[Hospitalizations for varicella in the Hospital Infantil La Fe, Valencia,Spain, 2001-2004].

INTRODUCTION: Varicella is a common, highly contagious disease. It is usually benign but has potentially serious complications. PATIENTS AND METHODS: To assess the clinical characteristics and the associated cost of varicella hospitalization, we reviewed the medical records of children hospitalized for varicella between 2001 and 2004. Children with coincidental varicella hospitalized for a different reason were excluded. RESULTS: Of 1177 children with varicella attended at the emergency room, 101 (8.6 %) were hospitalized. The median age was 3.2 years (21 days to 18.9 years). Twenty-eight ch

Analysis of Types of Skin Lesions and Diseases in Everyday Infectious Disease Practice-How Experienced Are We?

Rashes and skin lesions are a common reason for patient visits to emergency departments and physicians' offices. The differential diagnosis … lesions and 21.2% remained with undiagnosed skin lesions. The most common infectious diagnoses were chickenpox, herpes zoster and unspecified viral infections characterized by skin and mucous membrane lesions. The most common non-infectious diagnoses were urticaria and atopic dermatitis. Overall, the most common individual diagnosis (ICD-10 code) was "nonspe …

Herpes Zoster: Risk Factors for Occurrence, Complications, and Recurrence with a Focus on Immunocompromised Patients.

BACKGROUND: The varicella-zoster virus is a highly contagious human herpesvirus that primarily causes varicella (chickenpox) as an initial infection, targeting T cells, neurons, as well as skin cells, and can later reactivate to cause herpes zoster (shingles). Following reactivation, the varicella-zoster virus infection can lead to severe complications, the most common of which is postherpetic neuralgia. Risk factors include aging, immunosuppression, chronic diseases such as diabetes, cardiovascular disorders, respiratory conditions, and others. OBJECTIVE: In this article, we present an analy…

Bilateral Facial Palsy: An Atypical Neurological Complication of Varicella-Zoster Virus Reactivation.

Bilateral facial palsy is an exceptionally rare condition, often indicative of systemic involvement or severe underlying pathology. This report details a case of bilateral facial nerve paralysis in a young adult following recent varicella infection, an association infrequently documented in clinical practice. The patient presented with classic signs of bilateral facial nerve dysfunction alongside a history of recent chickenpox infection. Diagnostic workup confirmed bilateral axonal neuropathy, suggesting a peripheral nerve etiology linked to varicella-zoster virus reactivation. This case highl…

Vaccine hesitancy as a threat to global health? Rokitansky's De Varioloide Vaccinica-core issues in today's vaccination debates.

This contribution explores Carl von Rokitansky's 1828 dissertation De Varioloide Vaccinica in light of contemporary vaccination debates. At a time when smallpox was still a major global threat and vaccine hesitancy was widespread, Rokitansky systematically analyzed post-vaccination disease patterns. He proposed a differential diagnosis distinguishing between variola vera (true smallpox), variola vaccinica (vaccinal cowpox), variolois vaccinica (mitigated smallpox in vaccinated individuals), and varicella vaccinica (often misclassified chickenpox). Rokitansky anticipated modern immunological co…

[Varicella-zoster virus (VZV) acute retinal necrosis and recombinant zoster vaccine].

Varicella zoster virus (VZV) is responsible for chickenpox. Like all herpes viruses, after primary infection it enters into latency and can be reactivated afterwards. Many forms of symptomatic reactivation of VZV exist including acute retinal necrosis (ARN), an ophthalmic emergency which can lead to blindness. ARN is treated starting with high-dose intravenous acyclovir then with oral valaciclovir for a total duration of up to 3 months. Symptomatic reactivations of VZV are public health issues. The new Swiss 2022 vaccination plan includes the recombinant vaccine Shingrix. It effectively preven…

A case of tri-segmental cranial nerve V herpes zoster.

Varicella-zoster is the causative virus underlying varicella or "chickenpox" and herpes zoster or "shingles." Cases of disseminated disease have been widely reported in immunocompromised patients. We describe an interesting case of tri-segmental cranial nerve V herpes zoster here with discussion of the salient clinical features as well as brief discussion about ongoing trials for herpes zoster ophthalmicus prophylaxis. This case also highlights the importance of timely treatment and diagnosis, as the patient presented 6 days prior to hospitalization with a mild vesicular facial rash but was lo…

Herpes zoster-associated idiopathic thrombocytopenic purpura in a liver transplant recipient: a case report and overview.

