PURPOSE: Urinary tract infection (UTI) is the second most common indication for antibiotic therapy among inpatients in the United States. Ceftriaxone, a third-generation cephalosporin, is habitually chosen to treat inpatient UTIs due to familiarity, cost, and perceived safety. However, third-generation cephalosporins increase the risk of health care facility-onset Clostridioides difficile infection (HOCDI) more than any other antibiotic group, while no statistical risk exists for first-generation cephalosporins. Recent evidence comparing Enterobacterales susceptibility for first- and third-gen…

In this retrospective cohort of adult hematology-oncology and transplant patients, discontinuation of universal gloving did not result in significant changes in rates of central line-associated bloodstream infection, Clostridioides difficile infection, or vancomycin-resistant Enterococcus colonization. Active surveillance and subsequent isolation may be a viable alternative strategy to universal precautions.

Clostridium [Clostridioides] difficile infection (CDI) is one of the leading causes of diarrhea associated with medical care worldwide, and up to 60% of patients with CDI can develop a recurrent infection (R-CDI). A multi-species microbiota biofilm model of C. difficile was designed to evaluate the differences in the production of biofilms, sporulation, susceptibility to drugs, expression of sporulating (sigH, spo0A), quorum sensing (agrD1, and luxS), and adhesion-associated (slpA and cwp84) pathway genes between selected C. difficile isolates from R-CDI and non-recurrent patients (NR-CDI). We…

UNLABELLED: We assessed the efficacy and safety of fidaxomicin, a narrow-spectrum macrocyclic antibiotic, for treating inpatients with Clostridioides (Clostridium) difficile infection (CDI) in Japan. The objective was to demonstrate the non-inferior efficacy of fidaxomicin versus vancomycin. This Phase III, vancomycin-controlled, double-blind, parallel-group study enrolled adults with CDI. Patients were randomly assigned to receive fidaxomicin (200 mg twice daily, orally) or vancomycin (125 mg four-times daily, orally) for 10 days. The primary endpoint was global cure rate of CDI (proportion o…

BACKGROUND: Antibiotic re-exposure after an episode of Clostridioides difficile infection (CDI) is common and is a strong risk factor for recurrent CDI (rCDI). Two randomized controlled trials (RCTs) of vancomycin prophylaxis during antibiotic re-exposure were recently published, which we sought to evaluate by meta-analysis.
METHODS: We updated a recent systematic review search of MEDLINE and Embase via Ovid to 9 March 2026. RCTs of vancomycin during antibiotic re-exposure as secondary CDI prophylaxis were included. Quality was assessed by the Cochrane RoB2 tool. A Bayesian meta-analysis of 5…

BACKGROUND: Current guidelines recommend multistep algorithms incorporating nucleic acid amplification tests (NAATs) and toxin enzyme immunoassays (EIAs) for the diagnosis of Clostridioides difficile infection (CDI); however, the clinical significance of toxin-negative/NAAT-positive (toxin-/NAAT+) results remains uncertain. We aimed to compare clinical outcomes between toxin + and toxin-/NAAT+ cases.
METHODS: This retrospective study included adults diagnosed with CDI between September 2020 and September 2025 at a tertiary hospital in South Korea. Patients were classified by toxin EIA and NAA…

Vibrio cholerae, the aetiological agent of cholera, is predominantly an imported pathogen in high-income countries. We report a case of a 49-year-old Nigerian male returning from Africa with abdominal pain and watery diarrhea, who rapidly developed acute kidney injury. The initial diagnosis was Clostridioides difficile enteritis, based on GDH antigen and toxin detection in stool samples. Microbiological cultures subsequently revealed concomitant growth of Vibrio cholerae. The patient required intensive care unit management, including aggressive fluid resuscitation and antimicrobial treatment w…

BACKGROUND: Fulminant Clostridioides difficile infection (CDI) in patients with hematologic malignancies during chemotherapy-induced aplasia carries high mortality and limited treatment options. Our objective was to evaluate fecal filtrate transplantation (FFT) as a salvage therapy for fulminant CDI during aplasia, and to explore whether donor-recipient phage dynamics may contribute to clinical response.
METHODS: We conducted a single-center, prospective, protocol-defined pilot case series study including consecutive adults with hematologic malignancies, chemotherapy-induced grade-4 aplasia a…

Cancer patients are particularly at risk of Clostridioides (Clostridium) difficile (C. difficile) infection as a considerable healthcare burden and common cause of nosocomial diarrhea. This study aimed to investigate the risk factors and microbiological follow-up of C. difficile among hospitalized hematology-oncology patients in the central part of Iran. Of 1100 patients who were diagnosed with cancer receiving radiotherapy and/or chemotherapy,70 patients with clinical signs of gastrointestinal complications were entered in the study. The Glutamate Dehydrogenase test kit was used for the first…

OBJECTIVE: Clostridioides difficile is a major cause of healthcare-associated diarrhea worldwide. For years, metronidazole and vancomycin were considered the standard treatment for C. difficile infection. However, they are increasingly being associated with treatment failure and recurrence. In this study, we investigated the in vitro activity of dalbavancin and fourteen other antimicrobials against 155 toxigenic C. difficile isolates originating from patients with C. difficile-associated diarrhea.
MATERIALS AND METHODS: Antimicrobial susceptibility was evaluated by the MIC Test Strip, and the…

Clostridioides difficile is a Gram-positive, spore-forming anaerobe that causes clinical diseases ranging from diarrhea and pseudomembranous colitis to toxic megacolon and death. C. difficile infection (CDI) is associated with antibiotic usage, which disrupts the indigenous gut microbiota and causes the loss of microbial-derived secondary bile acids that normally provide protection against C. difficile colonization. Previous work has shown that the secondary bile acid lithocholate (LCA) and its epimer isolithocholate (iLCA) have potent inhibitory activity against clinically relevant C. diffici…

Clostridium (Clostridioides) difficile ribotype 027 (RT027) was detected in Germany for the first time in 2007 during an outbreak in the region of Trier, Rhineland-Palatinate and is today the most prevalent ribotype (RT) in Europe. We aimed to determine the changes in RT distribution and corresponding antimicrobial resistance in clinical C. difficile isolates between two time points (2007 and 2017) in one tertiary care hospital in Germany. C. difficile isolates recovered in 2007 and in 2017 (80 isolates per year, respectively) from patients at a Tertiary Care University Hospital in North-Rhine…

In a recent One Health genomic survey of community-associated Clostridioides difficile in Japan, S. Yoshizawa, K. Komori, K. Aoki, T. Sawa, et al. (Appl Environ Microbiol 92:e00397-26, 2026, https://doi.org/10.1128/aem.00397-26) deposited the genome of strain TUM20227, a human tcdA-/tcdB+ isolate assigned to the novel sequence type ST919. Reanalysis of its publicly available genome shows that TUM20227 belongs to cryptic clade C-III and carries a chromosomally encoded, monotoxin PaLoc-like locus encoding a 2,367-amino acid TcdB-family protein. This TcdB-like protein shares less than 83% amino a…

Following an extended period of declining prevalence of the epidemic Clostridioides difficile ribotype 027 (RT027) in Portugal, a genetically distinct, multidrug-resistant (MDR) RT027 strain with reduced susceptibility to vancomycin has emerged, causing a 15-month outbreak. This investigation provides epidemiological and genomic evidence for renewed circulation and evolutionary adaptation of this high-risk lineage. A comprehensive outbreak investigation was conducted in a tertiary-care hospital in northern Portugal between 2023 and 2025. Epidemiological and clinical data, antimicrobial exposur…
