Adjunctive minocycline treatment for major depressive disorder: A proof of concept trial.

OBJECTIVE: Conventional antidepressant treatments result in symptom remission in 30% of those treated for major depressive disorder, raising the need for effective adjunctive therapies. Inflammation has an established role in the pathophysiology of major depressive disorder, and minocycline has been shown to modify the immune-inflammatory processes and also reduce oxidative stress and promote neuronal growth. This double-blind, randomised, placebo-controlled trial examined adjunctive minocycline (200 mg/day, in addition to treatment as usual) for major depressive disorder. This double-blind, r…

[The neural bases of social cognition in major depressive disorder: a review].

INTRODUCTION: Major depressive disorder (MDD) is associated with a significant impairment of social and interpersonal functioning. Several neuroimaging studies have evaluated social cognition, i.e. how people with MDD process, store and analyze information about other people and social situations. METHODS: We conducted a focused review and selected manuscript published until August 2016 indexed on PubMed and PsycINFO, searching for the following keywords: "major depressive disorder", "major depression", "unipolar depression", "clinical depression", "fMRI", "emotion comprehension", "emotion pe…

Study of functional brain homogeneity in female patients with major depressive disorder.

Women are at a twofold higher risk of developing major depressive disorder (MDD) than that of men. However, the investigation of female MDD patients functional brain activity is rare and the detailed mechanism remains unclear. The present work is to explore the altered spontaneous neural activity measured with regional homogeneity (ReHo) in female MDD patients using resting-state functional magnetic resonance imaging (fMRI) technique. Twelve MDD females and twelve matched healthy participants were included in the study. The ReHo analysis method was used to detect regional homogeneity features …

Data-driven course trajectories in primary care patients with major depressive disorder.

BACKGROUND: The chronic nature of MDD has been acknowledged as one of the key determinants of the burden associated with depression. Unfortunately, so far described prognostic factors have been inconsistent, possibly due to used course outcomes that are often based on arbitrary criteria/cut-offs. Therefore, the aim of the current study was to use data-driven trajectory groups based on closely spaced weekly severity ratings, as outcomes in prognostic research. METHODS: The sample consisted of primary care patients with MDD (n = 153), who were followed up for a year with 52 consecutive weekly r

Do prefrontal midline electrodes provide unique neurophysiologic information in Major Depressive Disorder?

Brain oscillatory activity from the midline prefrontal region has been shown to reflect brain dysfunction in subjects with Major Depressive Disorder (MDD). It is not known, however, whether electrodes from this area provide unique information about brain function in MDD. We examined a set of midline sites and two other prefrontal locations for detecting cerebral activity differences between subjects with MDD and healthy controls. Resting awake quantitative EEG (qEEG) data were recorded from 168 subjects: 47 never-depressed adults and 121 with a current major depressive episode. Individual midl…

Major depressive disorder in older adults: benefits and hazards of prolonged treatment.

Antidepressants have been shown to reduce the risk of depression recurrence in adults, justifying prolonged antidepressant maintenance therapy for most if not all patients. However, older depressed adults may be at increased risk for antidepressant adverse effects. This article discusses the benefits and hazards of continued treatment in elderly depressed patients, and indicates which patients should and should not receive maintenance phase antidepressants. Most clinical trials conducted so far suggest that prolonged antidepressant use in older adults is efficacious to reduce recurrence rates.

Second-generation antipsychotics in the treatment of major depressive disorder: current evidence.

Major depressive disorder (MDD) is a chronic and recurrent mental condition leading to huge impacts on direct and indirect personal and public medical costs. To overcome such a serious mental disorder, we currently have a number of different classes of antidepressants, such as selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, noradrenergic and specific serotonin receptor antagonists, dopamine and norepinephrine reuptake inhibitors, along with newly introduced antidepressants (e.g., vilazodone and agomelatine). However, a number of well-controlled clinical t…

Adjunctive pregabalin in partial responders with major depressive disorder and residual anxiety.

BACKGROUND: Anxiety symptoms in depression result often in treatment resistance, residual symptoms, and persistent functional impairment. OBJECTIVE: To assess the effectiveness and safety of adjunctive pregabalin to antidepressants for residual anxiety in patients with major depressive disorder (MDD). METHODS: A retrospective chart review was conducted to identify partial responders among patients with MDD with residual anxiety. Twenty such patients (age, 58.4 ± 11.2 years; 15 women; baseline Hamilton Depression Rating Scale [HDRS], 17.1 ± 3.5) who received adjunctive pregabalin for residual…

Local cortical thinning links to resting-state disconnectivity in major depressive disorder.

BACKGROUND: Local structural and metabolic as well as inter-regional connectivity abnormalities have been implicated in the neuropathology of major depressive disorder (MDD). How local tissue properties affect intrinsic functional connectivity is, however, unclear. Using a cross-sectional, multi-modal imaging approach, we investigated the relationship between local cortical tissue abnormalities and intrinsic resting-state functional connectivity (RSFC) in MDD. METHOD: A total of 20 MDD in-patients and 20 healthy controls underwent magnetic resonance imaging at 3 T for structural and functiona…

Visual hallucinations during duloxetine treatment in a patient with major depressive disorder.

