Long-term follow-up of passive containment surgery in patients with aortic regurgitation.

The goal of this case-control study was to evaluate the long-term effects on cardiac dimensions, cardiac function and coronary circulation in patients with aortic regurgitation (AR) and left ventricular dilatation undergoing aortic valve replacement and application of the Acorn CorCap cardiac support device Of 10 patients with AR and ventricular dilatation who had a mechanical aortic valve implanted, 5 in addition received the cardiac support device. Cardiac dimensions and cardiac function were measured by echocardiography preoperatively and 1, 5 and 10 years postoperatively. The coronary circ…

[Endothelin--an important factor in coronary heart disease].

The peptide hormone endothelin was initially discovered in endothelial cells. However, it has recently been shown that endothelin is produced by cardiomyocytes too. Endothelin is the most potent of all known vasoconstrictors. The coronary circulation is highly sensitive to endothelin, and intracoronary infusion may lead to lethal coronary spasm. Patients with variant angina pectoris have a higher plasma concentration of endothelin. Moreover, exposure to cold leads to a further increase in endothelin. Therefore increased production of endothelin may be an important mediator of vasospasm. Acute …

Successful repair of iatrogenic acute aortic dissection with cerebral malperfusion.

We describe the successful treatment of a patient with iatrogenic acute aortic dissection including cerebral malperfusion as a complication of coronary artery surgery. After beginning cardiopulmonary bypass, a retrograde ascending aortic dissection associated with cerebral malperfusion was recognized. Systemic circulation was immediately arrested at 31 degrees C. After aortotomy, hypothermic selective antegrade cerebral perfusion was established. Replacement of the ascending aorta with coronary artery bypass grafting was performed without neurologic complications.

Congenital atresia of the orifice of the left coronary artery and its main stem.

A 13-year-old black male who died suddenly as a result of atresia of the left coronary ostium and left main coronary artery is presented. An association between this anomaly and supravalvar aortic stenosis is observed. The literature is reviewed and 14 similar cases are compared. Presentation of this anomaly is variable, as is survival and may be attributed to the development of an adequate collateral circulation.

Regression of coronary disease after bypass surgery: Urban myth or common finding?

OBJECTIVES: Coronary artery disease has been viewed as a relentless, progressive disease. We sought to describe the prevalence and distribution of regression of native vessel disease in coronary artery bypass patients and characterize its relationship with bypass grafting. METHODS: Among 619 patients who underwent bypass surgery in a radial artery trial, 405 had follow-up angiography available a mean of 6.2 ± 3.1 years (range, 0-14) after surgery. The percentage of diameter stenosis of each major native coronary vessel was reported by 3 cardiac specialists and classified into grades of nonflo

Use of transesophageal echocardiography in evaluating coronary arteries.

Because of the better resolution of higher frequency transducers and the proximity of the coronary arteries to the esophageal window, TEE is emerging as a valuable method for evaluation of coronary artery disease. TEE allows imaging of the proximal coronary arteries, measurement of coronary flow reserve, identification of coronary artery anomalies, and observation of wall motion during transesophageal atrial pacing. The application of TEE in evaluation of coronary artery disease will continue to grow as technology improves.

Blood flow to the heart from noncoronary arteries: an intriguing but challenging research field.

INTRODUCTION: It has been proven that blood supply to the heart can include blood from noncoronary collateral circulation. Whether this network can somehow be augmented to provide an alternative therapy for ischemic patients is an intriguing hypothesis with no clear answer yet due to the challenging nature of this research field. In an attempt to enhance noncoronary collateral blood flow, we experimented with left internal thoracic artery ligature and angiogenic growth factors in a canine model. MATERIALS AND METHODS: Eight dogs weighing between 20 and 29 kg were scheduled for three operation

Interpretation of changes in coronary flow that accompany pharmacologic interventions.

The interpretation of a change in coronary flow that accompanies administration of a calcium-entry blocker or other pharmacologic agent remains complicated by the variety of factors potentially altered by the agent that can themselves affect flow. These factors are reviewed in the context of steady-state coronary pressure-flow relationships, emphasizing the complexities induced by coronary artery disease. Limitations of currently available approaches for the measurement of coronary reserve, examination of flow heterogeneity, and the calculation of coronary vascular resistance are also addresse

Plasma microRNA-100 is associated with coronary plaque vulnerability.

BACKGROUND: Although numerous studies have reported altered plasma levels of various microRNAs (miRNAs) in patients with cardiovascular dise … th %LV (r=0.53, P<0.01) and %FV (r=-0.56, P<0.01). CONCLUSIONS: miR-100 might be released into the coronary circulation from vulnerable coronary plaques. This study provides insights into the role of miRNAs in coronary atherosclerotic disease.

Diastolic Left Main Coronary Artery Flow Reversal.

In patients undergoing percutaneous cardiac interventions, perioperative transesophageal echocardiography is used routinely, often revealing an unusual pathology that was not previously detected with transthoracic echocardiography. In this e-challenge, the authors present a patient undergoing percutaneous transcatheter aortic valve replacement, with preprocedural transesophageal echocardiography revealing an abnormal color Doppler signal near the left main coronary artery during diastole.

Simulating Stenotic Conditions of the Coronary Artery in a Lumped Parameter Model of the Cardiovascular System.

Coronary flow control mechanisms maintain the average coronary blood flow (CBF) at 4% of the cardiac output (CO) in normal adults, with no prior diagnosis of coronary artery disease (CAD), under resting conditions. This paper explores a pulsatile sixth order lumped parameter (LP) model of the cardiovascular system (CVS) which utilizes the average CBF approximated from CO along with arterial blood pressure (ABP) waveform to estimate the coronary microvascular resistance using non-linear least square optimization technique. The CVS model includes a third order model of the coronary vascular bed

Treatment of Angina Pectoris Associated with Coronary Microvascular Dysfunction.

Treatment of angina pectoris associated with coronary microvascular dysfunction is challenging as the underlying mechanisms are often diverse and overlapping. Patients with type 1 coronary microvascular dysfunction (i.e. absence of epicardial coronary artery disease and myocardial disease) should receive strict control of their cardiovascular risk factors and thus receive statins and ACE-inhibitors in most cases. Antianginal medication consists of ß-blockers and/or calcium channel blockers. Second line drugs are ranolazine and nicorandil with limited evidence. Despite individually titrated com

Alcohol and the heart. How it helps, how it harms.

Does regular consumption of alcohol protect against coronary artery disease but not against sudden death? In this article, Dr Sheehy examines several theories that have been proposed to explain the cardioprotective effect of ethanol and also reviews the factors related to sudden death in alcoholics.

Myocardial ischaemia in the hypertensive patient--the role of coronary microcirculation abnormalities.

Clinicians have long suspected that hypertrophied hearts have an inadequate coronary circulation. This clinical suspicion resulted from the observation that a large proportion of patients with left ventricular hypertrophy who have normal coronary arteriograms complain of angina and have ischaemia-like ST segment depression during effort. Hypertension is a known risk factor for the development of coronary artery disease and therefore the presence of angina in a patient with systemic hypertension is usually attributed to obstructive atherosclerotic coronary artery disease. However, angina may oc…
