BACKGROUND: Minimally invasive surgery for uncomplicated diverticulitis of the sigmoid (UDS) may be performed either as an intracorporeal procedure (LICR) or as laparoscopically assisted colon resection (LACR).
METHODS: Prospectively collected data of 40 selected patients who had undergone LICR for UDS between 1992 and 1994 were compared retrospectively with those of 34 diagnosis-matched LACR controls operated on at the same hospital between 1995 and 1996 to assess the short-term outcome.
RESULTS: There were no mortalities. LICR and LACR patients were well matched for age, gender, weight, Am…

BACKGROUND: We undertook a prospective evaluation of elective laparoscopic sigmoid colectomy for diverticulitis in order to assess the risks and benefits of this approach.
METHODS: Between November 1992 and November 1996, 54 consecutive patients were included in this study. Their mean age was 59 +/- 13 years (range, 36-81). The number of attacks of diverticulitis before colectomy ranged from one to four (mean, 2.2 +/- 0.7). The operative technique consisted of elective division of the inferior mesenteric vessels, left colonic flexure mobilization, and colorectal anastomosis using the cross-st…

We herein describe the ultrasonographic findings of 19 patients with clinical suspicion of acute diverticulitis. The criteria considered as evidence of the disease included the identification of the diverticuli, thickening of the wall of the loop, alteration of local mesenterium and selective pain. Diagnosis was complemented by CT scan, barium enema and, in determined cases, colonoscopy. Of the 19 cases studied, 18 corresponded to acute diverticulitis of the sigma and one to acute salpingitis. The former 18 cases were correctly diagnosed by ultrasonography (uncomplicated diverticulitis) and th…

PURPOSE: To evaluate the use of laparoscopy, CT, colonic enema (CE), and laboratory tests (white blood cell count (WBC), sedimentation rate (SR), and C-reactive protein (CRP) in diagnosing diverticulitis of the sigmoid colon.
MATERIAL AND METHODS: The diagnostic methods were prospectively evaluated in 88 patients, 30 of whom were referred for laparoscopy.
RESULTS: Fifty-two patients were found to have sigmoid diverticulitis: 20 patients by laparoscopy, 21 by CT, and 11 by CE combined with one positive laboratory test. Laparoscopy proved to be superior to the other diagnostic methods in diagn…

BACKGROUND: Diverticulitis is an uncommon condition in young women. When it occurs, it is often not recognized until complications such as perforation or fistulization occur. There has been no recent discussion in the gynecologic literature of diverticulitis in a young woman presenting as gynecologic disease.
CASE: A 31-year-old woman with a long history of "irritable bowel syndrome" developed a 4-5-cm left adnexal mass associated with mild discomfort and dysparunia. Acute worsening of her pain led to abdominal exploration and left salpingo-oophorectomy for an unruptured tuboovarian abscess. …

The paper reports a case of ureteral [correction of urethral] stenosis with ureterorectal [correction of urethro-rectal] fistula due to diverticulitis of the sigmoid [correction of sigma] which resolved following reconstruction of the urinary tract by ureterocystostomy [correction of urethrocystotomy] using a modified version of Boari's technique, with satisfactory renal functional recovery. On the basis of an analysis of the literature, the authors comment on ureteral [correction of urethral] stenosis and its etiological diagnosis.

The diagnosis and treatment of abdominal actinomycosis is difficult. We report a case of chronic diverticulitis of the sigmoid colon with stricture formation and obstruction, which was complicated by intramural actinomycosis. We suggest that diverticular strictures and actinomycosis may coexist more often than the literature suggests.

Solitary colonic diverticula are rare. Most have been described in the cecum or ascending colon. Solitary diverticula of the transverse colon are extremely rare, and there are only a few reports in the English medical literature, all occurring in adulthood. This paper reports the case of a 13-year-old girl with a solitary, true diverticulum of the transverse colon, presenting as acute diverticulitis.

