Outcomes with transforaminal endoscopic versus percutaneous laser decompression for contained lumbar herniated disc: a survival analysis of treatment benefit.

BACKGROUND: Contained lumbar disc herniations frequently cause back- and leg pain. Clinical outcomes with surgical treatment may be affected by the size and location of the disc herniation. The surgical directly visualized transforaminal endoscopic decompression has gained acceptance and popularity, while the simplified percutaneous laser disc decompression has fallen out of favor in spite of its initial success as a minimally invasive intervention. In an attempt to better understand the durability of both procedures, the authors performed a comparative analysis of clinical outcomes in patient

Tailoring the Surgical Approach to Chiari I Malformation with Intraoperative Ultrasounds: Advantages, Limitations, and Controversies.

INTRODUCTION: Decompression of posterior cranial fossa remains the mainstay of surgical treatment of Chiari malformation type I, though several surgical maneuvers have been differently combined to bony decompression aiming to increase the efficacy of surgery. In this context, intraoperative ultrasound (IOUS) claims to offer real-time feedback of adequacy of surgical decompression, thus accordingly tailoring the aggressiveness of surgery. The present review aims to highlight benefits, limitations, and areas in need of further investigation. MATERIALS AND METHODS: A comprehensive literature sea

Biomechanical Analysis and Clinical Study of Augmented Versus Conventional Endoscopic Orbital Decompression for Dysthyroid Optic Neuropathy.

Dysthyroid optic neuropathy (DON) represents a severe ocular complication in thyroid eye disease (TED) that can lead to vision loss. Although surgical decompression is a well-established treatment modality, the optimal decompression area remains controversial in orbital decompression surgery. Purpose: This study aims to develop and validate a finite element analysis (FEA) model of DON to compare the biomechanical behavior between patients undergoing conventional or augmented orbital decompression surgery, with potential clinical implications for surgical planning. Methods: FEA models were esta

Percutaneous laser disc decompression versus conventional microdiscectomy for patients with sciatica: Two-year results of a randomised controlled trial.

Background Percutaneous laser disc decompression is a minimally invasive treatment, for lumbar disc herniation and might serve as an alternative to surgical management of sciatica. In a randomised trial with two-year follow-up we assessed the clinical effectiveness of percutaneous laser disc decompression compared to conventional surgery. Materials and methods This multicentre randomised prospective trial with a non-inferiority design, was carried out according to an intent-to-treat protocol with full institutional review board approval. One hundred and fifteen eligible surgical candidates, wi

Targeting the Symptom-Driving Level in Multilevel Lumbar Stenosis Using Unilateral Biportal Endoscopy: A Strategy Reappraisal.

Background/Objectives: Multilevel lumbar spinal stenosis (MLSS) is frequently encountered in patients undergoing surgery for lumbar spinal stenosis, yet the optimal extent of decompression remains debated. While multilevel decompression (MLD) may address all radiological stenotic levels, it may also increase surgical invasiveness and operative time. Minimally invasive endoscopic techniques such as unilateral biportal endoscopy (UBE) allow for targeted decompression and facilitate staged surgical strategies. The aim of this study was to evaluate the clinical outcomes of selective single-level d

Five-year outcomes from a multi-center prospective randomized controlled trial of minimally invasive interspinous fixation versus open direct decompression for lumbar spinal stenosis.

BACKGROUND: Interspinous fixation devices (IFDs) provide indirect decompression and stabilization for lumbar spinal stenosis (LSS) and are potentially less invasive than open decompression, but long-term comparative data are limited. OBJECTIVE: To evaluate the long-term clinical efficacy, safety, and perioperative outcomes of indirect decompression and fusion with an IFD versus standard open surgical decompression in patients with LSS. METHODS: 48 patients were randomly assigned to IFD or decompression. Primary endpoints were changes in Oswestry Disability Index (ODI), Visual Analogue Scale

Impact of obesity on complications and outcomes: a comparison of fusion and nonfusion lumbar spine surgery.

OBJECTIVE Prior studies have shown obesity to be associated with higher complication rates but equivalent clinical outcomes following lumbar spine surgery. These findings have been reproducible across lumbar spine surgery in general and for lumbar fusion specifically. Nevertheless, surgeons seem inclined to limit the extent of surgery, perhaps opting for decompression alone rather than decompression plus fusion, in obese patients. The purpose of this study was to ascertain any difference in clinical improvement or complication rates between obese and nonobese patients following decompression a

Decompression With or Without Fusion for Lumbar Stenosis: A Cost Minimization Analysis.

STUDY DESIGN: Retrospective database review. OBJECTIVE: Compare 1-year episode of care costs between single-level decompression and decompression plus fusion for lumbar stenosis. SUMMARY OF BACKGROUND DATA: Lumbar stenosis is the most common indication for surgery in patients over 65. Medicare direct hospital costs for lumbar surgery reached $1.65 billion in 2007. Despite stenosis being a common indication for surgery, there is debate as to the preferred surgical treatment. Cost-minimization analysis is a framework that identifies potential cost savings between treatment options that have si

Surgical and nonsurgical treatments for lumbar spinal stenosis.

