Yoga vs. physical therapy vs. education for chronic low back pain in predominantly minority populations: study protocol for a randomized controlled trial.

BackgroundChronic low back pain causes substantial morbidity and cost to society while disproportionately impacting low-income and minority adults. Several randomized controlled trials show yoga is an effective treatment. However, the comparative effectiveness of yoga and physical therapy, a common mainstream treatment for chronic low back pain, is unknown.Methods/designThis is a randomized controlled trial for 320 predominantly low-income minority adults with chronic low back pain, comparing yoga, physical therapy, and education. Inclusion criteria are adults 18-64 years old with non-specific

Representation of minoritized people with cystic fibrosis in the Cystic Fibrosis Therapeutics Development Network clinical trials.

RATIONALE: Black and Hispanic people with cystic fibrosis (PwCF) experience worse health outcomes related to pulmonary disease compared with White, non-Hispanic PwCF. Improving outcomes in minoritized PwCF is a crucial need in the CF community, but prior evidence suggests that these communities are underrepresented in CF clinical research. OBJECTIVE: To determine whether CF clinical studies are representative of the overall CF population. METHODS: The study cohort included participants from recent CF transmembrane conductance regulator modulator studies: Characterizing CFTR Modulated Changes

Treatment of culturally diverse children and adolescents with depression.

This article is written for the practitioners treating depression in ethnic minority youth. It will review the context in which services are delivered to these youth: Researchers have recognized persistent ethnic differences in terms of utilization of services and unmet need. Furthermore, when ethnic minority youth do receive pediatric mental health care, the services that they receive may differ from those given to White patients. The reasons for these discrepancies have been examined in numerous studies, and have included contextual variables (economics, availability, and accessibility of se

Association between post-discharge destination and osteoporosis medication use after acute-care hospital discharge for hip fracture: a population-based cohort study.

UNLABELLED: Post-discharge destination was associated with differences in osteoporosis medication use after hip fracture. Discharge to long-term care facilities or home with long-term care services was associated with lower medication use. Prior treatment history was associated with differences in subgroup-specific estimates among community-dwelling patients, although formal interaction testing was not statistically significant. BACKGROUND: Post-discharge destination may influence osteoporosis care after hip fracture, but evidence is limited. METHODS: Using the National Health Insurance Serv

Risk factors for non-vertebral fractures in community-dwelling elderly: a 10-year follow-up study in New Zealand.

UNLABELLED: This 10-year study of 5000 + adults aged 50-84 found 20% experienced non-vertebral fractures. Higher risk was linked to female sex, older age, European ethnicity, lower education, living alone, alcohol use, prior falls/fractures, osteoporosis, arthritis, and antidepressants. Targeting modifiable factors (living arrangements, alcohol, antidepressants) could reduce fracture burden cost-effectively in older adults. BACKGROUND: Although there has been extensive research on non-vertebral fractures, their risk factors remain incompletely understood. This study aimed to examine risk fact

Geographic access to critical care obstetrics for women of reproductive age by race and ethnicity.

BACKGROUND: The goal of risk-appropriate maternal care is for high-risk pregnant women to receive specialized obstetrical services in facilities equipped with capabilities and staffing to provide care or transfer to facilities with resources available to provide care. In the United States, geographic access to critical care obstetrics varies. It is unknown whether this variation in proximity to critical care obstetrics differs by race, ethnicity, and region. OBJECTIVE: We examined the geographic access, defined as residence within 50 miles of a facility capable of providing risk-appropriate c

Water and sewage systems, socio-demographics, and duration of residence associated with endemic intestinal infectious diseases: a cohort study.

BACKGROUND: Studies of water-related gastrointestinal infections are usually directed at outbreaks. Few have examined endemic illness or compared rates across different water supply and sewage disposal systems. We conducted a cohort study of physician visits and hospitalizations for endemic intestinal infectious diseases in a mixed rural and urban community near Vancouver, Canada, with varied and well-characterized water and sewage systems. METHODS: Cohort members and their disease events were defined via universal health insurance data from 1995 through 2003. Environmental data were derived

Prostate cancer disparities in South Carolina: early detection, special programs, and descriptive epidemiology.

Available evidence suggests that there may be qualitative differences in the natural history of PrCA by race. If this is true then additional etiologic research is needed to identify places in the causal chain where we can intervene to lower PrCA rates in AA men. South Carolina may prove to be a useful context in which to study prostate cancer etiology, because of the presence of unique environmental exposures. For example, soil selenium and cadmium concentrations unique to South Carolina might have a differential affect in the rural areas of the state where ground water use is more common and

Hepatitis C micro-elimination through the retrieval strategy of patients lost to follow-up.

BACKGROUND AND AIM: World Health Organization sets up an ambitious and attainable goal to eliminate hepatitis C (HCV) by 2030. The previous diagnosed HCV patients lost to follow-up were considered as an important target group for HCV elimination. We conducted a call back program to retrieve the lost to follow-up HCV patients and link them to care in our hospital. By analyzing and comparing our result with that from other studies, we wish to improve our retrieval strategy and provide our experience to the general communities. METHODS: A list of the patients with a medical record showing seropo

Protective Role of Limitrin in Experimental Autoimmune Optic Neuritis.

