A Longitudinal and Multimodal Simulation-Based Curriculum Improved Internal Medicine Residents' Confidence in Procedures, Point-of-Care Ultrasound, and Clinical Decision-Making.

Introduction Internal medicine training has historically relied on classroom-based lectures, typically delivered through morning and noon conferences, which offer broad content coverage but limited experiential learning. Simulation-based education improves resident confidence, procedural ability, and clinical reasoning; yet it is often delivered in brief, standalone activities that lack longitudinal structure or integration within the broader curriculum. Few programs have a longitudinal simulation curriculum that spans the academic year and incorporates multiple modalities and topics. We devel

ZhongdaChat-ED: a medical large language model for personalized erectile dysfunction health consultation and professional clinical decision-making using retrieval-augmented generation.

Artificial intelligence (AI)-driven large language models (LLMs) hold potential for medical applications but face challenges, such as inaccurate or outdated training data. In this study, ZhongdaChat-ED, a personalized medical LLM integrating retrieval-augmented generation (RAG) technology, was developed to enhance erectile dysfunction (ED) counseling and clinical decision-making. The model was built using the open-source Deepseek-r1:32b framework, augmented with two specialized databases: a patient health consultation database and a clinical decision support database updated with real-time med…

Clinical Decision-Making and Risk of Malignancy when Parotid Gland Fine Needle Aspiration Cytology Indicates a Non-Neoplastic or Non-Diagnostic Finding.

INTRODUCTION: Non-neoplastic and non-diagnostic cytological findings present a diagnostic and therapeutic challenge in head and neck oncology. Both groups still harbor a risk of malignancy (ROM). Of note, ROM values have been counted from surgically confirmed lesions only. The purpose of this study was to evaluate the clinical course of patients with non-neoplastic or non-diagnostic fine needle aspiration cytology (FNAC) findings from a parotid gland lesion. METHODS: This retrospective cohort study comprises all 184 consecutive patients who visited the Department of Otorhinolaryngology - Head

Potential factors of false-negative sentinel lymph node biopsy for breast cancer and clinical decision-making value of axillary lymph node dissection.

OBJECTIVE: To investigate the potential factors of false-negative sentinel lymph node biopsies (SLNB) for breast cancer and the clinical decision-making value of axillary lymph node dissection (ALND). METHODS: We retrospectively selected 312 patients with breast cancer who underwent SLNB in our hospital from May 2021 to July 2024. These subjects were divided into a false negative group and a true negative group according to the SLNB results. Influencing factors were analyzed by Logistic regression. A total of 300 breast cancer patients who underwent ALND at the same time were selected as the …

Clinical decision-making during childbirth in health facilities from the perspectives of labouring women, relatives, and health care providers: A scoping review.

PROBLEM: For health care providers to ensure appropriate decision-making in clinical settings during childbirth, facilitators and barriers must be identified. BACKGROUND: Women who experience a sense of control by participating in the decision-making process, are more likely to have a positive birth experience. However, decision-making may involve hierarchies of close observation and control. AIM: The aim of the scoping review was to map and summarise existing literature on the process of clinical decision-making during childbirth from the perspective of labouring women, relatives and health…

Major Bleeding During Thrombolytic Therapy for Acute Lower Limb Ischaemia: Value of Laboratory Tests for Clinical Decision Making, 17 Years of Experience.

OBJECTIVE: Regular measurement of fibrinogen as dose guidance in catheter directed thrombolysis (CDT) for acute limb ischaemia (ALI) has recently been dropped from European guidelines based on inconsistent literature. This study aimed to determine whether low fibrinogen levels and high activated partial thromboplastin time (APTT) are associated with an increased major bleeding risk during CDT. METHODS: All consecutive patients treated with CDT for ALI in two Dutch hospitals between January 2004 and April 2021 were analysed retrospectively. Patients were treated with two dosing regimens (low d

The Clinical Utility of DCISionRT® on Radiation Therapy Decision Making in Patients with Ductal Carcinoma In Situ Following Breast-Conserving Surgery.

