Effect of previous miscarriage on depressive symptoms during subsequent pregnancy and postpartum in the first baby study.

Our objective was to test the hypothesis that nulliparous women with a history of miscarriage have an increased risk of depression during late pregnancy, and at 1, 6, and 12 months postpartum compared to women without a history of miscarriage. We conducted secondary analysis of a longitudinal cohort study, the First Baby Study, and compared 448 pregnant women with a history of miscarriage to 2,343 pregnant women without a history of miscarriage on risk of probable depression (score >12 on the Edinburgh Postnatal Depression Scale). Logistic regression models were used to estimate odds ratios at

Association Between HbA1c Levels and Postpartum Depressive Symptoms: Insights from Edinburgh Postnatal Depression Scale Screening After Gestational Diabetes Mellitus.

Background/Objectives: Depressive symptoms are more frequent following pregnancies complicated by gestational diabetes mellitus (GDM), prompting postpartum depression screening recommendations in the German S3 guideline. Although GDM is associated with increased postpartum depression risk, the relationships between postpartum metabolic parameters and the Edinburgh Postnatal Depression Scale (EPDS) scores remain unclear. We investigated whether metabolic markers measured during diabetes screening 6-12 weeks postpartum are associated with depressive symptoms. Methods: We retrospectively analyzed

Preventive effect of transcutaneous auricular vagus nerve stimulation on postpartum depression in cesarean women with antenatal depressive tendency: a randomized controlled trial protocol.

BACKGROUND: Postpartum depression is one of the most common psychological disorders among women after childbirth, with a global prevalence of approximately 10-20% and an even higher rate of 21.4% reported in China. Women undergoing cesarean section are at increased risk for postpartum depression due to perioperative pain, delayed recovery, maternal-infant separation, and psychological stress. Antenatal depression is also a major risk factor for postpartum depression, highlighting the need for effective preventive interventions in this population. Transcutaneous auricular vagus nerve stimulatio

Internet-Based Cognitive Behavioral Therapy for Preventing Postpartum Depressive Symptoms Among Pregnant Individuals With Depression: Multicenter Randomized Controlled Trial in China.

BACKGROUND: Women are particularly vulnerable to depression during pregnancy, which is one of the strongest risk factors for developing postpartum depression (PPD). Addressing antenatal depressive symptoms in these women is crucial for preventing PPD. However, little is known about the effectiveness of internet-based cognitive behavioral therapy (ICBT) in preventing PPD in this high-risk group. OBJECTIVE: This study aims to evaluate the short- and long-term effects of ICBT in preventing PPD among women with antenatal depressive symptoms. METHODS: Participants were screened for antenatal depr

Antenatal depressive symptoms and subjective birth experience in association with postpartum depressive symptoms and acute stress reaction in mothers and fathers: A longitudinal path analysis.

OBJECTIVES: Postpartum depressive symptoms (PDS) and acute stress reactions (ASR) after childbirth are frequently documented in mothers, but research is scarce in fathers. In a longitudinal path analysis, the interplay of depressive symptoms in pregnancy and the subjective childbirth experience of mothers and fathers are examined with regard to the development of PDS and ASR postpartum. STUDY DESIGN: One hundred eighty nine expectant couples were recruited between August 2006 and September 2009. They completed the Edinburgh Postnatal Depression Scale (EPDS) in the last trimester of pregnancy.

Advancing equitable postpartum depression screening in a level-IV NICU: a quality improvement approach shaped by planned experimentation.

BACKGROUND: Given the incidence and impact of postpartum depression (PPD) on families in the NICU, we aimed to increase the percentage of birthing parents screened for PPD from 0 to 90% over 18-months and provide psychosocial support. METHODS: Planned experimentation was used to design an effective screening framework in a level IV NICU. Interventions included process standardization, electronic health record (EHR) support, and data visualization for feedback. Data were displayed on a control chart and stratified for race and language to ensure equitable delivery. RESULTS: Provider screening

Antepartum obstructive sleep apnea screening and its association with postpartum respiratory depression: a retrospective cohort study (2020-2025).

BACKGROUND: Obstructive sleep apnea is not routinely screened for in the peripartum population, and its association with postpartum respiratory depression remains unclear. We evaluated positive antepartum obstructive sleep apnea screening and its association with postpartum respiratory outcomes. METHODS: We conducted a single-center retrospective cohort study of parturients undergoing routine antepartum obstructive sleep apnea STOP screening (loud snoring, daytime tiredness, observed apnea, high blood pressure) from July 2020 to June 2025. Delivery encounters with at least one STOP response w

Coping Styles, Postpartum Depression, and Anxiety in Romanian Women: A Cross-Sectional Study Using the Brief COPE Inventory.

Background/Objectives: Postpartum depression and anxiety affect up to 20% of women worldwide, yet remain understudied in Eastern Europe. Romania, one of Europe's most religious countries, provides a unique context for examining how coping strategies and religiosity influence perinatal mental health. This cross-sectional study characterized coping styles, depressive and anxiety symptoms, and religiosity among postpartum women from two western Romanian counties (Bihor and Timiș) and examined associations between coping dimensions and psychological outcomes. Methods: A cross-sectional study was c

Neuroendocrine and neuroinflammatory mechanisms in postpartum depression: From hormonal withdrawal to central immune dysregulation and emerging therapeutic targets.

