We describe the use of percutaneous balloon angioplasty of a stenotic truncal valve in a newborn with a common arterial trunk. A 12 mm balloon catheter was chosen, this representing 80% of the diameter of the ventriculo-truncal junction. The peak systolic gradient dropped from 45 to 15 mm Hg. The procedure can avoid, or certainly delay, surgical treatment of the stenotic truncal valve.

The authors prospectively studied 20 fetuses with mild dilatation of the lateral cerebral ventricles but no other detectable central nervous system abnormality. One case (5%) occurred in a diabetic pregnancy, and three (15%) involved twin pregnancies. Fifteen (75%) fetuses were male, and one (5%) had trisomy 21. Postnatal follow-up at 15-31 months showed a normal outcome in eight cases (40%), an uncertain prognosis in four cases (20%), and death in eight cases (40%). This is a report on a 20-year old patient who had been suffering from chronic constipation from her childhood. After radiography of the colon in double contrast in December 1986 the patient was admitted to hospital under suspicion of a perforation of the colon. The radiographic demonstration of the abdomen showed an exceptional dilatation of some segments of the colon but without any signs of perforation by free intraperitoneal gas. Subsequent examination led to the diagnosis of an idiopathic megacolon whereas a congenital as well as symptomatic and toxic megacolon could be excluded.

It has been suggested that patients with chronic supraventricular tachycardia may have impaired ventricular function, which returns to normal after surgical procedures that eliminate the tachycardia. Balloon dilatation (BD) of bioprosthetic valves was investigated in vivo and in vitro. Four children with stenotic bioprosthetic porcine valves in the pulmonary position underwent BD of the valve 10 to 24 months after its insertion. Average valve gradient was reduced from 47.5 to 27 mm Hg. Obstruction at the conduit-branch pulmonary artery connection became apparent after dilatation of the valve. A 35-year-old woman developed temporal lobe seizures. Isolated dilatation of the right temporal horn was demonstrated by computed tomography. She was asymptomatic for the next 10 months while on anticonvulsants before severe headaches, vomiting, and mental confusion prompted hospitalization. The results of the pneumatic cardia dilation in 17 patients are reported, which were yielded by means of a cardia dilation performed in doses. In this case an anaesthesia could be renounced. The pneumatic dilation performed in such a way proved as a risk-poor and effective method which loads the patient only insignificantly, is easily to be performed and has a large range of indication.

Percutaneous balloon valvuloplasty (BV) for pulmonic valve stenosis (PS) is increasingly becoming a nonsurgical alternative in patient management. However, the mechanism by which BV dilates the obstructive lesion has not been firmly established. We have had the opportunity to examine the effects of BV in the setting of PS and present two illustrative cases documenting the morphology after BV of the stenotic bicuspid and tricuspid pulmonary valve.

Eighteen patients with achalasia of the cardia diagnosed by radiography, endoscopy, and manometry were subjected to forced pneumatic dilatation under fluoroscopic control. The procedure was uneventful in all but one patient who developed a pneumomediastinum which resolved on conservative management. Most benign strictures of the esophagus can be successfully dilated with conventional bougienage techniques.