Duloxetine is a serotonin-noradrenaline reuptake inhibitor that is effective in the treatment of major depressive disorder (MDD) and chronic pain. The safety of duloxetine has been shown by many previous studies. We report a patient who experienced visual hallucinations after taking duloxetine. The patient experienced insomnia and chronic somatic pain. He began taking duloxetine after a diagnosis of MDD and developed visual hallucinations after the duloxetine dose was increased. The hallucinations disappeared after the duloxetine dose was decreased and then stopped; the dose of quetiapine was …

Switching and backward inhibition in major depressive disorder: the role of rumination.

Previous studies have demonstrated that individuals with major depressive disorder have difficulties switching attention from one task set to another. In the current study we examined whether ruminative thinking drives the switching deficits of depressed individuals. A secondary, more exploratory, goal of this study was to examine whether state rumination would impair depressed individuals' ability to activate a new task set, to inhibit a no longer relevant task set, or both. Participants underwent either a rumination induction or a distraction induction and then completed a backward inhibitio…

Glutamatergic approaches in major depressive disorder: focus on ketamine, memantine and riluzole.

The role of glutamate in modulating various mood states is being increasingly recognized and researched. Existing antidepressants have been shown to exert effects on various glutamatergic mechanisms, even if such agents are traditionally classified as working via other mechanisms, such as selective serotonin reuptake inhibitors. The NMDA receptor antagonist ketamine has been investigated in various mood disorders, especially major depressive disorder (MDD). It was found to produce a rapid, robust and persistent antidepressant effect. Although it can produce cognitive, dissociative and perceptu…

[Manic-depressive psychosis - bipolar disorder - an affective spectrum: a change of paradigms].

A key role of cyclicity or polarity in the development of mood disorders is examined in the light of historical analysis of the concepts of these disorders. A critical analysis of the current trends in the study of bipolar spectrum disorders (suggestions on a new definition of hypomania, broadening of the diagnostic boundaries of bipolar disorders in the field that have not been regarded previously as pathological ones or in connection with bipolar disorders, supporting evidence for conceptual continuum between bipolar and unipolar disorders) is presented. Further studies in this field are war

A mega-analysis of genome-wide association studies for major depressive disorder.

Prior genome-wide association studies (GWAS) of major depressive disorder (MDD) have met with limited success. We sought to increase statistical power to detect disease loci by conducting a GWAS mega-analysis for MDD. In the MDD discovery phase, we analyzed more than 1.2 million autosomal and X chromosome single-nucleotide polymorphisms (SNPs) in 18 759 independent and unrelated subjects of recent European ancestry (9240 MDD cases and 9519 controls). In the MDD replication phase, we evaluated 554 SNPs in independent samples (6783 MDD cases and 50 695 controls). We also conducted a cross-disord…

Dysfunction of preattentive visual information processing among patients with major depressive disorder.

BACKGROUND: Major depressive disorder (MDD) is a mental disorder characterized by depressed mood and inability to experience pleasure. Mismatch negativity (MMN) has been established as a correlate of preattentive change detection. In this study, preattentive processing of visual information in MDD patients was investigated using visual MMN (vMMN), and an abnormal vMMN was predicted in depressed patients relative to healthy participants because of dysfunction at the preattentive level. The processes underlying vMMN in MDD patients were also explored, and memory-based comparison preattentive pro…

Acupuncture for the treatment of major depressive disorder: a randomized controlled trial.

BACKGROUND: Over 50% of patients with major depressive disorder (MDD) either do not tolerate or do not respond to antidepressant medications. Several preliminary studies have shown the benefits of acupuncture in the treatment of depression. We sought to determine whether a 2-point electroacupuncture protocol (verum acupuncture) would be beneficial for MDD, in comparison to needling at nonchannel scalp points with sham electrostimulation (control acupuncture). METHOD: Fifty-three subjects aged 18-80 years, recruited via advertisement or referral, were included in the primary analysis of our ra…

Neurostimulatory and ablative treatment options in major depressive disorder: a systematic review.

INTRODUCTION: Major depressive disorder is one of the most disabling and common diagnoses amongst psychiatric disorders, with a current worldwide prevalence of 5-10% of the general population and up to 20-25% for the lifetime period. HISTORICAL PERSPECTIVE: Nowadays, conventional treatment includes psychotherapy and pharmacotherapy; however, more than 60% of the treated patients respond unsatisfactorily, and almost one fifth becomes refractory to these therapies at long-term follow-up. NONPHARMACOLOGICAL TECHNIQUES: Growing social incapacity and economic burdens make the medical community st…

Decreased aldosterone in the plasma of suicide attempters with major depressive disorder.

Hormones and neurobiological factors may be regulated differently in suicidal versus non-suicidal depressive patients. There is currently limited knowledge about the relation of substances in the Renin-Angiotensin-Aldosterone system to depression and suicidality. We therefore investigated whether plasma levels of renin and aldosterone differ between suicide attempters, non-suicidal depressive patients and healthy controls. Furthermore, we analyzed the relation of renin and aldosterone to psychiatric symptoms in the patients. Suicidal patients with MDD, adjustment disorder and dysthymia, as wel