IMPORTANCE: Few stroke survivors meet recommended cardiovascular goals, particularly among racial/ethnic minority populations, such as Black or Hispanic individuals, or socioeconomically disadvantaged populations. OBJECTIVE: To determine if a chronic care model-based, community health worker (CHW), advanced practice clinician (APC; including nurse practitioners or physician assistants), and physician team intervention improves risk factor control after stroke in a safety-net setting (ie, health care setting where all individuals receive care, regardless of health insurance status or ability t

The influence of socioeconomic status on the association between residential greenness and gestational diabetes mellitus in an urban setting: a multicenter study.

BACKGROUND: Inconsistencies were found between residential greenness and the risk of gestational diabetes mellitus (GDM), possibly due to variations in susceptibility among populations with different characteristics. However, little is known about whether this association could be modified by population characteristics like socioeconomic status (SES). This multicenter study conducted in a dense megacity aims to clarify these associations and explore the modification effects of demographic and socioeconomic factors. METHODS: The study included 19,618 pregnant women in 20 hospitals throughout S

Burden of Renal Cysts Imaging: A Survey of Patients among the Greater Plains Collaborative.

PURPOSE: Renal cysts are a frequent incidental finding on cross-sectional radiographic imaging. While most cysts are indolent, individuals with such cysts are frequently monitored for interval growth and potential malignant transformation, which is ultimately rare. In this study, we aimed to assess patients' values and preferences (believes and attitudes) about renal cysts. METHODS: We deployed a cross-sectional survey to a random sample of patients with a diagnosis of renal cysts who were identified by billing code and self-identification. We collected data about demographics, insurance stat

Identifying pre-diabetes 'hotspots' in Northern California using geospatial analysis: opportunities to target diabetes prevention strategies and improve health equity.

OBJECTIVES: The US Preventive Services Task Force recommends screening of adults aged 35-70 with a body mass index ≥25 kg/m2 for type 2 diabetes and referral of individuals who screen positive for pre-diabetes to evidence-based prevention strategies. The diabetes burden in the USA is predicted to triple by 2060 necessitating strategic diabetes prevention efforts, particularly in areas of highest need. This study aimed to identify pre-diabetes hotspots using geospatial mapping to inform targeted diabetes prevention strategies. A 'hotspot' is defined as a cluster of 3 or more neighbouring census

How new subscribers use cancer-related online mailing lists.

BACKGROUND: Online cancer-related support is an under-studied resource that may serve an important function in the information seeking, care, and support of cancer patients and their families. With over 9.8 million cancer survivors (defined as anyone living with cancer) in the United States alone and the number growing worldwide, it is important to understand how they seek and use online resources to obtain the information they need, when they need it, and in a form and manner appropriate to them. These are stated cancer communication goals of the US National Cancer Institute. OBJECTIVES: Our

Testing a Machine Learning-Based Adaptive Motivational System for Socioeconomically Disadvantaged Smokers (Adapt2Quit): Protocol for a Randomized Controlled Trial.

BACKGROUND: Individuals who are socioeconomically disadvantaged have high smoking rates and face barriers to participating in smoking cessation interventions. Computer-tailored health communication, which is focused on finding the most relevant messages for an individual, has been shown to promote behavior change. We developed a machine learning approach (the Adapt2Quit recommender system), and our pilot work demonstrated the potential to increase message relevance and smoking cessation effectiveness among individuals who are socioeconomically disadvantaged. OBJECTIVE: This study protocol des

Variations in practice and outcomes in the Canadian NICU network: 1996-1997.

BACKGROUND: Previous reports of variations in outcomes among neonatal intensive care units (NICUs) examined only specific subpopulations of interest (eg, very low birth weight [VLBW] infants <1500 g of birth weight [BW]). OBJECTIVES: We report on current practice and outcomes variations in a population-based national study of Canadian NICUs from January 8, 1996 to October 31, 1997. METHOD: Information on 20 488 admissions to 17 tertiary level NICUs across Canada was prospectively collected by trained abstractors using a standard manual of operations and definitions. Data were verified and an

Clinical Outcomes of Patients With Newly Diagnosed Acute Lymphoblastic Leukemia in a County Hospital System.

BACKGROUND: Major advances in the treatment of acute lymphoblastic leukemia (ALL) over the past decade have resulted in 5-year overall survival (OS) rates of 80% in mature B cell ALL, 50% in precursor B cell ALL, 50% to 60% in T cell ALL, and 60% to 70% in Philadelphia chromosome-positive (Ph+) ALL, as reported in studies from large, specialized centers. However, many patients treated in the community have limited access to novel therapies and stem cell transplantation (HSCT). PATIENTS AND METHODS: The purpose of this retrospective cohort analysis was to evaluate the clinical outcomes of pati