Postpartum depression (PPD) affects approximately 10-15% of new mothers worldwide with profound consequences extending to offspring and partners. While the classical "hormonal withdrawal hypothesis" has long emphasized the abrupt decline in estradiol, progesterone, and cortisol following childbirth, emerging evidence demonstrates that these endocrine perturbations converge with dysregulated neuroimmune pathways as central drivers of PPD pathophysiology. This narrative review integrates mechanistic evidence linking reproductive hormone withdrawal, HPA axis dysregulation, neurotransmitter imbala

Clinical feature grouping-based FT-Transformer for postpartum depression risk prediction: a longitudinal cohort study in perinatal women.

Postpartum depression (PPD) poses risks to maternal health and family relationships, making identification of high-risk individuals important. Most PPD prediction models treat clinical variables as an undifferentiated set and rarely use the clinical grouping structure of risk factors. Using perinatal and postpartum data from 1138 women, including clinical characteristics, biochemical indicators, and psychological assessments such as the Eysenck Personality Questionnaire (EPQ) and Beck Depression Inventory (BDI), with PPD defined as an Edinburgh Postnatal Depression Scale (EPDS) score of at lea

MKRN1 as a prioritized drug target for postpartum depression: evidence from druggable proteome profiling and multi-layer validation.

Postpartum depression (PPD) is a significant global health concern affecting women, yet effective and innovative therapeutic targets remain limited. Although genome-wide association studies (GWAS) have identified genetic risk loci, their underlying mechanisms and translational potential remain poorly understood. Therefore, we integrated PPD GWAS data with protein quantitative trait loci from two independent datasets to identify risk genes through proteome-wide association studies (PWAS). Validation was performed using colocalization analysis and Mendelian randomization (MR). To assess the safe

Impact of Vitamin D during Pregnancy towards Postpartum Depression: A Meta-Analysis of Dose-response Association and Intervention.

INTRODUCTION: Serum vitamin D (25(OH)D) concentration has been associated with PPD in previous studies. However, whether there is a dose-response relationship and if its supplementation during pregnancy can improve symptoms after delivery remains unclear. METHODS: Five databases were searched on September 27, 2025. Cohort studies were included for dose-response meta-analysis (DRMA), while randomized controlled trials (RCTs) were included for meta-analysis (MA) of supplementation. The outcome for MA of supplementation was postpartum Edinburgh Postnatal Depression Scale (EPDS) score using the e

Letter to the editor: Beyond reductionism in postpartum depression treatment and toward systems-oriented maternal mental health care.

Postpartum depression (PPD) is a heterogeneous mood disorder emerging within the first year after childbirth and shaped by interacting biological, psychological, and social factors. Although treatment is commonly adapted from major depressive disorder, the postpartum period represents a distinct and dynamically regulated physiological state. Rapid hormonal withdrawal, neuroendocrine recalibration, immune shifts, and adaptive neuroplastic changes occur concurrently, contributing to variable treatment response and clinical heterogeneity. Viewing PPD as a disorder of systems-level dysregulation h

Brexanolone infusion produces sustained anti-inflammatory and neurotrophic effects in patients with postpartum depression that predict symptom improvement.

Postpartum depression (PPD) is linked to neuroimmune dysregulation. Brexanolone, an intravenous formulation of the neurosteroid allopregnanolone and the first FDA-approved treatment for PPD, produces rapid and sustained antidepressant effects. However, its long-term mechanisms of action remain unclear. This study evaluated brexanolone's prolonged impact on two groups of biomarkers in whole blood: inflammatory mediators and growth/differentiation/neurotrophic factors. Whole blood was also maintained in culture (4 h) and subjected to lipopolysaccharide (LPS) stimulation of the TLR4 inflammatory

Building a Machine Learning Model to Predict Postpartum Depression from Electronic Health Records in a Tertiary Care Setting.

Background: Postpartum depression is a common mental health condition that can occur up to one year after childbirth. Recent studies have increasingly used machine learning techniques to predict its occurrence; however, few have comprehensively explored the use of electronic health record data, particularly in tertiary care settings where such data can be fragmented. Methods: We analyzed electronic health record data from 12,284 women who delivered at The Birth Center at Atrium Health Wake Forest Baptist Medical Center, excluding those with missing data or no prenatal or postpartum visits. Enduring effects of brief interpersonal therapy on depression from pregnancy through 1-year postpartum: A randomized clinical trial.

Postpartum depression is prevalent with negative consequences for both mothers and developing infants. Little is known about how much the effects of depression treatment, delivered during pregnancy, persist as relapse prevention through 1-year postpartum. This study evaluated whether improvements in depression symptoms and diagnostic status observed during pregnancy following prenatal treatment (randomized to brief Interpersonal Psychotherapy (IPT) versus Enhanced Usual Care (EUC)) persisted through 12- months postpartum. A prospective, evaluator-blinded, randomized clinical trial (RCT) was co
