrier. CNS manifestations including sensorineural hearing loss, dysarthria, and cognitive delay-persist despite ERT. Hence, CNS-specific biomarkers are needed to identify at-risk patients and guide monitoring and treatment. This clinical, retrospective study evaluated the utility of plasma glial fibrillary acidic protein (GFAP) as a potentia

Initial Impact of Loss of Copay Assistance From a National Nonprofit Fund on Outcomes of Retinal Disease.

PURPOSE: Many patients with neovascular age-related macular degeneration (nAMD), diabetic macular edema (DME), or retinal vein occlusion who have lost copay assistance from the DBA Good Days national charity fund have been forced to switch treatment from branded intravitreal antivascular endothelial growth factor (anti-VEGF) therapy to bevacizumab. This study sought to characterize the short-term impact of these changes on clinical outcomes. METHODS: This retrospective study included 89 eyes of 69 patients with nAMD, DME, or retinal vein occlusion who transitioned from receiving branded anti-

When Unilateral Leg Swelling Is Not Deep Vein Thrombosis: A Case of Disseminated Kaposi Sarcoma Revealing Advanced Human Immunodeficiency Virus.

Kaposi sarcoma (KS) is an acquired immunodeficiency syndrome (AIDS)-defining malignancy that can involve the skin, lymph nodes, and visceral organs in patients with advanced human immunodeficiency virus (HIV). We report the case of a 35-year-old man with no known prior medical history who presented to the charity care clinic complaining of recurrent thrush and two weeks of progressive right lower extremity swelling. His physical examination revealed painful, pruritic violaceous papules and plaques over the proximal right thigh that had been present for one week. He was referred to the emergenc

Near-source wastewater surveillance of SARS-CoV-2, norovirus, influenza virus and RSV across five different sites in the UK.

By tracking infectious diseases through sewage, municipal-scale wastewater surveillance has provided early warnings of future COVID-19 hospitalisations, identified biases in diagnostic testing, and is rapidly expanding to a broader array of pathogens. Despite applications in the targeted delivery of local interventions, near-source wastewater surveillance has received less attention and we know little about the near-source time series dynamics of contrasting pathogens. To address this, we conducted wastewater surveillance at five sites for SARS-CoV-2 and two sites for norovirus GI, norovirus G

Superficial Inferior Epigastric Artery Abdominoplasty Flap Breast Reconstruction Revisited.

INTRODUCTION: In 1863, Dr John Wood published the use of the pedicled superficial inferior epigastric artery (SIEA) flap for reconstruction of a forearm burn scar contracture. The first successful cutaneous free flap reported by Taylor and Daniel in 1973 was an SIEA free flap. Many thought mistakenly this first "groin flap" was based on the superficial circumflex iliac artery (SCIA). Although Taylor and Daniel planned on using the SCIA, it was quite small and the SIEA was much larger and more suitable for anastomosis to the recipient artery. The first SIEA flap for breast reconstruction was at

Racial disparities in patients with proliferative diabetic retinopathy treated with pars plana vitrectomy in an underserved population.

BACKGROUND: Proliferative diabetic retinopathy (PDR) is a serious microvascular complication of diabetes that can lead to irreversible vision loss. Prior research studies have documented how racial minorities with PDR experience less routine screening, less frequent treatment, and more complications. Although pars plana vitrectomy (PPV) is a widely used treatment modality for PDR, minimal research is currently available addressing how race impacts surgical presentation and outcomes. METHODS: This study is a retrospective case series involving a sample of patients who underwent PPV for PDR at

Reducing maternal, neonatal, and child mortality and improving quality of health care through a national task-shifting program for public hospitals in Liberia.

BACKGROUND: Contributing to the high hospital-based maternal, neonatal, and child mortalities in low resource countries and conflict zones is a shortage of health workers, especially physicians. Training programs, conducted over 12 years, have enhanced the skills of midwives, and nurses, to provide high quality, hospital-based, care to pregnant women, newborn infants, children, and adolescents. METHODS: A task-shifting partnership between the Ministry of Health, World Health Organisation, United Nations Population Fund, United Nations Children's Emergency Fund and the charity Maternal and Chi

Protocol for an intergenerational randomized controlled trial to enhance physical activity in older adults at risk for Alzheimer's disease.