Idiopathic (autoimmune) thrombocytopenic purpura has been previously reported as a rare complication in children and in a few adults following chickenpox. We report a case of varicella zoster virus-associated idiopathic thrombocytopenic purpura in an adult liver transplant recipient following dermatomal zoster. Idiopathic thrombocytopenic purpura developed 3 days after the onset of herpes zoster in our patient, with a nadir platelet count of 3000/mm3. The patient was treated with intravenous gamma globulin with recovery of thrombocytopenia after 3 weeks. Transplant clinicians need to be aware …

Herpes Zoster Vaccination: Insights into Efficacy, Safety, and Guidelines.

BACKGROUND: The varicella-zoster virus (VZV) is a human herpesvirus that primarily causes varicella (chickenpox) as an initial infection, characterized by distinctive skin lesions. It can later reactivate, leading to herpes zoster (shingles). Once reactivated, VZV infection may result in serious complications, the most common being postherpetic neuralgia. Fortunately, vaccination can prevent this condition. OBJECTIVES: In this study, we provide a comprehensive analysis of zoster vaccines, including clinical trials, safety profiles, and reimbursement guidelines across various countries. RESUL…

[Viral vaccines in the national vaccination program--views of the near future].

Both conventional virus vaccines and those being introduced in the near future are either live attenuated viruses or nonliving inactivated viruses, mere virus-like particles (VLP) or purified proteins. Live vaccines yield a "more natural" immunity, but in many cases also a nonliving vaccine is sufficient to protect from viral infections. Influenza vaccines are of both types. The commencing HPV vaccination program will be conducted with a VLP vaccine comprising two serotypes. A live chickenpox vaccine will be introduced in the national vaccination program in the next few years. In addition, a s…

Herpes zoster overview: natural history and incidence.

The varicella-zoster virus (VZV) causes two diseases. The primary VZV infection, known as chickenpox, typically occurs during childhood. Herpes zoster infection results later in life from reactivation of VZV in the dorsal root ganglia. Herpes zoster characteristically results in a rash with a unilateral dermatomal distribution, which usually resolves within 2 to 4 weeks. If the infection does not resolve after its acute phase, long-term complications, such as postherpetic neuralgia, may develop. The author discusses the natural history and incidence of primary VZV infection and herpes zoster a…

Insights into the pathogenesis of varicella viruses.

PURPOSE OF REVIEW: Varicella zoster virus (VZV) is a highly contagious, neurotropic alpha herpes virus that causes varicella (chickenpox). VZV establishes lifelong latency in the sensory ganglia from which it can reactivate to induce herpes zoster (HZ), a painful disease that primarily affects older individuals and those who are immune-suppressed. Given that VZV infection is highly specific to humans, developing a reliable in vivo model that recapitulates the hallmarks of VZV infection has been challenging. Simian Varicella Virus (SVV) infection in nonhuman primates reproduces the cardinal fea…

[TaqMan fluorescent probe-based real-time PCR assay for detection of varicella-zoster virus].

INTRODUCTION: Varicella-zoster virus (HHV-3) is spread by the respiratory route and disseminates to lymph nodes and then via lymph back to the skin, resulting with the rash of chickenpox. Like other alpha-herpesviruses, HHV-3 infects the neurons of the dorsal root ganglia, where it causes lifelong latency. Virus reactivation causes episode of herpes zoster (shingles). During severe HHV-3 infections molecular methods, such as real-time PCR (qPCR) assay, are recognized as a method-of-choice for detecting viral DNA in clinical specimens. The aim of this study was to develop the qPCR assay for det…

Varicella zoster virus infections in children after the introduction of live attenuated varicella vaccine.

PURPOSE OF REVIEW: Varicella zoster virus is the cause of both varicella (chickenpox) and herpes zoster (shingles). A live attenuated varicella vaccine was developed in 1974 and was approved in 1995 by the United States Food and Drug Administration for administration to both healthy children (>12 months of age) and adults who are susceptible to varicella. Many studies have shown varicella vaccine to be highly effective. Widespread use of the varicella vaccine in the United States has led to important changes in the epidemiology of the infection. The purpose of this review is to summarize the m…