BACKGROUND: The role of radiation therapy (RT) following breast-conserving surgery (BCS) in ductal carcinoma in situ (DCIS) remains controversial. Trials have not identified a low-risk cohort, based on clinicopathologic features, who do not benefit from RT. A biosignature (DCISionRT®) that evaluates recurrence risk has been developed and validated. We evaluated the impact of DCISionRT on clinicians' recommendations for adjuvant RT. METHODS: The PREDICT study is a prospective, multi-institutional, observational registry in which patients underwent DCISionRT testing. The primary endpoint was to

Clinical and laboratory profile in confirmed vs. suspected septic arthritis patients and its relevance in decision making: A comparative cross-sectional study.

PURPOSE: There were 10%-30% of patients with adult-onset septic arthritis (SA) exhibiting sterile synovial fluid (SF), and the uncertainty in the determining diagnosis of these patients posed a challenge in management. The purpose of this study was to investigate the differences between confirmed (Newman A) and suspected (Newman B & C) SA in adults. METHODS: This was a descriptive study with a cross-sectional study design conducted at a tertiary referral centre from July 2016 to February 2019. Patients aged over 18 years presented to the emergency department with clinical features suggestive

'We don't have recipes; we just have loads of ingredients': explanations of evidence and clinical decision making by speech and language therapists.

RATIONALE, AIMS AND OBJECTIVES: Research findings consistently suggest that speech and language therapists (SLTs) are failing to draw effectively on research-based evidence to guide clinical practice. This study aimed to examine what constitutes the reasoning provided by SLTs for treatment choices and whether science plays a part in those decisions. METHOD: This study, based in Ireland, reports on the qualitative phase of a mixed-methods study, which examined attitudes underpinning treatment choices and the therapy process. SLTs were recruited from community, hospital and disability work sett

Systematic review of magnetic resonance imaging characteristics that affect treatment decision making and predict clinical outcome in patients with cervical spondylotic myelopathy.

STUDY DESIGN: Systematic review. OBJECTIVE: To determine whether there are magnetic resonance imaging (MRI) characteristics in patients with cervical spondylotic myelopathy that affect treatment decisions or predict postsurgical outcomes or adverse events. SUMMARY OF BACKGROUND DATA: Although the role of MRI in confirming the clinical diagnosis of cervical spondylotic myelopathy and directing surgical management is well established, its potential value as a prognostic tool is largely unknown. METHODS: A systematic search was conducted using PubMed and the Cochrane Collaboration Library for

[Clinical reasoning and decision making in practice. A man with pneumonia and ill smelling watery sputum: the truth revealed after 80 years].

Источник пометил публикацию как отозванную

A 24-year-old man was admitted to hospital with pneumonia. On admission he was seen to have an asymmetrical build. During treatment of the lung infiltrate his clinical condition deteriorated. On the third day he coughed up great quantities of fluid which had a urine-like smell. The concentration of creatinine in this fluid was the same as in urine. On X-ray of the thorax, a massive accumulation of pleural fluid was seen. Shortly after aspiration of 1000 ml of pleural fluid the patient died. At autopsy, an ectopic kidney was found in the left thoracic cavity. The pneumonia had caused an abscess

A readily interpretable rule involving multiple forms of pairwise molecule comparisons with applications for clinical make-decision of breast cancer management.

Cells of breast cancer (BC) can metastasize to lymph nodes or other organs and is the high leading cause of female cancer deaths. Efforts to improve the performance of early detection and precise treatment are urgent and important for BC management. In clinical application, compared with highly complex classification functions, precise, simple and biologically interpretable algorithms can strengthen the understanding of disease development and facilitate the personalization of therapeutic strategies. In this study, a novel readily interpretable decision rule involving multiple forms of molecul

Utilizing nominal group technique to achieve consensus on a clinical decision-making tool for vocational rehabilitation at a grassroots level in South Africa.

BackgroundBarriers to the delivery of vocational rehabilitation have been reported as a lack of knowledge, skills and confidence in occupational therapists who deliver these services. A program aimed at overcoming these barriers was developed to provide training in the delivery of vocational rehabilitation services. The program includes three decision-making tools to facilitate not only knowledge translation but also practice changes. The tools are (1) Return-to-Work Decision Tree, (2) Performance-based Assessment Tool, (3) Job Match Tool.ObjectiveThe objective of the research was to obtain co