Lumbar spinal stenosis (LSS) is the most common indication for spinal surgery in older adults; however, the efficacy of surgery for LSS as compared to nonsurgical treatments remains unclear. Here, we reviewed numerous studies, including randomized control trails (RCTs), to compare nonsurgical and surgical treatments for LSS. The nonsurgical management of LSS includes medication, epidural injections, physiotherapy, lifestyle modification, and multidisciplinary rehabilitative approaches. Patients with LSS who do not improve after nonsurgical treatments are typically treated surgically using deco

Comparable increases in dural sac area after three different posterior decompression techniques for lumbar spinal stenosis: radiological results from a randomized controlled trial in the NORDSTEN study.

PURPOSE: To investigate changes in dural sac area after three different posterior decompression techniques in patients undergoing surgery for lumbar spinal stenosis. Decompression of the nerve roots is the main surgical treatment for lumbar spinal stenosis. The aim of this study was to radiologically investigate three commonly used posterior decompression techniques. METHODS: The present study reports data from one of two multicenter randomized trials included in the NORDSTEN study. In the present trial, involving 437 patients undergoing surgery, we report radiological results after three dif

Rhinogenous optic neuritis with full recovery of vision - The role of endoscopic optic nerve decompression and a review of literature.

BACKGROUND: Optic neuritis resulting from paranasal sinusitis is an infrequently described but clinically important and treatable entity. The role of optic nerve decompression has been well established in atraumatic optic neuropathies which are compressive in origin. However, its role in optic neuritis and other infective or inflammatory processes is lacking, and the role for early surgical intervention remains controversial. CASE REPORT: In this case report, we describe a patient who presented with sudden onset of right vision loss secondary to optic neuritis from pansinusitis. He was treate

Total body replacement with iliac bone graft and metal plate stabilization in lower cervical spine.

15 patients presented with cervical spine dislocation and kyphotic spondylosis due to traumatic fracture and vertebral body tumour underwent surgical decompression and fixation via the anterior approach in one stage. Decompression was achieved by resection of the body of the vertebra while fixation meant implantation of iliac bone graft with metal plate fixation. Early reduction of the dislocation was impossible or insufficient in most of the cases. Therefore the majority of operations were "late decompression". Patients with root or partial cord lesions had the most significant improvement. I

Neurovascular decompression for cranial rhizopathies.

For some cranial rhizopathies, such as hemifacial spasm and glossopharyngeal neuralgia, treatment has been unsatisfactory. Neurovascular decompression now offers a cure in almost 90% of these cases. In spite of the availability of alternative minor surgical procedures for trigeminal neuralgia, neurovascular decompression, being non-destructive, is a superior method of treatment. Vascular compression may also play a role in atypical facial pain and decompression may offer relief in this intractable condition. Vascular compression was not observed on exploration for nervus intermedius neuralgia,

Effect of Lycium barbarum polysaccharide on functional recovery after decompression in a rat model of degenerative cervical myelopathy.

STUDY DESIGN: Animal study. PURPOSE: To investigate the effects of Lycium barbarum polysaccharide (LBP) on functional recovery following decompressive surgery in a rat model of cervical spondylotic myelopathy (CSM). OVERVIEW OF LITERATURE: Surgical decompression halts the progression of CSM, but may leave residual neurologic deficits. LBP, derived from wolfberry, has been shown to modulate macrophage polarization and exert neuroprotective effects in acute spinal cord injury. We hypothesized that LBP would enhance functional recovery after surgical decompression in a rat model of CSM. METHOD

Posterolateral spinal cord decompression in patients with metastasis: an endoscopic assisted approach.

Spinal tumors that are radioresistant or cause bony compression of the spinal cord often require surgical decompression to protect or restore neurological function. Metastatic lesions and primary tumors such as multiple myeloma usually arise in the vertebral body, which can collapse and become unstable, and can compress the anterior columns of the cord. Laminectomy is often ineffective in these patients, and direct anterior decompression through thoracotomy is the widely-accepted solution to the neurological problem. The anterior surgical approach is particularly challenging in the upper thora

Effects of perineural steroid injections on median nerve conduction during the carpal tunnel release.

BACKGROUND/AIM: The treatment outcome of the median nerve compressive neuropathy in the carpal zone due to carpal tunnel syndrome (CTS) is represented by recovering the nerves sensibility, conductivity, condition and strength. Perineural application of betamethasone during the surgical decompression might result in faster recovery of compressed median nerve's conduction speed. METHODS: In this study 40 patients with CTS were randomly divided in the two groups. In the first group (n = 20) we performed the surgical decompression of the median nerve by the open release of the carpal tunnel, and