BACKGROUND: Physical inactivity is one of the most important modifiable risk factors for Alzheimer's disease in North America. Despite this, most older adults are physically inactive. It is currently unknown how to successfully motivate physical activity behavior in older adults at risk for Alzheimer's disease, and this knowledge is crucial for early and effective disease prevention. Prior research has shown that intergenerational social engagement and prosocial behaviours can enhance the health and well-being of older adults. OBJECTIVES: This manuscript describes the design of a randomized c

Involving young people in research investigating comorbidity associated with childhood-onset rheumatic disease: perspectives of a series of focus groups.

BACKGROUND: Childhood-onset rheumatic diseases, such as juvenile idiopathic arthritis, juvenile-onset lupus and juvenile dermatomyositis, appear to be associated with an increased risk of comorbidities in adulthood compared to the general population. For the first stage of a research project evaluating this topic, we wanted to capture views from young people with juvenile-onset rheumatic disease to ensure that further work was relevant to their lived experience and priorities. This study aimed to determine (i) which comorbidities young people identify as important, (ii) how they access informa

Clinical decision rules for diagnosis of Streptococcus pyogenes sore throat in Fiji: a prospective diagnostic accuracy study.

BACKGROUND: Acute rheumatic fever is an immune-mediated condition triggered by Streptococcus pyogenes sore throat and possibly skin infection, with a substantial burden in resource-limited settings. Clinical decision rules (CDRs) are commonly used to guide antibiotic treatment of sore throat based on signs and symptoms, but their diagnostic accuracy varies by study and setting. This work aimed to assess the accuracy of multiple CDRs in Fiji to diagnose S. pyogenes sore throat. METHODS: We conducted a prospective diagnostic accuracy study at two primary healthcare centres in Suva, Fiji, enroll

Patient-reported characteristics of pernicious anaemia: a first step to initiate James Lind Alliance Priority Setting Partnership driven research.

BACKGROUND: Pernicious anaemia (PA) is characterised by vitamin B12 deficiency due to autoimmune-mediated loss of gastric parietal cells and intrinsic factor - a specific transporter for B12's intestinal uptake. The Pernicious Anaemia Society (PAS) is a patient-driven charity that recently identified 10 research priorities for improved diagnosis and management of PA through a James Lind Alliance Priority Setting Partnership. To facilitate research addressing these priorities, the aim of this study was to survey PAS members to identify and characterise a cohort of patients to form a PA research

The association of smoking with different eating and dietary behaviours: A cross-sectional analysis of 80 296 United Kingdom adults.

BACKGROUND AND AIMS: Smokers typically have a lower body mass index (BMI) than non-smokers, while smoking cessation is associated with weight gain. In pre-clinical research, nicotine in tobacco smoking suppresses appetite and influences subsequent eating behaviour; however, this relationship is unclear in humans. This study measured the associations of smoking with different eating and dietary behaviours. DESIGN: A cross-sectional analysis of data from health assessments conducted between 2004 and 2022. SETTING: An independent healthcare-based charity within the United Kingdom. PARTICIPANTS

Towards stratified treatment of JIA: machine learning identifies subtypes in response to methotrexate from four UK cohorts.

BACKGROUND: Methotrexate (MTX) is the gold-standard first-line disease-modifying anti-rheumatic drug for juvenile idiopathic arthritis (JIA), despite only being either effective or tolerated in half of children and young people (CYP). To facilitate stratified treatment of early JIA, novel methods in machine learning were used to i) identify clusters with distinct disease patterns following MTX initiation; ii) predict cluster membership; and iii) compare clusters to existing treatment response measures. METHODS: Discovery and verification cohorts included CYP who first initiated MTX before Jan

Subtypes of avoidant/restrictive food intake disorder in children and adolescents: a latent class analysis.

BACKGROUND: The Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) describes three primary avoidant/restrictive food intake disorder (ARFID) subtypes including sensory sensitivity, lack of interest in food or eating, and fear of aversive consequences. Studies exploring these subtypes have yielded varying results. We used latent class analysis (LCA) based on the psychopathology of ARFID in a sample of children and adolescents to empirically identify classes. METHODS: We carried out a surveillance study of ARFID in collaboration with the British Paediatric Surveillance
