The winding road to developing a malaria vaccine. Study hypothesis.

In Africa, a child dies every 30 seconds from malaria, a vector-borne parasitic disease caused by Plasmodium spp, with higher mortality and severe forms of disease more frequently associated with Plasmodium falciparum infection. By looking at the natural resistance to malaria conferred by sickle cell trait, we hypothesize that a malaria therapeutical vaccine targeting the erythrocyte stage of the parasite through erythrocyte sickling could reduce parasite density and control the progression and severity of disease, thus decreasing the morbidity and mortality associated with severe forms of mal

Access to child-feeding counseling service and determinant factors among breastfeeding mothers in Ethiopia: a multilevel complex data analysis of 2019 Ethiopian mini demographic and health survey.

BACKGROUND: Inappropriate child-feeding practices lead to child morbidity and mortality in many countries around the world. In many low- and middle-income countries, mothers often lack access to appropriate child-feeding counseling services within healthcare settings. While several studies have examined recommended infant and child feeding practices, there is insufficient information regarding the accessibility of child-feeding counseling services in Ethiopia. Therefore, this study was conducted to assess the accessibility of child-feeding counseling services and determinant factors among brea

Prospective Genomic Surveillance of Severe Febrile Illness in Tanzanian Children Identifies High Mortality and Resistance to First-Line Antibiotics in Bloodstream Infections.

Severe febrile illness (SFI), a major cause of child mortality in Sub-Saharan Africa (SSA), is frequently caused by bloodstream infections (BSI), increasingly caused by antimicrobial-resistant organisms. However, antimicrobial resistance (AMR) prevalence and patterns in this population are largely unknown. We conducted a prospective cohort study of children aged 28 days to 14 years with SFI at a referral hospital in Tanzania (July 2022-September 2023) to determine the prevalence, pathogen profile, and AMR patterns of BSIs. Blood cultures were performed on all participants. Isolates underwent A…

Maternal and child life years gained by transfusing low titer group O whole blood in trauma: A computer simulation.

BACKGROUND: Using low titer group O whole blood (LTOWB) is increasingly popular for resuscitating trauma patients. LTOWB is often RhD-positive, which might cause D-alloimmunization and hemolytic disease of the fetus and newborn (HDFN) if transfused to RhD-negative females of childbearing potential (FCP). This simulation determined the number of life years gained by the FCP and her future children if she was resuscitated with LTOWB compared with conventional component therapy (CCT). METHODS: The model simulated 500,000 injured FCPs of each age between 0 and 49 years with LTOWB mortality relati

Predicting mortality in sick African children: the FEAST Paediatric Emergency Triage (PET) Score.

BACKGROUND: Mortality in paediatric emergency care units in Africa often occurs within the first 24 h of admission and remains high. Alongside effective triage systems, a practical clinical bedside risk score to identify those at greatest risk could contribute to reducing mortality. METHODS: Data collected during the Fluid As Expansive Supportive Therapy (FEAST) trial, a multi-centre trial involving 3,170 severely ill African children, were analysed to identify clinical and laboratory prognostic factors for mortality. Multivariable Cox regression was used to build a model in this derivation d

Effectiveness of antibiotic prophylaxis for acute esophageal variceal bleeding in patients with band ligation: A large observational study.

BACKGROUND: Esophageal variceal bleeding is a severe complication associated with liver cirrhosis and typically necessitates endoscopic hemostasis. The current standard treatment is endoscopic variceal ligation (EVL), and Western guidelines recommend antibiotic prophylaxis following hemostasis. However, given the improvements in prognosis for variceal bleeding due to advancements in the management of bleeding and treatments of liver cirrhosis and the global concerns regarding the emergence of multidrug-resistant bacteria, there is a need to reassess the use of routine antibiotic prophylaxis af

Perinatal Mortality in the United States, 2020-2021.

Perinatal mortality(late fetal deaths at 28 completed weeks of gestation or more and early neonatal deaths younger than age 7 days) can be an indicator of the quality of health care before, during, and after delivery, and of the health status of the nation (1,2). The U.S. perinatal mortality rate declined 30% from 1990 through 2011, was stable from 2011 through 2016, and declined 4% from 2017 through 2019 (1,3-5). This report describes changes in perinatal mortality, as well as its components, late fetal and early neonatal mortality, from 2020 to 2021, during the COVID-19 pandemic. Also shown

Use of refined perinatal mortality rate trends to evaluate the effect of a confidential inquiry.

Standardised perinatal mortality rates and cause-specific rates were used to evaluate the effect of a confidential inquiry into perinatal deaths in West Glamorgan obstetric units. Cause of death was classified by means of 5 pathological subgroups, and the denominator data were obtained from the computerised standard child-health system. The standardised perinatal mortality rate fell from 103 in 1981 to 85 in 1983, and the perinatal mortality rate fell from 15.9 to 9.4 per 1000 births in that period; 3 of the cause-specific rates declined considerably. The trend indicates an improvement in ante

Comparison of safety and effectiveness of antiretroviral therapy regimens among pregnant women living with HIV at preconception or during pregnancy: a systematic review and network meta-analysis of randomized trials.

Источник пометил публикацию как отозванную

BACKGROUND: Mother-to-child transmission is the primary cause of HIV cases among children. Antiretroviral therapy (ART) plays a critical role in preventing mother-to-child transmission and reducing HIV progression, morbidity, and mortality among mothers. However, after more than two decades of ART during pregnancy, the comparative effectiveness and safety of ART medications during pregnancy are unclear, and existing evidence is contradictory. This study aimed to assess the effectiveness and safety of different ART regimens among pregnant women living with HIV at preconception or during pregnan
