Symptomatic meningioma in the third trimester of pregnancy: a case report from Kenya.

Meningiomas, the most common primary central nervous system (CNS) tumors, are typically benign and hormonally responsive. They have a higher incidence in women, particularly during the reproductive years. Pregnancy may trigger rapid tumor progression and symptom onset due to elevated hormone levels. Although rare in pregnancy, meningiomas may adversely affect maternal and fetal outcomes, increasing the risk of complications such as preterm labor and gestational hypertension. Management strategies range from conservative monitoring to surgical intervention depending on gestational age, tumor si

Altered mental status in cancer-related emergency hospitalizations: prevalence and recorded reasons for admission.

BACKGROUND: Altered mental status is a critical clinical presentation in emergency departments, yet its frequency in cancer-related emergency hospitalizations and the clinical conditions recorded at presentation have not been well characterized. METHODS: We conducted a retrospective multicenter cohort study using data from the Cancer Registry-based Study on Cancer Care in Osaka (CanReCO), which links hospital-based cancer registry data with Diagnosis Procedure Combination (DPC) administrative data. The cohort included patients diagnosed between January 2019 and December 2022, and linked DPC h

Serum Hepcidin Levels are Associated with Poor Outcomes of Patients with Severe Traumatic Brain Injury: A Two-Center Observational Analytical Study.

BACKGROUND: Severe traumatic brain injury (sTBI) is related to disturbances in iron metabolism, and hepcidin is involved in the regulation of this process. However, it is unclear about serum hepcidin levels after sTBI. In this study, serum hepcidin levels were detected for association with severity and worse outcomes of sTBI. METHODS: In this two-center observational analytical study, age, gender, alcohol drinking, cigarette smoking, hypertension, diabetes mellites and hyperlipidemia were not significantly different between 154 patients with sTBI and 154 controls; and serum hepcidin levels we

Development and validation of an online machine-learning tool for predicting delirium risk in older adults with type 2 diabetes: A retrospective cohort study based on the MIMIC-IV database.

ObjectiveIn older adults with Type 2 diabetes mellitus (T2DM), the risk of delirium is significantly increased, driven by neuropathological alterations stemming from chronic insulin resistance. We utilized artificial intelligence and geriatric electronic health records to create an interpretable online machine-learning algorithm for predicting delirium risk. This tool facilitates prompt identification of high-risk elderly T2DM patients, enabling optimized interventions and improved clinical outcomes.MethodsThis retrospective cohort study identified older adults with T2DM using International Cl

Interaction of Obese Body Mass Index and Comorbidities on Inflammatory Profiles and Clinical Outcomes After Traumatic Brain Injury: A TRACK-TBI Study.

OBJECTIVE: Obesity has been associated with worse clinical outcomes after mild traumatic brain injury (mTBI; Glasgow Coma Scale score between 13 and 15). Augmented inflammatory response to mTBI appears to be an important modifier of outcomes for the patients with obesity, but the contribution of comorbidity profile has yet to be analyzed. The objective of this study was to examine the association of: 1. comorbidity profile; and 2. the interaction of obesity with comorbidity profile on clinical outcomes and inflammatory blood biomarkers after mTBI. METHODS: Participants were enrolled across em

Outcomes After Standardized Burr-Hole Surgery for Chronic Subdural Hematomas: A Population-Based Consecutive Cohort Study of 2655 Patients.

BACKGROUND AND OBJECTIVES: Chronic subdural hematoma (CSDH) is one of the most common neurosurgical conditions. However, current evidence on postoperative outcomes exhibits variability due to small sample sizes, nonstandardized outcome assessment, and variations in surgical techniques. The aim of this study was to overcome these limitations by assessing standardized outcome measures after surgical intervention for CSDH at a high-volume population-based center favoring a uniform burr-hole craniotomy (BHC) approach. METHODS: Adult patients (≥15 years) who underwent surgical treatment of a CSDH

Nocturnal hypoxemia and central apneas increase mortality, but not recurrent ischemic events after ischemic stroke.

BACKGROUND: The aim of the study was to investigate whether findings in cardiorespiratory polygraphy had an association with stroke mortality or ischemic event recurrence after ischemic stroke. METHODS: We prospectively studied 204 ischemic stroke patients who underwent cardiorespiratory polygraphy within the first 48 h after the symptom onset. We followed all these patients for a median of 6.2 years. We evaluated mortality, time of survival, causes of death and new ischemic events. RESULTS: Of 204 ischemic stroke patients, 43 died and 48 had a new ischemic event during the follow-up. The lo

Preinjury statin use is associated with improved in-hospital survival in elderly trauma patients.

BACKGROUND: Recent evidence demonstrates improved survival for septic and bacteremic patients receiving 3-hydroxy-3-methyl-glutaryl-CoA reductase inhibitors (statins). Victims of severe traumatic injury share similar inflammatory cascades to those seen in septic patients. We hypothesized that elderly (aged >/=65 years) trauma patients might derive a similar outcome benefit from preinjury statin use. METHODS: Using the National Study on the Costs and Outcomes of Trauma database (collected from 69 hospitals) we conducted a retrospective observational cohort study of the effects of preinjury sta

Herbal medicinals: selected clinical considerations focusing on known or potential drug-herb interactions.

Herbal medicinals are being used by an increasing number of patients who typically do not advise their clinicians of concomitant use. Known or potential drug-herb interactions exist and should be screened for. If used beyond 8 weeks, Echinacea could cause hepatotoxicity and therefore should not be used with other known hepatoxic drugs, such as anabolic steroids, amiodarone, methotrexate, and ketoconazole. However, Echinacea lacks the 1,2 saturated necrine ring associated with hepatoxicity of pyrrolizidine alkaloids. Nonsteroidal anti-inflammatory drugs may negate the usefulness of feverfew in

Type 1 diabetes mellitus and non-alcoholic fatty liver disease: a two-sample Mendelian randomization study.

BACKGROUND: NAFLD (Nonalcoholic fatty liver disease) is becoming an increasingly common cause of chronic liver disease. Metabolic dysfunction, overweight/obesity, and diabetes are thought to be closely associated with increased NAFLD risk. However, few studies have focused on the mechanisms of NAFLD occurrence in T1DM. METHODS: We conducted a two-sample Mendelian randomization (MR) analysis to assess the causal association between T1DM and NAFLD with/without complications, such as coma, renal complications, ketoacidosis, neurological complications, and ophthalmic complications.