The contrast enema and CT studies in 102 patients with a clinical diagnosis (41%) or surgically confirmed (59%) diagnosis of colonic diverticulitis were reviewed retrospectively to determine the sensitivity of the two techniques. Combined results from all patients showed that the contrast enema was correct in 77% of patients. The contrast enema was falsely negative in 15% and was indeterminate in 7%. The CT examination was diagnostic in 41%, consistent with the diagnosis of diverticulitis in 38%, and falsely negative in 21% of patients. Both CT and contrast enemas were more accurate in patient…

The one-stage intracolonic bypass procedure prevents gastrointestinal secretions and fecal content from coming in contact with an anastomoti … loading, gross dehiscences, or fecal peritonitis. This report presents 28 patients with perforated diverticulitis, all of whom were treated by one-stage intracolonic bypass procedures. Ten of the 28 patients had peritonitis, and 18 had pericolic abscesses. Results indicate no deaths and no anastomotic leakages. Three patients (10.7%) had a complicated postopera …

The case history of a patient with subcutaneous emphysema of the abdomen from sigmoid diverticulitis is presented. The mechanisms for developing abdominal wall emphysema are reviewed. Thus when the gas originates from the gastrointestinal tract, mechanical factors are mainly responsible for this phenomenon. These inculde a direct communication through a colonocutaneous fistula. Gas may, however, also be spread into the abdominal wall by high intraintestinal pressures. Infection with gas-producing organisms may be associated with subcutaneous emphysema. This is particularly true when the emphys…

INTRODUCTION: Colovesical fistula is a recognized complication of diverticulitis, but progression to testicular ischaemia is, to our knowledge, an unreported phenomenon. While colovesical fistulas occur in diverticular disease, the spread of infection to the testes is rare.
CASE PRESENTATION: A 47-year-old male with recurrent diverticulitis presented with abdominal pain and was found to have a pelvic abscess. Despite conservative management, the patient developed pneumaturia, dysuria, and right testicular pain. Preoperative scrotal ultrasonography with color Doppler demonstrated globally redu…

Small bowel obstruction (SBO) secondary to enterolithiasis in the setting of small bowel diverticulosis is rare, and diagnosis can be challenging. Enteroliths may cause complications including diverticulitis, perforation, and obstruction, typically requiring surgical removal. An 83-year-old woman presented with abdominal pain, vomiting, and constipation. CT imaging demonstrated an intraluminal lesion at the duodenojejunal junction adjacent to an inflamed duodenal diverticulum, consistent with diverticulitis and partial SBO. Initial conservative management included nasogastric decompression, in…

BACKGROUND: Distribution of the colonic diverticula differs in different populations, and right-sided colon diverticulitis (RCD) and left-sided colon diverticulitis (LCD) manifest distinct clinical features. Complicated diverticulitis (CD) mostly requires hospitalization and can be treated within a spectrum from observation to surgery. Treatment choice is formed depending on the patient's general condition, the presence of diffuse peritonitis, the localization of diverticulitis, Hinchey stage, and responsiveness to the prior treatment. Clinical disparities regarding right and LCD also led to t…

BACKGROUND: Acute colonic diverticulitis (ACD) is the most common complication of colonic diverticulosis. Body composition, that is, proportion, distribution and quality of muscle and adipose tissues may play a relevant role in ACD. Previously, only few reports with small number of patients analysed the prognostic role of body composition in ACD. Our purpose was to analyse associations between the occurrence of complications and parameters of body composition in patients with ACD in a large sample.
METHODS: This retrospective study included 646 patients with ACD. The duration of hospital stay…

BACKGROUND: Laparoscopic lavage as a treatment for perforated diverticulitis, Hinchey III, has been found safe and feasible in randomized trials. A few studies have reported functional outcomes and quality of life as secondary outcomes. This study investigated distress associated with dysfunction of the bowel or stoma, functional outcomes, and quality of life 2-3 years after surgery in a national unselected cohort.
METHODS: All patients in Sweden who underwent emergency surgery for perforated diverticulitis with purulent peritonitis (2016-2018) were invited to answer a comprehensive, study-sp…

Colo-salpingeal fistulae due to acute diverticulitis are rare entities. A 65-year-old female with a 5-day history of left iliac fossa pain, fevers and diarrhoea was seen in a metropolitan hospital. Initial computed tomography of the abdomen and pelvis characterized a left tubular adnexal structure with an air fluid level and normal-appearing colon and was reported as pyosalpingitis. Worsening sepsis despite antibiotics prompted further imaging where thickening of the sigmoid colon was noted, and the provisional diagnosis was revised to complicated acute diverticulitis with perforation into the…
