Osteoblastic cell responses to zoledronate treatment in-vitro: A systematic review.

BACKGROUND: Zoledronate (ZOL), a potent nitrogen-containing bisphosphonate (BP), is widely prescribed for osteoporosis and metastatic bone disease. While osteoclast inhibition is central to ZOL's therapeutic action and to the pathophysiology of medication-related osteonecrosis of the jaw (MRONJ), less is known about its direct effects on osteoblasts. Understanding dose- and time-dependent osteoblastic responses is essential for improving in-vitro modelling and informing regenerative strategies. METHODS: We performed a systematic review of in-vitro studies reporting osteoblastic responses to Z

The effects of anti-osteoporotic medication on fracture healing and outcomes: a systematic review and meta-analysis.

BACKGROUND: Bisphosphonates and teriparatide are used in the management of osteoporosis and reduction of fracture risk. However, their effects on fracture healing have been subject to debate, with concerns that the antiresorptive action of these drugs might impair bone remodelling and delay union. This systematic review and meta-analysis aimed to assess the effects of anti-osteoporotic medication on fracture union, clinical outcomes, complications, and patient-reported outcome measures for acute fractures in adults. METHODS: We searched MEDLINE (1946 to Sept 24, 2025), Embase (1974 to Sept 26

Previously Unreported TMEM38B Variant in Osteogenesis Imperfecta Type XIV: A Case Report and Systematic Review of the Literature.

Osteogenesis imperfecta (OI) type XIV is a rare recessive disorder caused by TMEM38B pathogenic variants that disrupt an endoplasmic reticulum protein essential for calcium homeostasis and bone mineralization. This leads to severe bone fragility, early-onset fractures, skeletal deformities, low bone mass, scoliosis, and variable features like blue sclerae or dental abnormalities. We present a case report of a 21-year-old Italian male with a novel homozygous TMEM38B splice variant (c.112 + 1G > T), detailing the clinical presentation, genetic findings, and therapeutic outcomes. The patient exhi

Regulatory Effects of Quercetin on Bone Homeostasis: Research Updates and Future Perspectives.

The imbalance of bone homeostasis has become a major public medical problem amid the background of an aging population, which is closely related to the occurrence of osteoporosis, osteoarthritis, and fractures. Presently, most drugs used in the clinical treatment of bone homeostasis imbalance are bisphosphonates, calcitonin, estrogen receptor modulators, and biological agents that inhibit bone resorption or parathyroid hormone analogs that promote bone formation. However, there are many adverse reactions. Therefore, it is necessary to explore potential drugs. Quercetin, as a flavonol compound

Denosumab in elderly osteoporotic patients. A narrative review.

OBJECTIVE: Denosumab, an antiresorptive agent, has shown results in improving bone mineral density and reducing fractures in postmenopausal women. While bisphosphonates are commonly used as initial therapy for osteoporosis, some studies suggest that denosumab could be an alternative initial treatment for high-risk patients, particularly the elderly population. This narrative literature review aimed to assess the use of denosumab in elderly individuals with osteoporosis, excluding its oncology applications. METHOD: Multiple online databases including Scopus, PubMed, EMBASE, Cochrane Central Re

Chitosan modified with lanthanum ions as implantable hydrogel for local delivery of bisphosphonates.

Osteoporosis is a disease that affects many people around the world. One group of drugs used to treat it are bisphosphonates. However, they have poor bioavailability and many side effects. Therefore, research around the world is focused on developing bisphosphonate delivery systems. In this paper, we would like to present the design of a hydrogel material with chitosan matrix modified with lanthanum, that could serve as an implantable hydrogel capable of sustained and slow release of Zoledronate. Various research techniques were used to characterize the materials, and the swelling ratio and wa

Advances in pathogenesis and therapeutic strategies for osteoporosis.

Osteoporosis, is the most common bone disorder worldwide characterized by low bone mineral density, leaving affected bones vulnerable to fracture. Bone homeostasis depends on the precise balance between bone resorption by osteoclasts and bone matrix formation by mesenchymal lineage osteoblasts, and involves a series of complex and highly regulated steps. Bone homeostasis will be disrupted when the speed of bone resorption is faster than bone formation. Based on various regulatory mechanisms of bone homeostasis, a series of drugs targeting osteoporosis have emerged in clinical practice, includi

Effects of periodontal and bisphosphonate treatment on the gingival crevicular levels of sclerostin and dickkopf-1 in postmenopausal osteoporosis with and without periodontitis.

OBJECTIVE AND BACKGROUND: Both periodontitis and osteoporosis are associated with osteoclast-related bone resorption. Bone metabolism is regulated by wingless-type MMTV integration site family (WNT), and WNT/β-catenin signals are controlled by physiological antagonists including dickkopf-1 (DKK-1) and sclerostin (SOST). This study examined the effects of periodontal and bisphosphonate (BP) treatment on the gingival crevicular fluid (GCF) sclerostin (SOST) and dickkopf-related protein-1 (DKK-1) levels in osteoporotic and systemically healthy postmenopausal women with and without periodontitis.
